Conditions of registration: Date of last inspection 3rd November 2005 Brief Description of the Service: 703 Burton Road is owned by the Robinia Care Group and is managed as a small family type unit. The home is registered to provide personal care and accommodation for 5 people in the category of adults with a learning disability aged between 18 and 65 years of age. The house is situated between Burton on Trent and Swadlincote and enjoys access to both areas. The house has been furbished to reflect the style of a family home. This is an overview of what the inspector found during the inspection. This was an unannounced visit and information was gathered both during and prior to the visit by reading records and speaking with staff and service users. The inspector was unable to examine staff records as the staff on duty did not have access to them. What the service does well: What has improved since the last inspection? What they could do better:
At the time of the visit the home did not have a Registered Manager, therefore a manager needs to be appointed. The staff did state that the Provider had interviewed the week before the visit. The decorating on the first floor is now shabby and in need of updating. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Detailed information contained within service users files assists the staff to meet the changing needs of the service users. EVIDENCE: The service users living at the home have done so for a number of years. The assessments for each person living at the home have been reviewed and rewritten as their needs change. The information recorded was detailed and comprehensive, however the ‘medical history’ area within one assessment was incomplete, although it was evident that there was significant medical history. Each person living at the home had been supported to contribute to the review process by their key-worker, this was evident within the files and when speaking with the service users. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Detailed information and risk assessments enables service users to develop new skills and take risks EVIDENCE: Information was separated into different files. Detailed personal service plans, developed by residents and staff, were in place. Management plans in place now dated and signed. Individual reviews for each person living at the home take place at least every six months and service users are involved as much as possible. Service users stated that staff supported them and encouraged them to be as independent as possible, this was evident on the day of the visit. Service users are encouraged to take responsibility for day-to-day tasks and keeping their home clean and tidy. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Good, positive community links provides additional support for service users and promotes independent living skills. EVIDENCE: Good links with the community are in place. The service users spoken with talked about the regular activities they attend, such as art and music therapy, discos, shopping both in a group and individually. Service users attend activities individually as appropriate and travel independently using public transport if appropriate and risk assessments are completed for these occasions. Service users living at the home are supported to keep in touch with family and friends. Support for residents outside the staff group was also available through advocacy services.

The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Appropriate staff training and clear records ensures that service users are safe. EVIDENCE: Service users spoken with felt that the staff provided them with the support they needed. This was evident within the files examined, service users attend appointments with other professionals as necessary, files examined evidenced input from the GP, optician, social workers and other professionals. The home keeps clear records of visits from professionals. The assessment of need was incomplete within one of the files examined, it was evident that the service user should have had significant information within the previous medical history section but it was blank. Staff spoken with stated that medication was administered by staff trained to do so, however as the Registered Manager had recently left the home, the Inspector had no access to staff files. The medication is stored safely and a clear record is kept of medication received and administered. The medication

The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22,23 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The systems in place for addressing complaints and safe guarding adults ensure that service users are safe. EVIDENCE: Systems are in place for raising and recording complaints. Service users have a copy of the complaints procedure in their file, and staff stated that this would have been discussed with them on admission. The procedure was also on display. Service users spoken with were aware of the procedure and confident that any complaint they may have to make will be addressed appropriately. Service users are able to raise any issues through the house meetings, however these are irregular; 10/06, 02/07, 03/07 or with key workers as part of the monthly reviews and minutes of these meetings are kept. Information on complaints and the monthly review forms is available in pictorial as well as written format. There are 3 complaints on record all appear to be resolved. The systems in place to safeguard residents monies have been strengthened, through daily monitoring. All staff have completed Safe Guarding Adults training.

Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24,30 Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Although the home is domestic in style, clean and tidy, the poor decoration in areas potentially prevents service users taking pride in their home. EVIDENCE: The home is domestic in nature and located within a residential area. It has a homely appearance and was clean and tidy at the time of this inspection. There are areas of the home, which are in need of updating, this was discussed with the staff on the day of the visit. All Service users have single rooms, and several choose to keep their doors locked. The bedrooms reflected the individual tastes and preferences. The home had a domestic style bathroom and separate shower room. Staff confirmed that the service users accommodated are able to use these facilities with minimal assistance.

Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 32,35 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Appropriate staff training, staffing levels and consistency helps to assist service users with development of new skills and builds on trust. EVIDENCE: The staff team is organised into three levels of responsibility, although at the time of the visit the home did not have a Registered manager. It appears that an appropriate skill mix is provided on each shift. The staffing levels provided enable staff to support residents, both individually and as groups, both inside and outside the home. The staff spoken with confirmed that they have regular supervision, the inspector was not able to examine the records and the staff had no access to the files. Staff spoken with stated that they had received appropriate training to meet the needs of the service users. Recruitment records were not examined on this occasion due to inaccessibility.

Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 37,39,42 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The home is run in the best interests of the service users, with quality assurance and quality monitoring providing the basis for improving the service. EVIDENCE: People living at the home were supported by a stable staff team, although the Registered Manager had recently left the home, the staff stated that the provided had interviewed and they were awaiting confirmation of the new manager. Staff support each other and work together as a team to improve the service that they provide for the service users. Records seen within the home were well maintained and up to date and information was stored appropriately. Systems were in place for servicing of equipment and services within the home. Records supported that equipment and services were properly maintained. Fire records are up to date and service users spoken with confirmed that they were aware of what to do in the event of a fire.
