Date of last inspection 8th August 2006 Brief Description of the Service: Ashcroft Hollow is a purpose built 45-bedded Care Home offering 24 hour nursing care. The home is located on the edge of Cannock Town set back from the main road / bus route. Registered Nurses and a team of fully trained care staff deliver care to the individual service users. Specialist equipment is available as required. Each service users has the benefit of en-suite w c facilities in their bedrooms. There are 4 separate day / quiet rooms and a dining room. Facilities are available for one rapid response service user. Community support services can be arranged as desired. Each service user has a named nurse and a key worker. The staff at the home is trained to care for service users requiring palliative care. The home has successfully received the Investors in People Award and the 1st home in the country to receive the Matrix award, which are both still in place and current. Fees range from £ 379 to £ 650 Ashcroft Hollow Nursing Home DS0000022307.V347773.R01.S.doc Version 5.2 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced inspection took place on 7th November by the lead inspector. The Manager was in the home and she and the administrator assisted the inspector. The provider Mrs Roberts and her son Mr Roberts were also in the home. The inspection included the following elements; A tour of the building, Observation and inspection of records relating to provision of care, Discussions with several service users, Discussions with several of the staff members on duty, Observation and sampling of other services provided such as catering and laundry, and an inspection of the managerial aspects such as staffing issues, training, recruitment and health & safety. The Inspector was made very welcome in the home and all assistance was given to gain the evidence required for the report. No complaints had been dealt with by Commission for Social Care Inspection since the last inspection. Service Users spoken to at the visit were complimentary about the home. One lady said, “She loved being in the home and the staff were like her family.” Another lady said, “The staff and activity gentleman kept them active, and she enjoyed the quizzes and games.” What the service does well: The manager and her staff provide excellent care in a warm and homely environment. The team of dedicated staff are highly motivated, well trained and compassionate to the individual needs of the service users. Many of the staff has worked at the home 8 to 10 years and they are proud of the care and attention they deliver during the 24hr day. Many of the staff was keen to chat to the inspector about how much they loved the service users and there was strong evidence that the staff relate well to the service users. The whole team of staff including the cook, laundress and domestics work extremely well together. The cook recently won an award from the Environmental Health Officer for the way in which she manages the kitchen. The home was exceptionally clean and tidy in all areas and hygiene standards were exemplary. Décor and fixtures and fittings were all in good condition. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. This judgement has been made using available evidence including a visit to this service. The manager and her staff spend a significant amount of time and effort making admissions to the home personal and well managed. Prospective service users and their families are treated with dignity, respect and understanding for the life changing decisions they need to make. There is a high value on responding to individual needs for information, reassurance and support. EVIDENCE: The management team at Ashcroft Hollow show substantial strengths and a sustained track record of delivering good performance and managing improvement. The service considers carefully the needs assessment for each prospective service user before agreeing admission to the home. This enables people to experience the home and what it has to offer. The manager on occasions will visit the prospective service user and their family to get to know them and answer questions. Clear information about contracts/terms and conditions, fees and extra charges is available in a format appropriate to the individual service users, and their family’s, capacity and language. Staff spend time with prospective service users to ensure they understand the terms and conditions of the placement. They are regularly reviewed and kept up to date, involving service users, their families and their representatives. All new service users receive a full comprehensive needs assessment before admission and staff are qualified and skilled to meet the specialist needs of a prospective service user. The staff are well informed by the manager about new service users and their individual care needs. Each care plan reflects the needs of the individual taking into account their cultural, religious and social preferences where this is appropriate. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. This judgement has been made using available evidence including a visit to this service. Service users individual health, personal and social care needs are set out in a care plan and fully met. Service users are assessed with regard to medication administration. Service users are treated with respect and their right to privacy upheld at all times. EVIDENCE: The management at the home have an ethos of involving service users in all aspects of their life. All service users have a personalised and detailed care plan. The home has effective systems in place to ensure the care plan is reviewed and updated monthly and arranges additional reviews when changes take place. Robust quality monitoring systems were evident. The care plan is used as a working tool and is understood by all staff. It is written in clear language. Each care plan includes a comprehensive risk assessment. Management of risk takes into account the needs of service users with their aspirations for independence and choice. The staff keep are up dated with training relevant to their role within the home. Records show that the home arranges for health professionals to visit frail residents in the home and provides facilities to carry out treatment. Staff keep a regular check on health aids, making sure they are working effectively and that each service users has the necessary aids to improve their quality of life Service users have choice over their personal care and are encouraged to be independent and responsible for their own personal hygiene where possible. The home operates efficient medication policy, procedure and practice guidance. Staff understand their role and responsibilities. Quality assurance systems