Conditions of registration: 1. Age range is 18 years - 65 years. Date of last inspection 27th July 2005 Brief Description of the Service: Blisworth Close is a care home providing personal care and accommodation for 6 younger adults with a learning disability. The Premises are owned by a housing association with each service user having a tenancy agreement. Mencap provides the care service. The home is purpose built and is located on a new housing development on the outskirts of Northampton. The home is two storey with four bedrooms located on the ground floor. All of the bedrooms are single with en-suite facilities. There is a well-maintained and enclosed garden with water features and raised flower borders. This is an overview of what the inspector found during the inspection. The focus of inspections undertaken by the Commission for Social Care Inspection is based upon the outcomes for residents, and upon their views of the service provided. The primary method of inspection used is case tracking which on this occasion involved tracking the care of three residents that live at the home through, reviewing care plans, discussion with the service users and staff, and observing care practice where possible. Prior to the inspection taking place, the inspector spent forty-five minutes planning the areas to focus on at this inspection, based upon information gained from reviewing the homes service history and the previous inspection reports. The inspection took place in the morning over a period of approximately five hours. What the service does well:
Within the residents care plans there was needs assessments in place, which included specialist support required. The care plans are very detailed containing information on the residents physical, psychological, personal and emotional needs. There are detailed risk assessments on the identified hazards surrounding each resident within their day-to-day activities at work and at leisure. There are opportunities for residents to live fulfilling lifestyles both in and outside of the home and to maintain contact with family and friends. An exercise bike is available within the home and records kept within residents care plans on its use and the resident’s health and fitness progress. Within the care plans there was records of residents engaging in alternative relaxation therapies such as reflexology and aromatherapy. There is a clear and effective system in place for residents and their families, to voice their concerns and be confident that their complaints will be listened to and acted upon. Residents are supported by a consistent and effective staff team, that are aware of their needs. The home is run in the best interests of the residents to protect their health safety and welfare.

Written documentation within the care plans demonstrates that, the support workers know each resident very well and that the care provided is person centred and tailored to meet each residents preferred lifestyle and capabilities. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4 & 5 There is sufficient information made available to prospective residents, and their families, to enable them to make a decision as to whether the home can meet their needs and aspirations. EVIDENCE: Within the residents care plans there was needs assessments in place, which included specialist support required. The statement of purpose, service users guide and the most recent report from the Commission for Social Inspection is made available to all prospective and existing residents and their families. Information provided in the Statement of Purpose verified that the procedure for new admissions into the home includes, a full needs assessment and the prospective resident being invited to the home on several trial visits, this practice was confirmed through discussion with staff. There have been no new admissions since the last inspection. Written contracts of are in place that states the terms and conditions between the home and the resident. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,8,9 & 10 Residents are provided with opportunities to make everyday choices and are supported in living as independent a lifestyle as possible. EVIDENCE: The care plans were very detailed containing information on the residents physical, psychological, personal and emotional needs. There were records of annual reviews taking place involving the residents, their representatives and other professionals. There were no records available within the care plans to demonstrate that they are being regularly reviewed in between the annual reviews taking place. The care plans did not contain the signatures of residents or their representatives. Within the care plans detailed risk assessments had been completed on the identified hazards surrounding each resident within their day-to-day activities at work and at leisure. Several of the risk assessments did not have the signatures of the author, the resident or their representative and the frequency of their reviews was not always identified in the risk assessments seen.

All service users main files are kept secure within the office/sleep in room. The working care plans available for support workers are kept in a locked cupboard in the dining room. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 11,12,13,14,15,16 & 17 There are opportunities for residents to live fulfilling lifestyles both in and outside of the home and to maintain contact with family and friends. EVIDENCE: All the residents attend local day centres, which are appropriate to their needs and age, all enjoying rest days each week, where they can partake in activities of their choice, within their home and the local community. To assist with providing choice for residents on where to visit on organised outings there is a ‘Days Out’ file containing visual cues and written information/brochures on tourist attractions that offer educational and fun activities. The residents rights to experience appropriate, personal relationships, is recognised within the individual care plans and treated with sensitivity and respect. Records of communication with families and family contact were seen in the individual care plans and resident’s contacts records demonstrated visits to the

