What the care home does well

The home provides a comfortable, safe and homely environment for people to live in. It is very well managed and organised. Residents have comprehensive care plans, and are fully consulted about things that affect their lives. The management team gain people`s views on the service by having regular residents` meetings and holding care reviews. Staff are well trained and supported by the registered manager and have a sound knowledge of residents` needs. The staff team work well together and have established a good working relationship with the community health care teams and local GPs.

What has improved since the last inspection?

The home has improved its care records to ensure that they meet the National Minimum Standards. The range of activities has been increased since the last inspection in January 2006. Staff training has been increased to ensure staff have the appropriate skills to protect and provide care for residents. Repairs have been carried out to paved areas in the garden. A new stair lift has been fitted.

What the care home could do better:

There was no recorded evidence within the home`s records to demonstrate that choice of food is being offered to residents.

Service Users have access to well maintained landscaped gardens and there are adequate parking facilities. The home is situated in the village of Barnby Moor. Located in a rural area the village has a public house and hotel. The home has access to a minibus and hired transport when necessary to access a wider range of amenities and promote social interactions beyond the immediate environment of the home. Barnby Court Care Home DS0000008626.V324926.R01.S.doc Version 5.2 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. The inspection took place over 5.5 hours. A tour of the premises was undertaken. The registered manager was provided with feedback at the end of the inspection. The main method of inspection used was called ‘case tracking’ which involved selecting three residents and tracking the care they receive through the checking of their records, discussion with them and the staff, and where more appropriate observation of interaction between staff and residents and related care practices. A sample of care records was inspected. Two members of staff were interviewed and the opinions of three residents were sought. A Pre- inspection questionnaire was supplied by the care home to the Commission for Social Care Inspection prior to the site visit being made. The registered manager makes available to all potential new residents a copy of the statement of purpose of the care home when they visit the home for the first time. A copy is displayed in the care home reception area. A copy of the last Commission for Social Care Inspection report is included in the statement of purpose. The home charges, £319 to £420 per week. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards1,2 3 & 6 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. A detailed assessment, which includes a risk assessment, is carried out before any new resident is admitted. An intermediate care service is not offered by the home. Residents are given in writing the cost of their stay at the care home. EVIDENCE: The registered manager stated the home has a statement of purpose, which is made available to the new residents. This sets out the resources of the care home. Residents who were being case tracked confirmed that they had written contracts and were able to state their contribution towards their stay at the care home. They all contained a detailed assessment including a risk assessment identifying the needs of the individual residents. This provided the essential information to enable a detailed care plan to be written. The registered manager stated that a dedicated intermediate care service is not provided by the care home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 7,8,9 & 10 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home identifies the health, personal and social care needs of each resident and records them on their care plan. All of the identified needs of residents are being met. The medication policy of the home is being followed. Staff, respect the dignity and privacy of residents. EVIDENCE: Individual residents’ care files were studied in detail to see if the information obtained at the initial assessment had been transferred into their care plan. Care plans set out the needs of the individual resident included social and care needs. If any risk had been identified the management of that risk was included in the care plan. Risk assessments stated what alternative action should be taken if problems still occurred. The health care needs of the residents were recorded on the care files. This included dental care, chiropody, eye care and any involvement by GPs, district nurses and consultants. Care files included a record of professional visits to residents. Staff stated care plans were informally reviewed weekly. The care home has a policy for self-medication. On the day of the site visit no residents were self-medicating. Lockable facilities are provided in bedrooms to enable residents to self-medicate. The registered manager stated that only trained staff, administer medication. She stated medication training is provided by an outside agency. The registered manager said, “I personally check the competency of all staff after training before they can give out medication”. Staff confirmed in formal interviews that medication training has been provided. A pharmacy visit has been made to the care home in December 2006; no requirements were identified. Resident spoken to stated that staff maintain their respect and dignity at all times. