This inspection was carried out on 3rd January 2006.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Adequate. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector found there to be outstanding requirements from the previous inspection report. These are things the inspector asked to be changed, but found they had not done. The inspector also made 8 statutory requirements (actions the home must comply with) as a result of this inspection.

What the care home does well

All service users related they were happy living at the home. Service users interacted amiably like siblings. Observation and documentation evidenced positive staff interactions. Staff had built good relationships with service users and understood their individual needs. Service users are able to take part in age, peer and culturally appropriate activities. The home demonstrated that service users are attending relevant day centre facilities, college and social clubs. Service users engage in their local community by going shopping, to church, to the theatre, bank, on walks in the locality and accessing local amenities.The establishment was warm, clean and had a homely atmosphere. A large, brightly decorated Christmas tree was on display in the lounge and decorations throughout the home reflected the seasonal festivities. All service users were in good spirits and were very outgoing. Service users related that they had a good Christmas and showed the Inspector their presents. The home promotes service users independence and choice. Independent living skills are actively encouraged. Service users were observed during the Inspection exercising their individual choice and participating in their own daily routines which including baking. Service users responsibility for house keeping tasks is specified in their individual plan.

What has improved since the last inspection?

The home has been registered to provide another single room. Beechwood now caters for six service users and currently has a vacancy.

What the care home could do better:

During this Inspection eight legal requirements were highlighted and one recommendation. Examination of healthcare files evidenced one service user had not had a recent eye test. All service users must benefit from optician appointments at regular intervals. This requirement is repeated from the last Inspection. The home must ensure service users health needs are monitored and promoted. Medication administration records must be accurately maintained. Service users must be protected by the home`s medication procedures. Refrigerated foods must be date labelled when opened. This requirement is repeated from the last Inspection. Service users health must be protected by the home`s food hygiene practices.Service users must reside in a well-maintained homely environment. The chest of drawers handles must be replaced in CA`s room. The dressing table drawer must also be fixed in MS`s room. Areas within the home must be touched up with paint and redecorated were needed. Identified areas include the hallway entrance and lounge area. The Registered Provider must ensure appoint a suitably experienced and qualified Manager. Service users must benefit from the stability of an efficiently managed home. Weekly fire alarm tests must be undertaken. Service users must be protected by the home`s fire safety procedures. Staff should ensure service users birthdays are appropriately acknowledged and recorded. Service users best interests must be safeguarded by the home`s record keeping practices.

