Conditions of registration: 1. All service users must be at least 35 years of age. Date of last inspection 28th September 2005 Brief Description of the Service: Barnett Lodge is a mid-terraced house with bedrooms on the first floor and a downstairs extension comprising of a self contained kitchen and fourth bedroom. It is within walking distance of local amenities. Barnett Lodge is able to accommodate up to three service users in the learning disability category [LD, LD(E)] and is registered for 35 years and above. The registered persons are Mrs Lemmon and her daughter, Mrs Small. Service users would live as part of the family with Mrs Lemmon; whilst Mrs Small, offers support when required, for example a respite carer. Barnett Lodge currently does not have any residents and has not done for three years. This has been the choice of Mrs Lemmon. The fees would be set at the local authority rates. This is an overview of what the inspector found during the inspection. This key inspection was announced as the provider/manager was not in when the inspector attempted to undertake an unannounced inspection. The provider/manager does not employ any staff and the home has not had any residents for three years. The inspector looked around the home, had discussions with the manager and looked at records and procedures. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1 and 2. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The home has information about the home in place which would benefit from being updated. The assessment process is based on the care management assessment process but would benefit from being clearer to ensure that needs of any new residents are met. EVIDENCE: The inspector asked the manager about the Statement of Purpose and the Service User Guide which would be provided to any new residents. The Statement of Purpose was not readily available but one was found. Both these documents required small amendments and the manager agreed to do this. The manager was unclear as to how she would undertake the assessment process, should she receive a referral, saying she had relied in the past on what was provided by the care manager. This apparently has led to some problems in the past. The manager is unlikely to take any private referrals and therefore unlikely to have to undertake a complete assessment herself. On further discussion, and searching, an assessment form was found which was suitable for the manager’s purpose, as long as it was accompanied by a full care management assessment. In the past, residents have visited the home, with their care manager and stayed for tea, and then maybe stayed for a week before deciding to move in. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The home would put careplans in place if any resident were to move in. EVIDENCE: There were no current care plans to look at as there are no residents living there. The manager did not use a particular format so the careplans from previous residents were looked at. Residents at Barnett Lodge tend to be low need and the careplans reflected this, although one would have benefited from more information. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 17. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. The home would provide comfortable everyday living, whereby residents could live as they please. EVIDENCE: The inspector was unable to assess outcomes for service users as there is currently no-one living at the home. The manager told the inspector how she used to discuss meal choices with residents, and one would sometimes prefer to buy their own meal from the shop sometimes. The manager has managed a vegetarian diet in the past and thought that she could manage most dietary requirements if she looked into it further. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. The home would be able to access healthcare provision. EVIDENCE: The manager feels that new residents could easily register with a local doctor. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Procedures are in place for the protection of adults but would benefit from being updated. EVIDENCE: The manager did have a complaints procedure but it did not include timescales and needed updating with regard to the name of the Commission. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 26 and 30. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Some areas of the home require redecoration or repair before residents could be accommodated in all bedrooms. EVIDENCE: Barnett Lodge is a mid-terraced house near the shops in Stoke Road, Gosport. The home has three single bedrooms without en suite facilities. Two bedrooms are upstairs and the other is on the ground floor, situated in an extension, with a separate kitchen area. However, one bedroom is below 10sq.m which is the minimum size for a bedroom. Communal areas include a kitchen/diner and sitting room. The bathroom is on the first floor. The home was found to be clean on the day of the visit. Mrs Lemmon has relevant information on infection control. The house was cold on the day of the inspection and Mrs Lemmon said the heating had broken. Someone was due to visit within a few days and would hopefully resolve the situation.

Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 37 and 42. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Whilst the manager has had some updated training since the home has had no residents, further attention is needed to fire safety before residents can move in. EVIDENCE: The manager has not updated any training since the last inspection, but previously attended three courses on first aid, health and safety and challenging behaviour. All cleaning fluids are kept in a lockable cupboard. There is a lockable box for medication which is locked to the wall. The manager has not recorded testing of smoke detectors since 2003, although she said she had been testing them occasionally. The fire extinguishers have not been professionally tested since 2003. The manager has not updated her own training in fire safety. Fire regulations have changed during the time the home has not had any residents and Mrs Lemmon must update herself on these. The