Records were retained of when residents had declined to go on outings, which confirmed that choice is respected. Records were seen of resident’s participation in physical exercise activities and swimming. An exercise bike is available within the home and records kept within residents care plans on its use and the resident’s health and fitness progress. Within the care plans there was records of residents engaging in alternative relaxation therapies such as reflexology and aromatherapy. Records were maintained within the care plans of resident’s weights and their nutritional needs. Some resident’s follow healthy eating plans, there is a five-week menu in place that is flexible and varied. There is a selection of Chinese, Indian and Italian take away menus available for residents, staff said that residents enjoy a take away meal at least once a week. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 & 21 The resident’s physical, emotional and healthcare needs are met. EVIDENCE: The care plans contained detailed information on the personal, physical and health care needs of residents. The personal and health care needs of the residents included specific risks assessments and the use of aids and adaptations to the home environment to minimise the risks. Within the care plans there was records of the involvement of a speech therapist, particularly in supporting the staff in caring for residents who have eating, drinking and swallowing difficulties. On the day of inspection all the residents had left the home, with the exception of one resident remaining who was awaiting the transport to take them to the day centre, the staff were observed treating the resident with dignity and respect, there was a homely atmosphere within the home, the resident was relaxed and spoke highly of the care received at the home, saying that they liked there room and that the staff were very nice. All residents are registered with a General Practitioner.
Blisworth Close (Royal Mencap Society) DS0000012713.V265965.R01.S.doc Version 5.0 Page 14 Records were available within the care plans of the involvement of the optician, dentist, chiropodist and speech therapist, records demonstrated that residents have ongoing support from health care professionals and records of annual health checks taking place. The staff administer all medication, the storage and administration records of medication was satisfactory. Many of the residents have lived at the home for several years and with age their needs and capabilities have changed. The care provided has been adjusted to meet their physical and emotional capabilities and personal wishes, for example records within care plans demonstrated that this need had been identified and in some cases a decision to reduce the number of days at the day centre to allow for more rest, was reached. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 22 & 23 There is a clear and effective system in place for residents and their families, to voice their concerns and be confident that their complaints will be listened to and acted upon. EVIDENCE: Residents and their families are each given a copy of the complaints procedure which includes the stages and timescales, for the process and informs the resident and their families of who to complain to, the procedure contains reference to The Commission for Social care Inspection (CSCI), however it does not contain the address or telephone number of the CSCI if residents or their representatives wished to forward their concerns or complaints to the CSCI. The complaints system in place for residents is in a format appropriate to their needs. Resident’s finances are handled appropriately, with accurate records of cash held in safekeeping. The home has the Northamptonshire Multi Agency Policies and Procedures available to follow if there was a need to report any suspected or actual abuse. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 24,25,26,27,28,29 & 30 The homes environment is fit for purpose and provides a range of communal options for service users to choose from EVIDENCE: The home is of domestic layout situated in a residential estate Residents have individual bedrooms that have been decorated and arranged to their own preferences. The bedrooms had been personalised to include televisions, music systems, books, videos and items of individual preference within, which provided residents with a choice as to how they spend their leisure time. All bedrooms had ensuites that comprised of a toilet and wash hand basin and bathrooms clean and fitted with aids and adaptations. The décor and furnishings within the home are to a good standard. The communal space available enables service users to spend time on group activities. Externally the grounds provide a secure pleasant garden area that has been landscaped by residents and staff, to include raised flower borders, container plants and outside seating. JUDGEMENT – we looked at outcomes for the following standard(s): 32,33,35 &36 Residents are supported by a consistent and effective staff team, that are aware of their needs. EVIDENCE: The support worker available on the day of the inspection was an established member of staff employed by Mencap, and was very knowledgeable of the resident’s individual needs and the level of support required by each. The resident who was available at the beginning of the inspection, appeared very relaxed and related well with the member of staff and the staff member was observed interacting well with the resident with respect and dignity. The member of staff on duty informed the inspector that they had attended a range of training courses to enable them to meet the needs of residents and had achieved a National Vocational Qualification level 3,in care, the inspector was informed that training is planned to cover manual handling updates and infectious diseases. The staff rota demonstrated that staffing levels are available in sufficient numbers to ensure that resident’s needs are met. Minutes from in-house staff team meetings and organisational support workers employee forum meetings were available to view and records of a one to one

EVIDENCE: There are clear and accessible policies and procedures in place for staff and residents. The homes record keeping practice is good and all the care and administration records looked at were satisfactory. There is a service user feedback system in place to gain the views of residents and their representatives, which aims to identify areas for improvements to the service delivered, records of self monitoring were looked at which included service delivery and monthly health and safety monitoring. There were extensive risk assessments in place in relation to cleaning products in use and manufacturers data sheets available to comply with the control of substances hazardous to health (COSHH)

There are comprehensive policies and procedures available for staff to access to provide guidance on organisational consistency in service delivery. Staff said that they are well supported and that working at the home was very rewarding. Written documentation within the care plans demonstrated that, the support workers know each resident very well and the care provided is person centred and tailored to meet each residents preferred lifestyle.
