The number of persons for whom accommodation is provided shall not exceed 44 (forty-four) in the following categories 30 Elderly Infirm clients of whom a maximum of 13 will be under the care of the Multidisciplinary Rehabilitation Team 14 Highly dependent, severely physically disabled clients Date of last inspection 16th May 2005 Brief Description of the Service: Barrington Lodge is a Nursing Home providing nursing care to a total of 44 older persons. 14 beds are on the ground floor; these may also be used for younger adults who have severe physical disabilities. Whilst this home provides care for people with physical disabilities it is primarily a nursing home for those of advanced years and who are very frail; so any service users admitted under the physical disability category need to be compatible with the main client group. The home can also accommodates up to 3 service users (within the two categories OP and PD) who are terminally ill and require palliative care. The home is not registered to provide care for people with Dementia nor any other form of mental health problems or learning disabilities. There are 33 bedrooms 11 of which are double rooms and 22 are single rooms. The home has two lounges; a large one adjacent to the dining area and a smaller quiet lounge for more private meetings with visitors. The main lounge leads out onto a raised patio with seating which in turn looks out over a large well-maintained garden that is accessible to service users. The dining room is accessed via the lounge through large sliding doors. The bedrooms are located on three floors, which can be accessed either by stairs or a passenger lift. A new passenger lift has been installed. There is limited parking for approximately 8 cars at the front of the building. The home has achieved an Investors in People award. This is an overview of what the inspector found during the inspection. This was an unannounced inspection conducted at midday in January 2006 to monitor the home’s progress in meeting previous requirements and to check the general well being of service users and the safe running of the home. A number of service users and their visiting relatives contributed most helpfully to the findings in this report and their contributions are acknowledged. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): The Statement of Purpose provides important information for prospective service users and detailed pre-admission assessments are in place for each service user - including details of their nursing needs. This ensures prospective service users will know their needs can be met. EVIDENCE: The key standard (3) in this section was not reassessed on this occasion as it was fully met on the previous visit. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8 11 The arrangements for care planning, the provision of health care and the procedures for dealing with medication are all satisfactory so as to ensure the social and health care of service users can be met in a private and sensitive manner. The home is clearly very respectful of service users’ final wishes so as to assure service users know that their last days will be as comfortable as their condition allows. EVIDENCE: Standards 7, 8, 9 and 10 were assessed as met at the previous inspection and there was every indication from the service users’ comments and in their care plans that these standards continue to be met in full. This was confirmed by checking a number of the services users’ case files. A standard system of documentation is use and was well organised and detailed. Relatives and residents state that the care here ‘cannot be faulted’. Standard 11, about supporting service users in their final days, was discussed in some detail on this occasion. The home has been selected to take part in a project to provide guidance and procedures for ensuring the home provides a personalised service for those who are dying. Particular emphasis is given to the wishes of the resident and family so that every detail of their care is thought through and recorded in some detail rather exchanging information

The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): The daily routines in this home are reasonably flexible within the constraints of a large service with many frail residents. Service users are being supported and encouraged to maintain links with family and friends and to exercise choice and control over their lives in so far as they wish and are able to do so. Service users are receiving a wholesome, appealing and well balanced diet in a congenial setting in accordance with their recorded requirements and preferences. So service users can be assured their daily lives are well catered for. EVIDENCE: Whilst all these standards were assessed as fully met during the previous inspection visit it was pleasing to note details of a special relationship that had blossomed between two residents - confirming that service users are still able to develop new and meaningful relationships within the care home and the staff team is commended for supporting residents to do so. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 The home has a clear and simple procedure for dealing with complaints so service users’ are confident their concerns will be dealt with promptly and effectively. To ensure vulnerable service users are safeguarded from abuse the home has written policies and procedures about the protection of service users and their property; this includes procedures for passing on concerns to the relevant authorities including the CSCI. EVIDENCE: These two key standards were assessed as fully met in the previous visit and no complaints, nor any suspicion of abuse, arose during the course of this inspection. Several relatives, as well as many of the service users, were interviewed during the inspection and they stated that the home is well run and any concerns they might have are dealt with sympathetically and effectively. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 26 The layout of the home and the manner in which it is generally being maintained means that this is a safe, comfortable and suitable environment for the service users. The premises are being kept clean, hygienic and are now free from offensive odours and systems are in place to control the spread of infection and safe guard service users . EVIDENCE: This two key standards were assessed as fully met on the previous occasion and a tour of the home was made to confirm satisfactory arrangements are in place to maintain the premises in a manner that will met service users’ needs including those who are frail and have physical or sensory difficulties. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 29 The number of staff employed and their skill mix are appropriate to the assessed needs of the current service users in this home so this ensures that their needs are being met. The home has a well developed staff induction, training and support regime in place to ensure they are competent in their jobs. Staff recruitment can generally assure Service users that they are in safe hands although the failure carry out all checks could potential compromise their safety. EVIDENCE: All standards with the exception of standard 28 have been assessed as fully met. It was clear from the high standards of care provided that service users are safe from abuse and neglect but one matter of recruitment had been overlooked during the recent Christmas break – that of the completion of all necessary checks before a member of staff is employed to look are residents. In one instance the police checks (CRB) had been applied for but the ‘Protection of Vulnerable Adults’ list (POVA) had not been checked as required to ensure staff are properly assessed before employment. The manager gave an undertaking not to allow the person concerned to continue any more shifts until that check has been completed. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 35 38 The registered manager is qualified, competent and experienced to meet the home’s stated aims and objectives. Quality monitoring is based the views of relatives, visitors and service users so as to ensure the home is being run in the best interests of the residents. Systems are in place to ensure that their property and money (held by the home) can be held securely. Provision is also made for service users to manage their own affairs if they wish. The home continues to protect the health, safety and welfare of service users. EVIDENCE: Inspection reports over a number of years indicate the skill and dedication with which this home is being managed. The service users attest to the quality of the services including personal care, catering and the comfort of the setting. Money records are in place and a sample of these records indicates they are well organised to protect the service users from financial abuse. A requirement was made to keep the record of accounts up to date and in the home itself; this is now in place. No hazards were identified.
