This inspection was carried out on 20th June 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Ashgrove provide a high standard of care to individuals. This was clearly communicated by all of the service users who spoke to the inspector during the visit. The home has developed good links with health care professionals in the area and seeks appropriate professional advise as and when required. The care team support individuals to maintain their independence where possible and clearly promote choice in daily living activities. The menu offers a varied well balanced diet, which takes into account individual`s likes and dislikes. The standard of food was described to be `excellent` by service users.

What has improved since the last inspection?

There were no requirements made following the last inspection. This is a home that operates well and remains committed to providing a high standard of care which is linked closely to the National Minimum Standards for older people. The homes management and staff are continuously monitoring and reviewing the service and strive hard to maintain a well run home.

What the care home could do better:

There were no requirements made at this inspection. As already stated the management and staff continue to work hard in meeting the requirements expected of them to deliver a personal service to all living at the home.

The registered proprietors are Mr and Mrs B W Case who have been at the home since 1987 and live in the converted coach house on the same site. Ashgrove is partly an old vicarage and partly purpose built extension, located in the small village of St Martins, Nr Oswestry, accommodation is all on ground floor level for service users, and is within easy reach of local resources. It is set out in its own grounds with pleasant gardens and a terrace for residents to use.There is a large garden where home grown produce is cultivated. Visitors to the home are frequent. The home has good links with the local community. The inspection was unannounced and started at 10.30am and lasted 2 hours. All 10 places were occupied. The inspection included discussion with service users and staff. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 All new service users are assessed before moving into the home to ensure their needs can be appropriately met. EVIDENCE: A relatively new service user told the inspector they were seen by the proprietors before moving into the home. A comprehensive assessment for the individual was provided to the home by the service user’s social worker. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 & 9 There is a clear care planning process in place. The staff are sensitive to the needs of each service user and meet these in a professional manner. Medication is dealt with in accordance with the homes policies and procedures. EVIDENCE: Evidence of individual care plans were seen at the inspection. These were seen to be reviewed regularly. Written evidence and discussions with service users informed the inspector that service users’ health care needs are fully met. All records relating to the receipt, administration and disposals of medication were satisfactory. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 14 & 15 Individuals are encouraged to exercise choice in relation to daily living. Service users are offered a varied well balanced diet, which takes into account individual’s likes and dislikes along with seasonal changes. EVIDENCE: Service users were happy to share with the inspector their experiences of daily living at the home. This indicated that individual choices are promoted and encouraged with service users. All service users were very complimentary about the standard and choice of food provided. It was apparent that the menu is changed to incorporate seasonal changes. A menu is recorded to support this. JUDGEMENT – we looked at outcomes for standard(s) 16 There is a clear complaints procedure in place. EVIDENCE: It was confirmed by the deputy manager that she was not aware of any complaints since the last inspection. Service users were complimentary about the staff group listening and dealing with any issues that they raise with them in a satisfactory manner. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20 & 24 The home is well maintained and comfortable both indoors and outdoors and offers accessible grounds and seating for service users. Individual accommodation is furnished and equipped to a high standard. EVIDENCE: Observations made during the home tour showed the home to be wellmaintained and comfortable with appropriate furnishings and lighting. Bedrooms seen on the home tour were personalised with individuals’ own possessions giving each room a unique identity of the individual occupying it. JUDGEMENT – we looked at outcomes for standard(s) 27 The home is staffed to a level that ensures service user needs are met at all times. EVIDENCE: Staff seen and discussion with the deputy manager confirmed staffing levels are appropriate to the number and needs of the service users living at the home. A duty rota is maintained in the homes diary, which confirmed observations and discussions. JUDGEMENT – we looked at outcomes for standard(s) 35 Service users are encouraged to handle their own finances. When this is not possible support from family or a representative is sought. EVIDENCE: Discussion with the deputy manager confirmed that the home does not deal with any individuals’ finances. Discussion with two services users confirmed the home are not involved in dealing with their finances.