confirm that policy is put into practice. Medication records are monitored as part of the individuals’ health care. The home promotes service users independence and those individuals assessed as being able are encouraged to take their own medication. The need to respect service users’ privacy and dignity when delivering health and personal care is a key part of the homes aims and objectives. The staff is fully aware that this also applies to all areas of the service users life. The registered manager routinely observes staff attitude and approach to privacy and respect and constantly seeks and values service users’ views and experiences. The wishes of individual service users about dying and terminal care, and the arrangements they want after death is openly and sensitively discussed with both the service user and their family during the development of the care plan. These are clearly recorded, respected and known to the staff delivering the care. The home has a detailed policy, procedure and practice guidance to help staff when handling terminal care and death. All staff receives in house training and practical advice in caring for these service users, and has continuous support and opportunities to discuss any areas of anxiety and concern. Facilities within the home are provided to allow relatives and friends to assist with their care if the service users want this. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12 to 15 Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Service users lifestyles do match their expectations; they maintain contact with family and friends and exercise choice as preferred. Service users receive a wholesome, appealing and balanced diet of their choice. EVIDENCE: Service users are now able to enjoy a full and stimulating life style with a variety of options to choose from. The home has sought the views of service users and has recently employed a new activity organiser that has considered their varied interests and abilities when planning the routines of daily living and arranging activities. Routines are very flexible and service users can make choices in major areas of their life. The routines, activities and plans are service users focussed, regularly reviewed, and can be quickly changed to meet individual residents needs. The activities coordinator is responsible for creating meaningful activities and experiences both in the home and the wider community. The inspector evidenced a ‘quiz’ taking place during the visit and some service users were really joining in and others were listening. Service users are encouraged to keep in contact with family and friends and visitors are welcome at any time and facilities are available for them to have a drink with the Ashcroft Hollow Nursing Home DS0000022307.V347773.R01.S.doc Version 5.2 Page 13 service user. Service users can choose to see visitors in their own room, a lounge or the garden areas. The home records and actions any restrictions, which the resident may make on who they wish to visit them. The staff team give service users the opportunity and support to remain independent. They are encouraged to be responsible for their own money for as long as they wish, and are able to maintain their independence. Staff will give help when it is needed and they have encouraged contacting advocacy groups in the past for some service users. The home’s policies, procedure, guidance and quality assurance systems ensure that service users are protected from financial abuse. The service has clear guidelines with regard to rights of service users to be able to read their records and staff spend time with them making sure that they are fully aware of the information which the home keeps. The manager discussed with the inspector that she feels food is highly important and meal times are a social occasion. Service users appreciate the good quality food they are served and being able to have a drink when they wish and the availability of snacks. They also enjoy being able to eat in their own room when they wish. The cook in the home is qualified and experienced in cooking for older people, is an important member of the care team and is well aware of the recorded dietary and cultural needs of each service user. The cook is committed to involving service users in menu planning and making sure that they are able to enjoy the food they prefer and like. The menu is varied, balanced and nutritious. The menu includes a variety of dishes that encourage service users to try new and sometimes unfamiliar food. The home have recently been issued 5 stars by the Environmental Health Officer following their annual inspection. Staff give assistance to those service users who have swallowing or chewing difficulty, or to those service users who need help to eat, they are discrete and sensitive. The mealtime was evidenced to be relaxed, staff were patient and helpful, and allow service users the time they needed to finish their meal comfortably, encouraging them to feed themselves. Service users and their relatives are confident that their complaints are listened to, legal rights are protected and that they are protected from abuse of any kind. EVIDENCE: Ashcroft Hollow has a highly developed, clear complaints procedure that highlights the importance of complaining or making suggestions for improvement. The complaints procedure is very widely distributed, and is highly visible within the service and local community. Service users and others associated with the service demonstrate a very clear understanding of how to make a complaint and they are fully aware of what can be expected to happen if a complaint is made, including the response time. Outcomes of complaints are managed very effectively with sensitivity. The service can evidence that it has learnt from the process, and the same issues do not reoccur. Service users and others associated with the home stated to the inspector that they are extremely satisfied with the service provision, feel safe and well supported by the professional management team. The policies and procedures regarding protection of service users are of good quality. Ashcroft Hollow Nursing Home DS0000022307.V347773.R01.S.doc Version 5.2 Page 15 The manager of the home ensures through training, supervision, review and quality monitoring that care staff fully complies with the policies and procedures provided in relation to protecting and safeguarding the rights of the service users. Ashcroft hollows principle aims and objectives include the promotion of the individual’s right to live an ordinary independent life when ever possible and to enjoy the rights and responsibilities of citizenship. The home enables all service users to enjoy a quality lifestyle, which includes being able