This inspection was carried out on 12th May 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

The home is run for the benefit of its service users and provides good quality care in a well-maintained and pleasant environment. Visitors were made welcome and relations in the home were relaxed and friendly. Service users stated that the staff were always friendly and got on well together. Service users said that they were always treated with dignity and respect and nothing was too much trouble for the staff. It was stated that the manager and staff are approachable and all service users spoken to praised the quality and choice of food available at the home. Staff stated that they enjoyed working at the home and that they were provided with regular training and updates in order for them to do their job effectively.

What has improved since the last inspection?

Since the last inspection re-decoration has continued within the home and a recommendation made at the last inspection for the home to keep records of possessions brought into the home by service users has been implemented.

What the care home could do better:

There was nothing major found on this inspection that required improvement and service users spoken to found this question hard to answer as they felt that the home was already providing a good quality service. 2 service users stated that they would benefit from more activities being provided. Service users should be more involved in the development of their individual plan of care as very few of the service users spoken to understand the care planningprocess and were unaware that they had a plan of care. Staff spoken to felt that staffing levels were sufficient to meet the needs of service users, however they did state that at times they could benefit from an extra staff member for the period that the evening meal was being prepared and served and this should be investigated.

The home is set within its own grounds and has a large car parking area at the front of the home with attractive flowered borders. The rear garden is wheelchair accessible and has a walkway with grab rails for service users, the garden is well maintained and mainly laid to lawn with trees shrubs and flowers. The home is registered with the Commission for Social Care Inspection for a maximum of 30 older people and older service users who have a physical disability. The inspection took place over 3.5 hours and was unannounced. On the day of the inspection the homes manager was unavailable and the inspector was assisted by the senior carer on duty and also by the home’s owner. The home is registered for 30 service users and on the day of the inspection there were 28 service users living at the home. Evidence for this report was obtained by speaking with 12 service users, 2 visitors, and 11 members of staff, the homes owner and from reading and inspecting records, touring the home and from observing the interaction between staff and service users. The recommendation made at the last inspection has been addressed by the home and there was 1 recommendation made as a result of this inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3, 4 & 5 Each service users has a needs assessment carried out before they move into the home and the home meets the needs of individual service users. Prospective service users, their relatives and friends are welcome to visit the home and assess the suitability of the home prior to moving in. EVIDENCE: The manager carries out an individual needs assessment prior to service users moving into the home and this was confirmed by service users. All service users spoken to felt that their individual needs were met by the home and staff was observed supporting service users appropriately. Some of the service users and one relative confirmed that they had the opportunity to visit the home prior to moving in to make sure that it was suitable. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 & 10 The health care needs of service users are met and service users health, personal and social care needs are set out in a plan of care, however there is a need for the home to inform service users about the care planning process, which would enable them to be more fully involved. Service users are treated with dignity and respect at all times and there right to privacy is upheld. EVIDENCE: Care plans were seen for a number of service users and these were clear and easy to follow and contained all relevant information. Service users spoken to stated that their needs were met by the home and comments such as “ I have never been treated so well” “I want for nothing” and “They do everything for me” were made, however when questioned about care plans very few of those service users spoken to were aware that they had a care plan and did not fully understand the care planning process and it is recommended that service users are given information about the care planning process to enable them to be more involved. Service users are registered with a number of different GP’s in the local area and dental checks are carried out at local surgeries. Staff was observed inter-reacting with service users and it was clear that service users were treated with dignity and respect and this was confirmed by those service users spoken to. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13, 14 & 15 Service users are able to keep contact with family and friends and visitors are always welcome. The home respects service user wishes on whom they do or do not wish to see. Service users exercise their own choice and have control over their own lives. Meals at the home are wholesome and appealing and provide a well balanced diet. EVIDENCE: There is a notice in the entrance hall at the home, which gives clear information on visiting, there are no restrictions but visitors are asked to respect meal times and all visitors are required to sign the visitor’s book. 2 visitors were spoken to during the inspection and they confirmed that they were always made to feel welcome and that there were no restrictions on visiting. Service users spoken to confirmed that they are able to exercise control and choice in their day-to-day lives and made their own decisions on what, if any activities they wished to take part in. The home keeps appropriate records about the food provided to service users and operates a four-week rolling menu, which is changed regularly. Staff was observed consulting residents about the choices available and all service users spoken to stated that the food was good and that portions were ample. Staff were observed supporting service users at meal times and one visitor regularly comes to lunch with her mother who is a resident at the home and she stated that the food was excellent. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 17 & 18 There is a simple, clear and accessible complaints procedure, which includes timescales for the process and any complaints are logged and responded to appropriately. Service users legal rights are promoted and protected and the home is committed to the protection of its service users EVIDENCE: Service users spoken to were confident about raising any concerns they may have and stated that they would address any complaint they may have to a staff member or to the homes manager. The home has a policy and procedure for dealing with any complaints and this contained all of the required information and gave details of how to contact the CSCI. Staff members spoken to were aware of the complaints procedure. There was information in the home with regard to legal advice and independent advocacy and all of the service users are on the electoral roll. Service users spoken to confirmed that they were able to exercise their right to vote at the recent general election. Staff have received training on adult protection and the home has a whistle blowing policy and also a copy of the Hampshire Adult Protection procedure. Staff spoken to were aware of their responsibilities in this area and knew what to do should they suspected any form of abuse had taken place. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 24 & 25 Service users have access to safe and comfortable indoor and outdoor communal facilities and service users private accommodation is furnished and equipped to ensure comfort, privacy and independence. The home is homely in appearance and decorated to a high standard. Individual bedrooms are safe and comfortable and service users can have their own possessions around them. EVIDENCE: A tour of the building was undertaken and this confirmed the above judgement. Communal areas were well lit and the lounge/dining area was well furnished. All of the service users individual rooms seen had been personalised and contained the required furniture and fittings, many had their own TV and telephone and service users confirmed that they were able to bring in their own possessions. JUDGEMENT – we looked at outcomes for standard(s) 33, 36 & 37 The home is run in the best interests of service users and service users benefit by the accounting and financial procedures of the home. Policies and procedures within the home safeguard service users rights and protect their interests EVIDENCE: Regular staff meetings are held and service users are consulted on a one to one basis. The home has a quality assurance system and service user surveys are carried out, there is a comments book in the entrance lobby to the home, which enables visitors to inform the management on how well it is meeting its aims and objectives. Staff members spoken to confirmed that they receive regular supervision and that annual appraisals are carried out. Records were kept secure and those seen were clear accurate and up to date and the home has policies and procedures in place.