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12,13,14 &15 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Residents are encouraged to keep in contact with their family and friends. All visitors are made welcome by staff at the care home. Catering arrangements for the home do not reflects the residents’ choices, preferences. EVIDENCE: Residents stated that relatives and friends who come to visit them are made welcome. The registered manager described details of the activities provided for residents. Activities take place within the home and within the community. An activity organiser has been appointed and there is a possibility of her hours being increased in the future. Residents stated that they go out into the community for events. Residents stated, that they feel they have control over their individual lives. The registered manager provides details of the home’s menu. A requirement was made at the last inspection in January 2006 that the menu must demonstrate choice is being offered to residents. At the site visit the menu board displayed the wrong date and menu. Residents did not know what was being provided for lunch and they confirmed that no choice had been offered. There was no written evidence within care records that special dietary needs were being met. It was agreed with the registered manager that this requirement would be met within 48 hours. This judgement has been made using available evidence including a visit to this service. Standards 16 & 18 The home listens to residents’ views and wishes and acts on them. There are procedures in place to protect residents from any possible abuse. Staff have received appropriate training to protect residents from being harmed. EVIDENCE: Staff have been trained in the recognition and management of abuse. There are procedures and policies in place to ensure that appropriate action is taken should anyone report any suspicion of abuse to the registered manager or the proprietor. Residents state they feel safe living in the care home. The home has received no complaints since 2004.The Commission for Social Care Inspection has received no complaints. No investigations have been carried out in respect of a suspicion of abuse or poor care practice. The home has therefore acted appropriately to protect residents and ensure they live in a safe environment. The registered manager has acted to ensure that residents can raise concerns with any member of staff. These standards are therefore met. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 19 & 26 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents live in a comfortable, homely, clean environment with a choice of communal areas and personalised bedrooms. The infection control policy of the home is being followed. EVIDENCE: A tour was made of the care home, all areas were very clean and smelt fresh. There was evidence of ongoing maintenance being carried out. Residents spoken to confirmed their total satisfaction with the environment of the care home. The bedrooms viewed during this visit had furniture and fittings, which met the National Minimum Standards. Individual bedrooms have been personalised by the residents. Areas within the care home have been redecorated. A new stair lift has been fitted. Staff were observed to be wearing protective clothing when providing personal care. Staff stated that the home is a safe place in which to work. They confirmed that the infection control policy and the health and safety policy of the home is being followed. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 27,28,29 & 30 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home has sufficient staff on duty with appropriate skills to meet the needs of residents. The home has a robust recruitment policy, which protects residents. Training is being provided to staff, residents’ needs are being met. EVIDENCE: Residents confirmed there are always sufficient staff on duty to meet their needs. One resident stated “this home is first-class, staff will do anything they can to help you. I have no complaints, the manager is very approachable, all staff are good”. No negative comment were made by any resident, during the site visit. Residents stated that they felt safe living in the care home. They confirmed that care is provided in a safe and sensitive manner. The registered manager stated that she follows the care home’s recruitment procedure when employing new staff. The recruitment records for two new members of staff were seen and found to contain all the information required by the Care Home Regulations. Evidence was found that ongoing training including specialised training is being provided to staff. The registered manager described the training plan for the next 12 months. It was a requirement at the last inspection to produce a yearly training plan and ensure that all staff were trained in the identification Barnby Court Care Home DS0000008626.V324926.R01.S.doc Version 5.2 Page 18 and management of abuse. The registered manager confirmed this training has been provided to all staff. The staff stated that they were confident in being able to meet the needs of the residents. Staff described the reporting procedure to be followed if they had any suspicion of abuse taking place. They confirmed that they feel able to use the home’s whistle blowing procedure. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 31,33,35 &38 Quality in this outcome area is excellent This judgement has been made using available evidence including a visit to this service. The home has an experienced registered manager in post and leads the team effectively. Staff are competent and are provided with training and supervision. The recruitment policy of the home is being followed to ensure residents are protected. There are staff in sufficient numbers, with appropriate training on duty at all times. There is a comprehensive management audit system in place. EVIDENCE: The care home has a registered manager in post, who has successfully completed the registered manager course since the last inspection. The staff stated that she is very supportive and approachable. A resident stated “our manager is very good and easy to talk to”. The inspection of care records provided evidence of a comprehensive audit system being used by the company. A regional manager came to the home during the site visit and explained this system to the Inspector. There was evidence of a regional management audit on residents’ files seen during the site visit. Recommendations had been made where appropriate and evidence was found that the registered manager had acted upon recommendations. The registered manager described the policies and procedure to protect residents’ financial interest. Financial records have been completed in accordance with the companys policies and procedures. The company had issued terms and conditions to residents which included the contribution to their stay at the care home. There were no health and safety issues identified during site visit.
