Conditions of registration: None stated Date of last inspection 15 December 2005 Brief Description of the Service: 703 Burton Road is owned by the Robinia Care Group and is managed as a small family type unit. The home is registered to provide personal care and accommodation for 5 people in the category of adults with a learning disability aged between 18 and 65 years of age. The house is situated between Burton on Trent and Swadlincote and enjoys access to both areas. The house has been furbished to reflect the style of a family home. This is an overview of what the inspector found during the inspection. This was a routine unannounced inspection, carried out over 7 hours. The inspector case tracked the care of two service users and looked at their files, their medication and their monies. All service users were spoken with during this inspection. All staff, including the Registered Manager, on duty at the time of the inspection participated in general discussions. What the service does well: What has improved since the last inspection? Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, 8 and 9 People living in the home were involved in all aspects of their care, and made decisions about their daily lives and supported to take responsible risks as part of their development. Staff had a good understanding of the service users support needs. EVIDENCE: New documentation was being introduced for service users files. Personal service plans had been developed, which were detailed and provided guidance for staff. Service users had been involved the development of care plans and had signed the documentation. Detailed management plans had also been developed, although these were not dated. The support provided to the people living in the home was reviewed at least every six months. The daily logs provided detailed information about the daily lives and activities of the people living in the home. Observation of service users and the records indicated that they made decisions and were involved in all aspects of their daily lives. Staff provided support and assistance whilst promoting independence. People were encouraged to take responsibility for day to day tasks and keeping their home clean and tidy. Regular agenda items included activities, menus and staffing, as well as any other business. The minutes of these meetings supported that service users were able to raise any issues and given responses. People in the home were enabled to take responsible risks, and these had been identified and planned for. Service users had been involved in the development of risk assessments, and had signed to agree these. Risk assessments were also completed each time an activity took place, taking into account the service users as well as the experience and abilities of the staff. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11, 12, 13, 14, 15, 16 and 17 People living at the home were supported to live as ‘normal’ a life as possible. Links with the community were good and supported and enriched service users’ social and educational opportunities. Family contacts were encouraged and maintained as appropriate. EVIDENCE: Staff were observed supporting service users to use practical life skills, such as making hot drinks, and washing and drying the dishes. Service users were supported to use public transport independently. Discussion with service users indicated that they attended various activities outside of the home, such as college courses, art and music sessions, and community facilities such as local shops, hairdressers and public houses. The people living in the home discussed last years holidays, and stated that they were looking to book this year’s holiday in the near future. Service users were encouraged to maintain links with their friends and families, and several service users regularly went to stay with their families.

Service users were well supported by advocacy services. Good use was made of the facilities in the local community. The home was well placed for public transport links and close to both Swadlincote and Burton on Trent. Staff routinely supported people to access community facilities, such as the hairdressers, bank and local shops. The routine at the home was very relaxed and homely. The people living there were comfortable in their surroundings and made good use of all areas of the home. All service users interacted well with each other and with the staff. Service user participated in household chores and with some aspects of maintaining the garden area. The service users participated in some aspects of meal preparation within the domestic setting. They were involved in the planning the evening meals for the week and going shopping for the food. People chose daily what they would like for breakfast and lunch. Records of meals were kept. Friday night was fish and chip night, and service users also ate out at cafes and public houses. Staff supported service users to be as independent as possible, and supported people to make drinks, prepare snacks, and help with the cooking. Service users were observed having drinks as they wished. Support and encouragement was provided to service users to assist them to maintain a healthy diet. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18, 19, 20 and 21 Service users personal and health care needs were met with support and assistance from staff at this time. The systems for the administration of medication were good with clear and comprehensive arrangements in place to ensure service users medication needs were met. EVIDENCE: Conversations with the people living in the home supported that the routines were flexible and each service user had their own routine, including individual rising and retiring times. The support and assistance required by individual service users was recorded in their personal service plan. Service users were dressed in age-appropriate clothing and it was clear that they were encouraged and supported to attend to their personal appearance. Staff supported service users to attend health care appointments. Information about specific medical conditions was available in service users files. No issues were noted around the administration of medication at the time of this visit. Systems were in place to ensure that medication was available for social leave. Death and dying had been discussed with the service users and/or their families, and relevant information recorded in the files.

JUDGEMENT – we looked at outcomes for standard(s) 23 Systems for safeguarding money were not satisfactory, placing service users at potential risk of financial abuse. EVIDENCE: A recent incident at the home demonstrated that staff were aware of the whistle blowing policy and felt supported to use this procedure. Management dealt with this incident appropriately and professionally using the correct procedures. People living at the home were supported to staff to manage their own money, and to hold their own bank accounts. The systems in place to safeguard money in the home need strengthening, as discrepancies had occurred. JUDGEMENT – we looked at outcomes for standard(s) 33 The deployment and numbers of staff appeared satisfactory to meet the needs of the current service user group. EVIDENCE: A copy of the duty rota for July 2005 was provided. The staffing levels provided enable staff to provide one to one support as well as group support to service users, both inside and outside the home. The Registered Manager was provides support for staff and was able to cover any shortfalls in shifts.
