What the care home does well

Residents spoken with said that they are well treated by the staff and that the staff are kind and treat them with respect. It was reported that the nurses are knowledgeable and provide a high level of nursing care. Residents said that relatives are made very welcome at home. The activities co-ordinator provides a wide variety of activities that the residents appear to very much enjoy. The layout allows care to be delivered in spacious surroundings with residents benefiting from country views and communal areas that are bright and airy. Meals are varied and home cooked, being nicely presented and offering choice and variety.

What has improved since the last inspection?

The registered manager has introduced new care plans that are easy to follow. More comprehensive risk assessment are in place to monitor nutrition, dependency and mobility. The system of monitoring medications has been improved to reflect why medications are given.

What the care home could do better:

Some work is needed to ensure that the care plans accurately reflect the current care needs of the residents particularly in terms of risk assessments. It is also important that all care staff receive regular supervision from senior staff to ensure that they remain up to date and that their professional development is maintained. The registered manager needs to ensure that the recruitment process is followed and any staffs appointed do not commence work until all information is available. The exterior of the building also needs some attention to prevent residents being at risk of falling masonry

The home is situated in the Warwickshire countryside close to Ashorne Village.The mainly single accommodation is provided on two floors with access via passenger lifts or stairs. The home’s décor and furnishings are to a high standard and are domestic and homely.The home has established and well maintained gardens, which are accessible to all residents including those who may require wheelchair access.Alpha Health Care Ltd is the current owner having acquired this home in November 2004. This inspection took place over 5 hours and is the first of two inspections to be conducted this year. A tour of the premises was undertaken, looking at all communal areas, bedrooms and service areas, such as the kitchen, lounges, bathrooms and bedrooms. Six residents’ care plans were examined which gave an overview of the care prescribed and delivered, these records also contained information as to how any identified health care needs were being met. Records related to staff recruitment were examined along with records related to the upkeep and maintenance of the home. Medication administration and records related to this area were also seen. Six staff, 10 residents and one relative were spoken to. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 The assessments completed by registered manager or her deputy at the home, prior to residents moving in, ensure that residents’ personal, health and social care needs can be met by the staff working at the home. EVIDENCE: The initial assessments of the last three residents to move into the home were reviewed. The layout allows all relevant information to be collected and thus enable the registered manager to make the decision if the residents’ needs can be met. Each resident’s care plan is written using the information from the initial assessment. Visits to the home often take place prior to admission. One resident was currently spending a month at the home with a view to a permanent move. Bromson Hill nursing home does not provide intermediate care but does on occasions provide short stay respite care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 9 and 10 Residents are treated with dignity and respect and although some individual care plans do not accurately reflect the resident’s current care and support needs these needs are generally being met. EVIDENCE: A number of care plans were inspected and found to contain information needed for staff to ensure care needs are met. Since the last inspection the manager has introduced clear risk assessments for nutrition, falls, care dependency and pressure area risk. It was noted in several of the care plans that reviews had not been undertaken sine June 2005. Many of the residents were spoken to by the inspector, both in groups and privately. All were happy with the care provided and stated they were well looked after. Comments such as “the care is smashing” and the “girls are delightful and helpful” were reported. Throughout the day it was observed that there is an extremely good rapport between staff and service users with general observation of care practice supporting service users’ views that staff treat them with respect and understanding. Policies and procedures are in place for the safe handling and administration of medication with improvements in recording made since the last inspection. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 15 Meals provided are varied and offer a balanced diet for residents. Arrangements and management of mealtimes are well organised and present no obvious risks to the safety, well being and comfort of residents. The current arrangement for activities and entertainment provide adequate recreation or motivation for residents. EVIDENCE: Lunch was taken in the main dining room with the inspector sitting at a table with four residents. A choice of three courses was offered and over five deserts offered. The meal was of an extremely high standard and was nutritious and tasty. All residents spoke highly of the food and choices offered. During discussions with residents it was stated that on the whole activities were satisfactory but some residents would like to go out more often. The activities are displayed in the lounge area, on posters and in a monthly newsletter. Three residents stated that they particularly liked the indoor bowls and tennis. Other recent events included a trip to a local country park and a strawberry tea. A falconry display was held at the home recently with one relative writing to the manager stating “ a lovely treat for the residents and relatives. We are so appreciative of all the effort that goes into the activities programme, so a very big thank you”. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 Procedures are in place to ensure that complaints are handled in a structured manner within clear timescales and an objective response is made. EVIDENCE: The written complaints procedure is in the statement of purpose and service users guide. Residents spoken to knew who to speak to if they were unhappy or wanted to make a complaint. All of the relatives spoken to knew how to make a complaint. A number of minor concerns have been logged with evidence of swift resolution by the homes staff noted. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,24 and 26 Residents are encouraged to bring in personal items for their rooms to make them feel more at home. All residents have access to all parts of the home and grounds including wheelchair users. The home is clean and hygienic in residents’ areas but potential risks to health were identified at the front of building EVIDENCE: The home is spacious with a number of sitting areas for residents including conservatories that face the homes court yard. Many of the bedrooms were visited during the inspection. It was noted that many of the service users have made their rooms very homely and taken the opportunity to personalise rooms with items from home or their family. Two resident reported that this made her feel more at home. Residents reported that the home is always very clean and tidy with no smells or “dirty, dusty areas”. It was noted that areas at the front of the home by the main front door are cordoned off due to failing masonry and the potential of further falls. This must be assessed with immediate effect and appropriate repair made. Access to the home is not compromised with a second entrance to the home used by all staff residents and visitors. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 29 Staffing levels were in sufficient numbers and skill mix, with indications that residents’ needs were being met appropriately. The procedures for the recruitment of staff are not robust. Appropriate pre employment checks are not being carried out or evidenced in the home thus potentially leaving residents at risk. EVIDENCE: The registered manager is presently recruiting staff with a good response. Staffs spoken to report they are happy to work at Bromson hill and many have worked at the home for several years. Service users spoken to said that the staff were usually available to meet their needs and also had plenty of time to talk with them although a couple felt that more staff could be employed. During the visit it was clear that all residents needs were being met promptly and appropriately. The staff file of most recently appointed member of staff was reviewed and indicated that not all necessary recruitment checks to ensure the protection of service users have been completed. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35 and 36 Procedures are in place to manage residents’ monies and valuables so their interests are safeguarded. Most care staff receive support and guidance from the registered manager or trained nurses, this needs to be developed to include all staff and be structured around development and training needs, so safeguarding the health, safety and welfare of the service users. EVIDENCE: The policy of the care home is not to handle residents’ money or valuables with this being the responsibility of relatives, advocates or the resident. Systems are in place for providing support and supervision to care staff but currently this is not being provided on a regular basis. All care staff should be supervised at least six times a year. Residents spoken with reported that staff were very good at caring and knew what they were doing. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards.
