Conditions of registration: 1. Service users in the LD category to be accommodated from the age of 35 and over 31 May 2005 Date of last inspection Brief Description of the Service: Barnett Lodge is a mid-terraced house with bedrooms on the first floor and a downstairs extension comprising of a kitchen and fourth bedroom. It is within walking distance of local amenities. Barnett Lodge is able to accommodate up to three service users in the learning disability category [LD, LD(E)] and is registered for 35 years and above. The registered persons are Mrs Lemmon and her daughter, Mrs Small. Service users would live as part of the family with Mrs Lemmon; whilst Mrs Small, offers support when required, for example a respite carer. Barnett Lodge currently does not have any residents. This is an overview of what the inspector found during the inspection. There were no requirements in the previous inspection report, dated 31 May 2005. However, as the home has not had any service users for two years and Mrs Lemmon had not had any formal training for some years, there was an expectation that, if the service was to continue, the Commission needed to be satisfied that she has the necessary training skills and expertise to manage adults with a learning disability. Since the last inspection the Commission wrote to Mrs Lemmon on 9 September 2005 to ask if a decision had been made regarding the future of Barnett Lodge continuing as a care home. The unannounced inspection took place between 10am and 12.30pm and time was spent during the inspection to further discuss the options regarding registration and Mrs Lemmon’s intention regarding admitting prospective service users, as well as applying to the adult placement scheme as a carer. Barnett Lodge is currently registered for adults with a learning disability, from the age of 35 years and upward. Mrs Lemmon stated that she would still like to continue caring and ideally would prefer providing a respite service for adults with a mild learning disability. Since the last inspection Mrs Lemmon has attended three courses on first aid, health and safety and challenging behaviour. The majority of the forty-three standards were not inspected on this occasion as the home is not accommodating any service users and there have been no admissions or discharges since 15 September 2003. In the past Mrs Lemmon has provided a secure and loving environment for adults with a mild learning disability, offering companionship and stability. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 4 and 5 The home’s admission procedure ensures that prospective residents have opportunities to visit the home. Relevant terms and conditions of residency are available. EVIDENCE: There have been no admissions since 2001 and the home has not had a service user since September 2003. Since the last inspection on 31 May 2005 the home received one enquiry about a vacancy but this was not for an immediate placement. Prospective service users would be given the opportunity to visit the home prior to admission. There would also be a protracted introductory process to ensure that the service user was suited to the home. However, Mrs Lemmon stated that she would prefer to provide a respite service. A discussion took place regarding the merits of adult placement and Mrs Lemmon said that she would look into the scheme. The home has a statement of purpose and service users guide. Mrs Lemmon previously charged a prospective resident at the Social Services rate for Portsmouth or Hampshire for adults with a learning disability. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 and 23 The home has a satisfactory complaints procedure and an adult protection procedure, to safeguard residents from abuse. EVIDENCE: The home has a complaints procedure, which is included in the statement of purpose. It was agreed that the procedure would be updated to change the name from the National Commission for Social Care to the Commission for Social Care and Inspection. There have been no complaints received by the home or the Commission. As stated elsewhere in this report, the home does not currently have any residents. Mrs Lemmon has a copy of the Hampshire adult protection policy and the document ‘No Secrets’ from the Department of Health. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24, 25, 26, 27, 28 and 30 A very good standard of accommodation is provided ensuring residents live in a homely, comfortable and safe environment. EVIDENCE: Barnett Lodge is a comfortable mid-terraced house near the shops in Stoke Road, Gosport. Since the last inspection a double glazed lounge window has been fitted to the front of the house. The home has three single bedrooms without en suite facilities. Two bedrooms are upstairs and the other is on the ground floor, situated in an extension, with a separate dining room, sink and cooker. However, one bedroom is below 10sq.m [which is the minimum set for a bedroom]. Mrs Lemmon’s office, which is a bedroom on the first floor, was measured to see if could be used as a service user’s bedroom. The room, although square, only measured 8.58sq.m. The home has a ground floor toilet. The bathroom is on the first floor. There is sufficient room in Barnett Lodge for up to two or three residents. JUDGEMENT – we looked at outcomes for the following standard(s): 40 and 41 Prospective service users rights and interests would be safeguarded and protected by the home’s policies and procedures. EVIDENCE: Mrs Lemmon has been running Barnett Lodge since 1966, with support from her daughter, Mrs Small. Neither Mrs Lemmon, as the main provider or Mrs Small, the other registered person, have any formal qualifications. Mrs Small due to family commitments is not able, as this stage, to enrol on the registered managers course and would, as part of the course work, need to have service users in the home, in order to complete her portfolio and modules. Since the last inspection, a follow up letter was sent to the registered persons regarding the need to have formal training in core subjects such as first aid, health and safety, manual handling, infection control, food hygiene, adult protection, medication training and updating their knowledge on relevant learning disability subjects, if the intention was to continue providing a care service. She is planning to continue with training courses in the core subjects. The home has the necessary records required as well as all the policies and procedures to effectively run a small care home.
