What the care home does well

The outcomes for the people who use this service are very good. Daily life and social interaction is very good. On the day of the inspection there was a constant stream of visitors to the home, some joining their relative for lunch in the dining room. The service users themselves informed the inspectors that staff and management will do all they can to ensure their lives are enjoyable and as fulfilled as possible. The inspector joined the service users for lunch in the dining room and found the experience very positive. The food was very good with good menu choices. Those service users who needed assistance with eating were assisted in a manner that was discreet and respectful. The environment was clean and fresh. The home provides many areas for service users and their families to meet in private. The gardens are very well maintained and have good quality garden furniture. On the day of the inspection service users were enjoying morning coffee on the patio. The internal auditing system works well picking up and addressing issues on matters of the care of service users.

What has improved since the last inspection?

There is a new conservatory providing extra communal space for people who use the service. The storage area for medicines has been extended allowing staff more space to work. All the requirements left at the last key and the last random inspections were met.

This was a very positive inspection.

Date of last inspection Brief Description of the Service: The Bushey House Beaumont is a 19th Century Grade II listed mansion with a purpose built extension in keeping with the original house. The view over a lake and parkland can be seen from the library, dining room and patio. The home is spacious and furnished and decorated to a high standard in keeping with the period style of the building. Individual rooms have en suite facilities and in addition to these there are assisted bathing and toilet facilities to meet the needs of elderly and physically disabled residents. The home is close to the local amenities of Bushey High Street. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. This judgement has been made using available evidence including a visit to this service. Service users are assessed prior to admission, thus ensuring that the service is able to meet their needs. EVIDENCE: Service users are fully assessed prior to admission to the home. Four service users assessments were inspected and they contained full information on the service users’ needs and wishes. The pre-admission assessment was documented and kept in the service user’s file. The home will only admit a service user whose needs can be met. A copy of the contract was given to each service user on admission. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The health, social and personal needs of the service users are set out in care plans. Health needs are fully met in a manner that respects the dignity of the service user. Medication is administered and recorded appropriately. EVIDENCE: Care plans and risk assessments were in place and detailed how care was to be provided. Six care plans were examined and contained all the relevant information that will equip staff to meet service users needs in a consistent manner. Medication was well maintained and recorded. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14 & 15. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The daily life and social activities meets the needs of the service users. The home welcomes visitors. Service users exercise control over their lives. EVIDENCE: On the day of the inspection the sun was shining. Many service users were in the garden enjoying the sunshine. All service users spoken with told the inspector that they have access to the gardens. There was an exercise class in the morning that involved very gentle exercise this was well attended. In the afternoon there was a quiz in the dining room this was also very well attended and enjoyed by the majority of the service users who live in the home. There was a monthly schedule of activities that is available to all the service users. The dining room appears to be the social centre of the home. This is a large airy room overlooking extensive well-maintained gardens that have a rural aspect. At lunchtime, the tables are attractively set. Service users have two choices for lunch with an additional vegetarian option. Where required, service users were assisted with lunch in a very a discreet manner that respected their dignity. Pureed lunch was very well presented. Care plans detailed the life histories of the service users and what they like and dislike and how they like to spend their time. There was a steady stream of visitors on the day of the inspection. Family members were observed to join their relatives for lunch. Service users spoken with on the day of the inspection stated that they were happy with the daily life of the home. The hairdresser calls one day per week and was there on the day of the inspection. Service users are protected from abuse. EVIDENCE: All complaints (both written and verbal) were treated seriously. They were responded to promptly in accordance with the home’s complaints policy. Staff spoken to were aware of how to respond if there was to be an incident of abuse in the home. Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. The environment is safe and very well maintained. The home is very clean, hygienic and pleasant. EVIDENCE: The home is set in extensive, well maintained grounds. There is sufficient garden furniture to allow the service users to spend time in the garden shaded from the sun. The home is in very good decorative order. Individual room are decorated and furnished to meet the needs of the service users and to reflect the personality. The fixtures and fittings are of a high quality and the home is well set out and maintained. There is a selection of communal areas giving the service users and their visitors a choice of areas to sit quietly and meet their visitors. The home was well-lit, clean, tidy and smells fresh. There was a good supply of well-maintained equipment available to staff to assist service users where necessary. Work to provide a much needed enlarged clinical room and replace the disused hydrotherapy pool with a conservatory has been completed. This judgement has been made using available evidence including a visit to this service. The needs of the service users are met by well-trained caring staff who are recruited appropriately. EVIDENCE: There is a core of well-trained dedicated staff who deliver care to high standards. Staff spoken with informed the inspector that they receive regular supervision from their line manager. These staff were friendly and welcoming of the inspection process. Service users spoken with were clear that the staff met their needs in a manner that respected their dignity and privacy. Staff training records indicate that staff receive mandatory training and staff spoken with informed the inspector that they are encouraged to extend their training 57 of care staff have now completed their NVQ level 2 award. The home works to the ratio of one staff member to five service users. Four staffing files were inspected and they were found to contain all the all the necessary documentation including evidence of Criminal Record Bureau checks and two references. The home does not use temporary staff. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The home is well managed and is run in the best interests of the service users. The health and safety of the service users is protected and promoted. EVIDENCE: The home is well managed in the best interests of the service users. Service users and staff stated that they would have not problem taking issues to the manager, as she is very approachable. Efficient systems are in place to monitor staffs’ adherence to policies and procedures. The management approach to service delivery means that the outcomes for a service user living there are excellent. All the necessary health and safety checks were in place. Equipment is serviced regularly and there is a fire plan in place and fire drills take place regularly.
