This inspection was carried out on 12th December 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The three relatives spoken to during the visit were positive about the care being delivered and those residents who were able to express their opinions were complementary about the staff and the way they are supported. The food being served during the visit was well received by the residents of whom a seven said that they enjoyed it. However although a number of the residents are not able to able to express their enjoyment verbally they were eating enthusiastically on the day of the visit.

What has improved since the last inspection?

It was a positive inspection with evidence of improvement since the last inspection being maintained. The Manager and staff have undertaken a significant amount work to ensure that they are able to demonstrate their compliance with the standards especially in relation to the training provided. There were six requirements identified during the last inspection and all but two have been met at or before this inspection.

What the care home could do better:

Although the proprietor visits the home on a very regularly written record of this must be prepared monthly. The storage area in the rear of the building, ground floor, must be cleared to allow for safe progress through the area.

The CSCI must be informed at that time so that this condition can be removed. 19th May 2005 Date of last inspection Brief Description of the Service: Belle Vue House is a purpose-built two-storey care home of traditional brick build and tiled roof construction. It is situated in the Heaton suburb of Newcastle upon Tyne. The home is well located with ease of access to the nearby Chillingham Road and its’ variety of public amenities including shops, cafeterias, schools, public houses, parks, churches and regular transport services. The home is registered to provide care with nursing to people over the age of 65 years who suffer from Dementia. This unannounced inspection was carried out over a period of 3 hours on one day by two inspectors followed by a visit by the lead inspector for 3 hours. This is the second unannounced inspection the home has had this year and all of the core standards have been examined over the inspections. Both reports should therefore be looked at for the full picture. During the inspection twelve residents, four relatives and six staff were spoken to. The records examined included, four care plans and medication records, training records and the records for complaints. The inspector also viewed staff files including evidence of their training and supervision. There were six requirements identified during the last inspection and four recommendations. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3, 6 A detailed assessment is undertaken by the senior staff of the home before any resident is admitted and the Manager ensures that the resident are able to be cared for in line with the homes registration. The home is not registered for and therefore does not provide intermediate care. EVIDENCE: Four care plans were examined and they each contained the necessary information to allow a judgement to be made as to their suitability for the home and that they could be cared for safely. The senior staff visit the residents prior to being accepted for placement and a document is completed which is then added to and form the initial information on which the care plan is developed. All of the care plans examined contained a pre-admission assessment. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 10 Care plans providing details of each resident’s needs are in place. Staff promote and maintain residents’ health and ensures access to health care services to meet assessed needs. The residents are treated with respect and are privacy is maintained as far as possible. EVIDENCE: The Manager, Mr Herrity has continued to maintain the improvements made to the care plan records, which describe the way in which care is being given to the residents in the home. Each resident has a detailed care plan, which shows a process of assessment, planning of care, evaluation and re-assessment. Those examined were completed effectively and were in sufficient detail to allow the care to be provided according to the nursing assessment. There are a variety of assessment tools, including moving and handling, dependency and assessment of the person’s skin and likelihood of developing pressure sores. Equipment was available to assist staff in preventing pressure sores and promoting continence. Belle Vue Nursing And Residential Home DS0000000393.V258091.R01.S.doc Version 5.0 Page 10 Records are in place that shows that health care professionals such as opticians and specialist nurses were consulted and visited both regularly and as needed. The residents in the home have a degree of dementia and as such their views regarding living in the home are difficult to obtain. However those spoken to who were able to describe their feelings describe staff as being helpful and assisting them to do things in a respectful and discreet way and example being “the staff are always nice and helpful”. Staff spoken to were aware of the need to maintain privacy and were acting in an appropriate way during the visit. The bedroom and toilet/bathrooms have locks in place to be used by staff when personal care is being delivered. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15 Service users lifestyles match their expectations and preferences in line with their social, cultural, religious, and recreational needs and interests. Service users are being helped to exercise choice and control in their lives. Arrangements for residents to maintain contact with their family and friends and the local community are suited to each individual’s needs and vary accordingly. Service users are receiving a wholesome balanced diet in pleasing surroundings at convenient times. EVIDENCE: Residents spoken to confirmed that they are encouraged to take control of their daily routines in simple but important ways including the time they get up, what and when they eat and how they spend their time. Gender of staff for personal care tasks are identified and met. A weekly activities programme for residents was seen and showed a fair range of events taking place although this is dependent on the daily preferences and changing needs of the residents. Records showed that some relatives were taking residents out for visits to the local community facilities. Menus demonstrated a good range and choice of food was being offered to the residents. Mealtimes are flexible according to Belle Vue Nursing And Residential Home DS0000000393.V258091.R01.S.doc Version 5.0 Page 12 individual needs. Food likes and dislikes are recorded and lists of these are kept by the homes cook. The resident’s weights are recorded and there was no evidence of a trend of weight loss. There is a protocol of action for those residents loosing weight including referral to the General Practitioner. The residents’ bedrooms were personalised reflecting individual choices and preferences and three residents asked about their bedrooms said they were happy with the decoration. Residents have visitors at any time and are able to use their own rooms, the small lounges or the larger, busier lounges to receive them. Two relatives spoken to on the day confirmed that they are welcomed to the home. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16, 18 The residents and their families have shown willingness to raise their complaints or concerns and there is evidence that they are taken seriously and acted upon. EVIDENCE: There is written guidance in place on dealing with complaints, which is available in a number of different areas including posted on the wall and in the service user guide. Two staff spoken to could describe the way in which a complaint could be made. The six complaints which have been made since the last inspection were examined and the records showed that these have been investigated and either resolved or the complainant chose not to pursue them further. Two relatives spoken to on the day of the visit confirmed that they felt satisfied that any concerns or complaints raised with the Manager or staff would be dealt with appropriately. Standard 18 was examined at the last inspection and was met however the planned Protection of Vulnerable Adults training has not yet been provided and therefore remains a recommendation. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 The home generally safe, and well maintained and is well provided with toilets and bath with the on-suit facilities and communal toilets. The bathing areas offer a choice between showers and baths. The bedroom areas are tidy and decorated in line with the resident choice. EVIDENCE: During the visit both of the inspectors toured the premises and noted that the décor of the home was good and odour free. Some areas have been recently redecorated and were pleasant and homely. A number of bedrooms were examined (at least 10) and were decorated and furnished in a pleasant and homely way. There was no odour present during the visit. It is recommended that an audit be requested from the Communicable Disease Nurse be arranged as a way of assisting in the future planning. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 30 The home is staffed with appropriate numbers of staff and there are qualified nurses on duty in sufficient numbers to meet the needs of the residents. The staff are recruited and selected using a system which ensures that they are able to care for the residents and are going to offer a safe place for them to live. The Manager is recruiting staff according to the home’s policies and the two staff files examined were complete. EVIDENCE: Staffing rotas showed that the Manager is ensuring that enough staff are on duty to meet the staffing levels set down prior to the change to the CSCI without reduction. It was noted that when sickness and staff holidays occur they are usually being covered by homes own staff. When this is not possible agency staff are being used, late reporting does occasionally result in fewer staff being on duty for short periods. Two staff records were examined and were complete including application forms records of interviews and the necessary checks including Criminal Records checks and two references. The manager has arranged a number of training sessions on a variety of subjects as well as statutory training. The staff confirmed that they are being encouraged and supported to attend. The training record for all of the staff was examined, there is evidence of a lot of training being provided and confirmed that all staff had received statutory training. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 35, 38 Resident’s personal finances are being managed appropriately. Generally the home is maintained in a safe condition and the staff are aware of health and safety issues however the room at the rear of the building remains cluttered and unsafe to be used. Evidence of the monthly visits by the proprietor or his representative are not being provided. Belle Vue Nursing And Residential Home DS0000000393.V258091.R01.S.doc Version 5.0 Page 17 EVIDENCE: The personal records kept in the home of residents who are receiving assistance to manage their finances are detailed, logical and appropriate. Receipts were in place for purchases made on behalf of residents and signatures of either two staff or one and the service user were in place, and there is a small float available for the staff to access. It is acknowledged that the proprietor makes regular and unannounced visits to the home however there remains an outstanding requirement for these to be recorded and made available to the CSCI. The storage area in the rear of the building ground floor, which is a fire exit, was tidier than at the last inspection however it remains cluttered and contained flammable materials. This must be cleared and made safe to be used by staff.
