The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Provides a good standard of care and maintains the health, safety and well being of its residents. Provides a good training structure for staff and encourages them to progress by achieving qualifications at NVQ and in specialist subjects such as dementia care.

What has improved since the last inspection?

Staff morale is said to be improved following the disruption, which inevitably follows a period of substantial change. Whilst not necessarily improved, credit should be given when previously good standards in care practice are maintained.

What the care home could do better:

Some record keeping could be improved in terms of consistency; medication practice will continue to require development and monitoring on an ongoing basis. There are some relatively minor maintenance and housekeeping issues that will require some action in order to maintain a pleasant, safe and homely environment for residents and staff.

It is registered to provide personal care and accommodation for up to 40 older people, including those with dementia or physical disabilities and is owned and operated by Quantum Care Limited of 4 Silver Court, Watchmead, Welwyn Garden City, Hertfordshire, AL7 1TS. The home is organised as three units, including one of 16 beds for older people with dementia. Accommodation is provided over two floors, in single rooms, all of which are en-suite. There is a passenger lift and the home is accessible to whelchair users. The home benefits from attractive gardens to all sides, which provide a source of activity and enjoyment for residents and includes an extensive patio area. This unannounced inspection took place over five hours and provided an opportunity to meet the newly appointed manager and to discuss issues of mutual interest such as the registration process and how she sees Beane River View developing in the future. This is the second inspection of the year and where standards were assessed during the previous inspection of the 25th April 2005, they will not necessarily be commented on again unless there is cause to do so. The main focus of the inspection was discussions with the manager, residents and staff and a check on some environmental and housekeeping aspects of the home. This was a very satisfactory inspection, the care outcomes seen were good and this view was supported by the homes’ residents. ”We are very well looked after” was one comment made. There were very supportive and positive views expressed by both staff and residents about the new manager and the previous good standard of record keeping and care practice has been maintained. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,9 &10 The standard of care plans seen, including those for recently admitted residents remain generally good. This should provide staff with the information they need to be able to know what the resident’s needs are and what help they require. Residents spoken to were very positive both about the standard of care they receive and also the way that it is given. There were sufficient serious errors in some areas of medication to raise concern about the standard of monitoring. EVIDENCE: A sample was taken of Care plans to monitor content and the standard of recording, both of which were satisfactory. Staff and resident interaction was seen to be relaxed and respectful throughout the inspection. Medication records checked included some errors where medication was not being given as prescribed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,14 & 15 The standard of food was commented on generally very positively and residents meetings, held regularly, enable residents to express their point of view about food and any other issues that affect the way that the home is run and operated. Activities are continuing to be developed in line with the wishes and preferences of residents. EVIDENCE: Observation of meals on each unit during the inspection, looking at menu choices displayed and records of meetings held with residents and above all from talking with residents. Details of activities were discussed with the activities organiser, activity programmes were seen and records of events held were seen as well as being observed on the day of the inspection. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 & 26 In general the home is comfortable and provides a pleasant environment for those living and working there. The storage of linen, ancillary items etc was sometimes untidy and inappropriately sited, for example in bathrooms, and these could be rendered more pleasant for residents if surplus furniture was removed and surfaces tidied. Some areas need to be re-carpeted to recognise the increased use they now experience. EVIDENCE: Storage of some linen was untidy and presented some risk of infection. Bathrooms were cluttered and items such as incontinence pads were on display, whilst some furniture was stained and/or apparently unused. Carpets in two lounges, now used more following the designation of a dining/lounge as an exclusively dining room require upgrading as they are marked or stained. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 28 Staff numbers and skills appear adequate to meet the needs of the current residents of the home. EVIDENCE: The recruitment file of a recently appointed member of staff was looked at in detail and found to be satisfactory, with all the necessary checks and records completed. Staff spoken with during the inspection were positive about the availability of training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32 & 38 The newly appointed manager has the experience and ability to successfully take the home forward, and is being well supported by her management team and staff as well as by the residents. As the errors found in respect of medication were serious, if only few, Standard 38 cannot be assessed as fully met on this occasion. EVIDENCE: Those staff and residents spoken with during this inspection were positive about the efforts made by the new manager. Staff spoken to said that morale had improved following a period of necessary change, which, as was to be expected, proved challenging for those affected. NO STATUTORY REQUIREMENTS This section sets out the actions, which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards. The Registered Provider(s) must comply with the given timescales. No. 1 Standard OP9 Regulation 13(2) Requirement Medication administration practice and recording must be monitored to ensure that the medication prescribed is being correctly administered. Linen must be stored in a way that does not present a risk of infection. Bathrooms must be cleared of any surplus or excessive ancillary items and due care and attention given to the creation of an acceptable appearance and environment for residents using them.
