This inspection was carried out on 11th October 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The Manager/Provider felt that as no staff were employed that there was no loss of communication and therefore tasks were carried out efficiently and effectively. The home is run as a `family home` with full participation from the Residents. The home offers a good consistency of lifestyle, 2 of the residents continue to attend the Crawford Day Centre for 5 days a week and the third Resident for 3 days and they continue to work at a Chemist on Mondays and Fridays. The Residents have a good social life and are out most weekends during the day in Sheerness and using public transport on most occasions.

What has improved since the last inspection?

Since the last Inspection all 3 of the Residents have attended their Individual reviews and the care plans and risk assessments have been updated in line with any agreed changes and targets. The Resident who self medicates now has a lockable facility to store their medicine to control blood pressure safely. The Manager/Provider is now making a determined effort to complete the Registered Managers Award and is looking to attend an Adult Protection training course in the near future. It was anticipated that some new flooring in the kitchen and utility room would be purchased during the summer, however due to personal circumstances this has been delayed and will now be carried out during the winter.

What the care home could do better:

It would appear that the Residents live their lives in the way that they want whilst still being in a safe and caring environment.

Residents receiving care will be over 19 years of age Date of last inspection 21st June 2005 Brief Description of the Service: Avalon is a Home for three adults with a learning disability. The Residents have their own room and live very much as part of the family in a large family home. The home itself is very near the coast in Minster on The Isle of Sheppy. The Residents are within easy reach of Sheerness, which affords access to local shops. Transport is by bus or taxi, there is a main line railway station in Sheerness. The home is located on an unmade road which local residents do their best to keep reasonable. The Home has a large well-maintained garden, which is enjoyed by all the Residents. The home has several cats and nine chickens; the Residents enjoy having them around and are involved in their care. Graham Cummings carried out the Unannounced Inspection on the 11th October 2005 arriving at 13:10, there was nobody at home and the Inspector waited and left at 14:25. The Inspector returned on the following day,12th October 2005 at 13:40 and left at 15:50 The Inspector looked through records and spoke to the Manager/Provider. The Residents were all attending the Crawford Day Centre. The home continues to run well since the last inspection. The Residents have been living at Avalon for over 11 years, since Medway was split from Kent to become a unitary authority Residents contracts had not been renewed, this has now been addressed and contracts are being drawn up following reassessments of the individual placements, this has given the home more security. The home has just carried out a Resident survey regarding the care they receive, there were no adverse comments and all 3 of the Residents wish to remain at the home. The Inspector left the home with no concerns for the health, safety or welfare of the Residents. What the service does well: What has improved since the last inspection? Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 5 Residents have written contracts EVIDENCE: Since Medway split from Kent County Council to become a unitary authority, the contracts of the Residents had not been renewed, however following the persistence of the manager/Provider all of the Residents have, since the last inspection, been reassessed by the funding authority and they have now put contracts in place. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,8 Residents know that their changing needs are reflected in the care plan. Residents are consulted and participate in all aspects of life in the home. EVIDENCE: Residents have recently attended their individual reviews and participated in the writing of the new care plans drawn up from the outcome of the review. Residents have also participated in a quality review survey carried out by the Manager/Provider, there were no areas of complaint or dissatisfaction from the Residents. Residents are encouraged to participate in the daily running of the home and are responsible for carrying out household chores and collection of eggs from the chickens kept at the rear of the garden. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11,12,13,14,15,17 Residents have an opportunity for personal development. Residents take part in age appropriate activities. Residents are part of the local community. Residents have contact with family and friends. Residents have a healthy and nutritious diet EVIDENCE: Residents continue to attend the Crawford Day Centre in Sheerness, 2 of them attend 5 days and 1 attends for 3 days and works at a Chemist on Mondays and Fridays. The Residents go into Sheerness at weekends by themselves using local transport and have the option to go to a local disco run by Mencap on Friday nights. The Residents are known to their neighbours and the local community and often come back from their outings with updated local information. One Resident has overnight stays with a member of their family and the two others have contact as and when they wish. Residents are offered a healthy and nutritious diet. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 Residents receive personal support in the manner they prefer. Residents Physical and emotional needs are met. Residents who self medicate are protected by the homes policies and procedures. EVIDENCE: Residents had individual reviews and participated fully in the writing and updating of the care plans with their views and choices. Residents are registered with a local Doctor, Dentist and Optician of their choice in Sheerness. Since the last review lockable storage has been provided for the Resident who self medicates. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 23 Residents are protected from abuse, neglect and self harm EVIDENCE: The Manager/Provider is looking to attend an Adult Protection training course in the near future, they will contact some of the local care homes and training agencies to book a place. The Manager/Provider was informed of the new Adult Protection Protocols that have recently been produced by Kent County Council and advised to read them and ensure that the homes policies and procedures are updated to reflect any changes. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,30 Residents live in a homely and comfortable environment. The home is clean and hygienic. EVIDENCE: The home is spacious, clean, safe and free from any offensive odours with ample seating and separate areas for individuals to be by themselves apart from their bedroom. The garden has a seating area and at the end of the garden there is a small area sectioned off for a chicken coop. Residents participate in the collection of eggs and cleaning of the coop. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39,42,43 The home is well run and Residents have an input into the way this happens. Residents health, welfare and safety are promoted. Residents benefit from competent and accountable management of the service. EVIDENCE: The Residents have been living with the Providers for almost 11 years and the home is run as a normal ‘family home’. The Provider does need to complete the NVQ 4 Registered Managers Award, the Manager/Provider has assured the Inspector that this will be addressed as soon as possible on their return from holiday. The Inspector was not able to confirm with the Residents any details in the report as they were all at the Crawford Day Centre. The Inspector has no reason to believe that the Residents do not benefit from a well run home or that their rights and views are not considered in the development of the home. The Inspector felt that the Residents health, safety and welfare were promoted and protected and that the management of the home was competent.