A private individual Mrs Pretim Singh owns the home. Service users benefit from residing in a warm, family atmosphere where 24hour care is provided. The home has four single rooms and one shared double room. All bedrooms are located upstairs. The home does not have a passenger lift; therefore it is unsuitable for service users with mobility needs. Service users have access to two lounges, which includes a dining area and a rear patio garden with garden furniture. Local amenities include a culturally diverse parade of local shops, post office and food outlets in Upton Lane. Several bus routes serve the home. The nearest underground station is Upton Park, on the District Line. The nearest British Rail station is Forest Gate. This Unannounced Inspection was undertaken on a Tuesday evening and lasted approximately four hours. The overall objective of this Inspection is to ensure service users are receiving the best possible care and their welfare is safeguarded and promoted at the care home. In addition to checking the home’s compliance with the legal requirements made at the last Inspection. The Inspector spoke to all service users currently living at the home. Individual discussions were also held with the two Support Workers on duty. Service users files, health records, daily logs, medication and medication administration records, fire safety records and other relevant documentation were examined. A tour of the premises was also undertaken. The Inspector would like to thank all service users and staff members for their assistance with this Inspection. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2&3 Beechwood Residential Home has the necessary policies and procedures, which assist prospective service users in deciding if the home can meet their needs. EVIDENCE: No new service users have moved into the home since the last Inspection. Service users’ files evidenced that service users were admitted to the home only after a full assessment had been undertaken. The assessment process involves the service user, their relatives and appropriate health and social care professionals. A pre-admission assessment is always completed by the home. The home has eligibility criteria and service users are only admitted into the home if their needs can be met. The eligibility clearly identifies the dependency levels the home can sufficiently care for. Where service users needs had increased the home evidenced that they had taken appropriate action. This included contacting Social Services for additional funding and support. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6, 7, 9 & 10 Service users individual needs and choices are promoted at Beechwood Residential Home. EVIDENCE: All service user files were examined during this Inspection. Service users assessed and changing needs are reflected in their individual care plan. The home evidenced that care plans were regularly reviewed. Staff demonstrated that assistance and support was given to service users to make decisions about their own lives. Examination of the home’s daily logs, and service users’ plans, evidenced that service users exercised choice in their daily lives. Service users’ individual choices were noted in their daily record. On the evening of the Inspection service users were seen making decisions and their wishes were being respected. Service users are supported to take risks as part of an independent lifestyle. All risk assessments have been updated as required from the last Inspection. The home has a confidentiality policy. Service users files are stored in locked cabinets in the Manager’s office. JUDGEMENT – we looked at outcomes for the following standard(s): 11 - 16 Service users lifestyle needs are individually promoted at Beechwood Residential Home. EVIDENCE: Beechwood Residential Home DS0000022830.V275503.R01.S.doc Version 5.1 Page 13 Service users’ files, observation and discussions with staff evidenced the opportunities service users have to fulfil their spiritual needs and have opportunities for self advocacy, independent skills and therapeutic interventions. Service users attend church on Sundays, attend a women’s group, music therapy and their rights are promoted through advocacy involvement. Service users are able to take part in age, peer and culturally appropriate activities. Service users attend various day centres, social clubs and participate in daily leisure activities. During the Christmas period service users attended the pantomime at Stratford Theatre and various Christmas parties. On the evening of the Inspection service users were observed listening to music, doing artwork, watching television, chatting with staff and relaxing in their rooms. All service users were in good spirits and were very outgoing. Service users related that they had a good Christmas and showed the Inspector their presents. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 - 21 Service users personal and health needs are generally promoted. All service users must benefit from regular optician appointments and improved medication administration practices. EVIDENCE: Service users receive personal support in the way they prefer and require. Times for getting up/going to bed, baths, meals and other activities are flexible and dependent on service users needs and preferences. Service users choose their own clothes and their overall appearance reflects their age and individual personality. All service users had their hair cut or styled for the Christmas period. Female service users had manicured nails and all service users were dressed in a modern fashion. Documentation evidenced that service users health is monitored and potential complications and problems are identified and dealt with at an early stage, including prompt referral to an appropriate specialist. Examination of healthcare files evidenced one service user had not had a recent eye test. All service users must benefit from optician appointments at regular intervals. This requirement is repeated from the last Inspection. The home must ensure service users health needs are monitored and promoted. Beechwood Residential Home DS0000022830.V275503.R01.S.doc Version 5.1 Page 15 The home has developed a comprehensive medication folder with various policies, including medication administration, security and disposal of medication. Medication is stored in a locked cabinet, which is stored in the staff office. Each service user has a medication profile that includes their photograph, medication details and allergies. The Inspector examined medication and records and one gap was found in the medication administration record. Medication administration records must be accurately maintained. This requirement is repeated from the last Inspection. The home has a Death and dying policy. Each service user had a record of wishes relating to growing older, illness and dying. . Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 23 Service users safety and welfare must be better promoted to minimise risk and safeguard them from abuse or harm. EVIDENCE: The home has produced a Complaints, Adult Protection, Whistle Blowing procedure, Absence without Authority policy and Aggression towards staff policy. A Management of Service Users Money, Financial Affairs and Acceptance of Gifts policy are also in place. The complaints procedure is also available in a pictorial format. The complaints book was examined and evidenced that there had been no complaints since 2001. Documentation relating to adult protection is available in the Manager’s office. All staff interviewed however were not clear on what should happen during an adult protection allegation. The Registered Manager must ensure all staff are aware of the Adult Protection procedure. Service users must be fully protected from abuse, neglect or self-harm. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24 - 30 Beechwood Residential Home provides service users with a homely environment. General maintenance however must be further addressed. EVIDENCE: Beechwood Residential home is located in a residential street in Forest Gate, in the London Borough of Newham. The premises is a terraced property which blends easily into the neighbourhood and presents as a family home. The home is comfortable, clean and free from offensive odours with sufficient and suitable heating and ventilation. This home is unsuitable for prospective service users with mobility needs as all bedrooms are on the first floor and second floor and a passenger lift is not available. Local amenities include a parade of local shops, post office and food outlets in Upton Lane. The home is in close proximity to the Green Street shopping area and Woodgrange Road, which has a range of culturally diverse shops, services and amenities. Stratford, Ilford and East Ham shopping areas are also easily accessible by public transport. Several bus routes serve the home. The nearest underground station is Upton Park, on the District Line. All bedrooms were inspected. Bedrooms were lockable with sufficient space for personal items. Service users can bring their own furniture and can decorate and personalise their rooms subject to fire and safety requirements. Service users’ bedrooms contained televisions, compact disc players, computers, family photographs, pictures, ornaments, collectible items, items made by service users and certificates of achievement. Service users have access to two lounges. The front communal lounge is comfortable and homely. Leather settees promote a contemporary atmosphere. Service users have access to a large television, a video and a music system. A range of compact discs, videos, books and games are also available. A large, brightly decorated Christmas tree was on display in the front lounge and wall decorations reflected the seasonal festivities. The rear lounge includes a dining area, comfortable seating, a music system and a exercise bike. Service users have their own notice board, which includes information in pictorial formats and Makaton signs. A utility room with laundry facilities adjoins the dining lounge. There is a choice of two bathrooms, one with shower facilities and the other having a deep bath. Bathrooms were attractively tiled and facilities were of a high standard. Service users have their own telephone, which is located in the ground floor hallway. Service users additionally benefit from a well-maintained patio garden with flowers and garden furniture. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31, 32, 33, 35 & 36 Beechwood Residential Home has sufficient staffing and service users benefit from an effective staff team. EVIDENCE: Staff evidenced that they knew and supported the main aims and values of the home. This included their ways of working, respective roles and responsibilities. Staff demonstrated an awareness of service users’ individual needs. Service users additionally evidenced staff’s ability to meet individual needs. Service users were observed interacting amiably with staff. Staff interacted in an equally, good-natured manner. Staff interviewed expressed their enjoyment in working at the home and commented on their wish to advocate and enhance service users lives. Staff National Vocational Qualification (NVQ) training is in progress. The staff members on duty reported that they were currently enrolled on respective NVQ 2 and NVQ3 training courses. An on-call system is in operation in the event of an emergency. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 37, 41 & 42 Service users welfare, health and safety must be better promoted through the home’s practices. EVIDENCE: A private individual Mrs Pretim Singh owns the Beechwood Residential Home. The Manager’s post is currently vacant due to the previously Acting Manager leaving. Service users and staff confirmed that the Registered Provider has been providing management cover and visits the home every day. The Registered Provider must appoint a suitably experienced and qualified Manager. Service users must benefit from the stability of an efficiently managed home. All service users files and daily record books were examined. It was noted that staff had not acknowledged a service users birthday in their daily record. Instead reference was made a week later when a party was held. Staff should ensure service users birthdays are appropriately acknowledged and recorded. Accidents and incident records are appropriately maintained. There have been no accidents since the last Inspection. More diligence must be undertaken in regards to fire safety and food hygiene. Examination of the fire safety records evidenced that the fire risk assessment has been reviewed. Gaps were noted in the weekly fire alarm tests between 7/8/05 to 21/8/05 and 2/10/05 to 22/10/05. The home must ensure the fire alarm is tested on a weekly basis. Service users must be protected by the home’s fire safety procedures. Refrigerated foods must be date labelled when opened.
