Conditions of registration: None Date of last inspection 11th May 2005 Brief Description of the Service: Buchan House is a purpose built home for the elderly, built on two floors and was previously owned and operated by the Local Authority. Stairs and a passenger lift provide access to the upper floor. The home has enclosed gardens and a visitor’s car park, and offers a dedicated and safe facility for service users with dementia. All rooms are single occupancy and have en-suite toilet and washing facilities.The home is situated in the King Hedges area of Cambridge, close to local facilities and amenities. This is an overview of what the inspector found during the inspection. This was an unannounced inspection, and the third visit of the year. The inspector met with a relief manager who has been moved from another home pending a new manager, who has now been appointed, taking up the position. The inspection took place between 11am and 2 pm, and included observation and discussions with staff and service users over lunch. A number of new staff have been recruited recently, and CRB returns and references are being taken prior to them commencing their work. The home currently has 6 vacancies, and a number of prospective service users are considering the home. The beds that are block purchased by the local authority are all filled. What the service does well: What has improved since the last inspection?
Since the last inspection, further management changes have taken place, and at the present time, a relief manager is in post pending the home’s new manager taking up her position, which will be in early November. In addition better systems of recording have been introduced, ensuring that information is more readily available. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 and 6 Service user’s needs are assessed prior to moving to the home, ensuring they both service users and their families can expect their needs to be met appropriately. EVIDENCE: The homes’ relief manager confirmed that prior to any service user moving into the home, a full assessment of need would be undertaken and the home being clear that these needs could be met. The service receives assessment information from the commissioning team, but does not rely on this, having found previously that they can be a little out of date. Visits to the prospective service user are made, either to their home or to the hospital. Wherever possible, prospective service users would visit the home, more often the visits are made by family members. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 and 9 Individual care plans identify the needs of service users, and how these needs should be met giving care staff clear guidance. Policies and procedures ensure medication is administered appropriately and safely. EVIDENCE: At the present time, the relief manager is reviewing the current care plans, and introducing additional plans where required. For example new care plans are being introduced to reflect better the wishes of the service users at night, including detail regarding their choice of bedtime, when they get up, whether they like a night time drink etc. Another care plan being developed reflects the social choices made by service users, this is being written with input from the homes activity co-ordinator. The home has developed a much better relationship with the GP practices they use and the district nursing service, which now includes weekly surgery visits by the GP, a separate weekly district nurse visit, regular visits from the community psychiatric team and psycho geriatrician. These visits are in addition to callouts for emergency or routine treatment visits by district nurses.
Buchan House I53 I03 S15236 BUCHAN HOUSE V247430 200905 STAGE 4.doc Version 1.40 Page 11 Any individual consultations take place in service users rooms, ensuring their privacy at all times, and the manager has issued instructions to night staff to ensure that if a service users chooses to be up during the night, they are appropriately covered, both to preserve dignity and to ensure they are warm and comfortable. She has also introduced a system to record where service users are awake during the night, to ensure that any patterns are identified and referred to the GP where necessary. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 A variety of events and activities have been arranged or are planned, ensuring service users have a choice in all areas of their lives. EVIDENCE: As already mentioned within the report, the new manager is reviewing care plans, and ensuring a care plan is in place to reflect service users individual social needs. The home’s activity worker has improved the activities available, which was identified during the last inspection report, and this has continued, and this was confirmed during the inspection by some of the service users. Within the premises the providers also operate a day centre, and this is available to be used by service users in the home where this is appropriate. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Policies and procedures are in place to protect service users, and ensure comments and concerns are responded to. EVIDENCE: The home has a complaints policy and procedure in place, which remains in line with the standards. The CSCI has received some complaints since the last inspection, but these have been appropriately investigated by the home where required. An adult protection policy is in place, and the two unit managers have both undertaken the three-day adult protection course run by the local authority. All care staff receive regular POVA training. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 and 26 Buchan House offers a well-maintained environment in which service users are safe, and provides clean and hygienic accommodation to those living there. EVIDENCE: Since the last inspection, the carpets in the main corridors in the dementia care unit have been replaced with a tiled floor, which has eradicated the problems with unpleasant odours. This was subject to an additional visit by the CSCI on the 13th July, and risk assessments were provided by the home to meet requirements left following this visit. During this inspection, the floors were seen to be clean, and free from any spillages, and the manager confirmed that staff are vigilant, and any issues are immediately dealt with. Unit managers are now responsible for a daily check, and the relief manager ensures that she also looks around the premises at least once a day. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 and 30 Sufficient staff are employed by the provider to ensure the needs of service users are met, and the staff receives training to ensure they can competently meet these needs. EVIDENCE: At the present time, the home operates with a team leader and 5 care staff during the mornings, and a team leader and 4 care staff during the afternoon, with 5 care staff working at night. In addition the unit managers each work a 5-day week including alternate weekends, which together with the home manager does ensure that the home has sufficient management cover. The home has a full staff complement at the present time, although they are waiting fro recruitment checks on 2 new starters to be completed. In addition a recent recruitment drive has identified a number of prospective starters to cover future vacancies both at Buchan House and other homes within the group. An extensive training programme is now in place, including protection of vulnerable adults, Infection control, fire safety, moving and handling, and a programme of NVQ training is now underway. In addition, the district nurses and Community Psychiatric team are also providing regular training sessions for staff, and a dietician visits the home monthly to provide advice and support in all nutritional matters.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 33 and 38 Service users interests are respected at all times, although management changes have been significant in recent months. EVIDENCE: Recent changes have improved the emphasis in the home on service users participation and the relief manager is ensuring that care plans reflect the wishes of service users. It is important that this continues when the new manager starts, which it is understood will be early in November, and an application for her registration must be submitted to the CSCI as soon a possible. The home has appropriate policies and procedures to maintain the healthy and safety of service users and staff, and this is supported by the risk assessments on place.