to attend places of religion. The service has effective arrangements in place to make sure all residents have the opportunity to vote in elections. No complaints had been received at the home or by the Commission For Social Care Inspection and no safeguarding alerts had been recorded with regard to abuse. This judgement has been made using available evidence including a visit to this service. The service users live in a well-maintained, safe and comfortable environment. The home is very clean and hygienic. EVIDENCE: The management and staff at Ashcroft Hollow encourage service users to see the home as their own home. It provides a very well maintained, safe, comfortable, attractive home, which has all the specialist equipment and adaptations needed to meet individual service users needs. As well as a good selection of general aids such as hoists and variable height beds, the home also ensures that equipment is individualised for each service user and all staff members are trained in the safe use of aids and equipment. There is evidence that the home meets the changing needs of all service users, and especially where they have specialist care needs. The layout and design of the home is suitable to meet the specific needs of the people who live there. The fixtures and fittings are of a high quality, clean, well maintained and adapted to meet the needs of the individual service users. There is a selection of very pleasant communal areas, giving the service users a choice of a place to sit quietly, meet with family and friends or be actively engaged with other service users. Service users told the inspector that they can bathe whenever they wish and the temperature in the home can be changed to meet their personal choice, especially in their own rooms. Ashcroft Hollow was well lit, clean and tidy and smelt fresh. The management and domestic staff have a proactive infection control policy and they work closely with their own staff to ensure that infections are minimised. Clinical waste is properly managed and stored. Call bells were seen left within reach of service users and these are responded to promptly. The management monitors the effective function of the system. This judgement has been made using available evidence including a visit to this service. Service users needs are in safe hands at all times and their needs are met by the number of well trained staff and the skill mix working in the home. The staff is competent to undertake their role in the home and this is a result of the robust recruitment policy and practice in the home. EVIDENCE: Ashcroft Hollow has a well-developed robust recruitment policy and procedure that has the needs of service users at its forefront. The manager sees the recruitment of professional nurses and good quality carers is as integral to the delivery of an excellent care service. The manager maintains extremely high standards and expectations of her staff, with the recruitment of the right person for the job being more important than the filling the vacancy. The management team prioritise the training and facilitate all the staff members to undertake external qualifications beyond the basic requirements. In house training and formal training are part of an ongoing training plan. The manager encourages staff to share skills and knowledge with colleagues. She defines the roles and responsibilities of staff through accurate job descriptions and specifications. One senior carer is the training officer and has taken the role to a new level with her ongoing commitment and professionalism. Service users told the inspector that the staff are very sensitive to their needs. The home does not use agency staff at all. The staffing levels were appropriate to the dependencies of the current service users, and the manager constantly monitors the dependencies to ensure that sufficient staff are working in the home to meet the service users needs. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. This judgement has been made using available evidence including a visit to this service. The service users live in a home that is run by a highly professional and caring manager. The ethos, leadership and management of the home are exemplary. The home is run in the best interests of the service users and their financial interests are safeguarded. The health, safety and welfare of service users and staff are promoted and protected. EVIDENCE: The registered manager has the required qualifications and experience required by the Commission For Social Care Inspection; she is competent to run the home and openly meets the company’s aims and objectives. The manager competently manages the service demonstrating efficiently the provision of value for money. The manager is visionary in their approach to the service and is Ashcroft Hollow Nursing Home DS0000022307.V347773.R01.S.doc Version 5.2 Page 21 able to demonstrate through formal qualifications, and/or experience and ability that they are highly competent in the following areas: care of older people, conditions and diseases associated with old age, quality assurance systems, equal opportunity issues, development and implementation of the services policies and procedures, good people skills, strong leadership of staff which leads to confident workers, responds to need and provides an excellent role model. Other professionals know the manager consistently provides high quality services. The manager ensures that staff follows the policies and procedures. Staff are issued practice handbooks and easy access to all documents, which are discussed during supervision, staff training and team meetings. Spot checks and quality monitoring systems provide management evidence that practice reflects the homes policies and procedures. The registered persons are very committed to ensuring the health, welfare and safety of residents and staff. Records are clearly written and up dated; the quality assurance system confirms that the findings from risk assessments have been actioned. There is strong evidence that the ethos of the home is open and transparent with the views of both staff and residents listened to, and valued. There is a detailed business and financial plan and the home has the regular support of a qualified accountant and business advisor. The insurance cover in place ensures that the home is well able to fully meet any loss or legal liabilities. The home actively encourages service users who wish and are capable, to manage their own money and valuables, making sure they have the facilities to do this safely. The home has very efficient systems to ensure effective safeguarding and management of service users money including records keeping. Service users have access to their records whenever they wish.
