The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

Camden Lodge is a comfortable and well-decorated home that provides a good standard of care for service users. Service users looked happy and well cared for and those interviewed were able to confirm this. There have been some changes to staff working at the home. Service users have benefited from the recruitment of new staff as the increased levels of competent and committed staff have improved the standards of care. The living and working environment is very comfortable with a good rapport between service users, management and staff. The manager at the home has been in post for almost a year and it is the inspector`s opinion that the home has benefited from her leadership and direction. The recruitment of an area manager has also provided valuable management support to the home.

What has improved since the last inspection?

The home now has a category of dementia added to the registration certificate. The manager is also now registered with the CSCI. Care plan documentation has been reviewed to provide a comprehensive assessment of service users needs, their goals and how these are to be met. The home now provides culturally appropriate foods and a variety of food to meet the individual needs of service users. An assessment and consent form for service users who wish to self medicate is now available. Some staff have received training in adult protection, moving and handling and food hygiene. Service users who were identified at the last inspection as prone to falls have been referred to an Occupational Therapist. Offensive odours in some bedrooms have been eliminated through the replacement of carpets and floor coverings. Some bedrooms and the lounge have been redecorated. A quality assurance exercise has been carried out to seek the views of service users and other stakeholders on the quality of care offered by the home.

What the care home could do better:

A restated requirement is made for all staff to receive adult protection training as currently only 3 staff have received training. Medication must be stored at temperatures below 25 degrees Celsius. Named service users must be consulted regarding their wishes and decisions about their care and social interests. A named service user must be referred to the relevant health professionals to ensure that their needs are met. Staff records must include a recent photograph, a full employment history and a statement as to their mental and physical fitness.

There are 24 registered places for people over the age of 65 with dementia. This home is purpose-built and has a lift and a garden. All bedrooms are single and have en suite toilet and washbasin. There are bedrooms on each of the three floors and a large lounge/dining room on the ground floor. The registered provider owns two other homes in the area. The aims of the home include “ to offer a warm welcome, which combines comfort with the support of a caring professionals.” Camden Lodge Residential Care Home G59 S10654 Camden Lodge V220948 06.06.05 Stage 4.doc Version 1.20 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This announced inspection was 6 hours and 15 minutes in duration. Present were the registered manager, Ms Eleni Constantinou and Ms Michelle Harrington the area manager. The home is registered for 24 service users over the age of 65 with dementia and at the time had 1 vacancy. 5 of the 23 service users had a diagnosis of dementia. As part of the inspection process the inspector was able to speak to approximately 7 service users and a visiting relative. Feedback on the quality of care offered by the home was generally very positive. 19 requirements were issued following the last inspection, of these, 18 are met and 1 restated. A further 5 requirements were issued bringing the total number of requirements following this inspection to 6. Requirements made relate to areas such as seeking the views of service users in all aspects of their daily life, storage of medication, staff training and staff records. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 4 Through staff training and consultation with service users, the home has demonstrated its commitment to meeting the needs of service users and improving the standards of care. EVIDENCE: Following the last inspection the home successfully applied to the CSCI for a category of dementia to be added to the registration certificate. This means the home is now able to admit service users with a diagnosis of dementia. The manager advised that a meeting had been held to inform service users about the change in the home’s registration. Following this meeting service users were given letters to sign to acknowledge they had been informed. The inspector was able to view copies of the letters signed by service users and minutes of the meeting held to inform service users and address concerns that they had. 11 members of staff have received training in dementia to improve the home’s capacity to meet service user’s needs. The home has two Greekspeaking service users, one of whom has limited English language skills. There are no Greek-speaking staff working at the home but the manager speaks Greek and is able to converse with this service user. The manager advised that in her absence, if staff experienced communication difficulties with this service user they would ring her and she would speak to the service user on the Camden Lodge Residential Care Home G59 S10654 Camden Lodge V220948 06.06.05 Stage 4.doc Version 1.20 Page 9 telephone. Feedback received from service users on the standard of care provided by the home was very positive. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9 and 10 There has been noticeable improvement in care planning documentation. To ensure a consistent standard of care the wishes and decisions of all service users must be sought and recorded. The storage and recording of medication has improved. Medication must be stored at appropriate temperatures to ensure the health of service users is not compromised. Service users are treated with respect but where a service user’s health care needs are not met this affects their sense of worth and dignity. EVIDENCE: The home has revised its care plan and risk assessment documentation to encompass a variety of service user needs, goals and how they are going to be achieved. Four service user files were inspected. All contained comprehensive care plans and risk assessment documents and a photograph of the service user. There was evidence of monthly reviews of care plans. Three of the four files examined contained care plan agreements that had been signed by both the service user and a representative of the home, however this was missing from the fourth file. The inspector queried this with the manager, as the service user in question was very articulate and able to make decisions regarding their care. The inspector was informed that this was an oversight that would be rectified. The registered person must ensure that the named Camden Lodge Residential Care Home G59 S10654 Camden Lodge V220948 06.06.05 Stage 4.doc Version 1.20 Page 11 service user is consulted regarding their care and records of their wishes and decisions maintained. Service user files examined contained evidence of visits by a variety of health care professionals such as General Practitioner, District Nurse, Opticians, Ambulance Crew and Chiropodist. An interview with a service user revealed that although they had hearing difficulties they did not have a hearing aid and they could give no explanation of why this was the case. An examination of their care plan showed conflicting information. It was acknowledged that they had hearing difficulties but could communicate through writing. It was also noted in the same care plan that they could communicate verbally very well and make needs known. Following an interview with this service user it was confirmed that they had very limited verbal communication and relied on writing to make their needs known. While the care plan showed visits from health professionals to administer eardrops or syringe their ears, there was no mention of how the hearing problems could be resolved in the long-term. A discussion with the manager revealed that the service user in question tended to remove their hearing aid. The inspector suggested that efforts be made to find out why the service user removed their hearing aid as it might suggest it was not working or uncomfortable. The manager must ensure that the hearing needs of a named service user are addressed. The home now has a controlled drugs register although there were none in the home at the time. The home has a refrigerator for the storage of medication and records are maintained of temperatures. Temperatures of the area where medication is stored were frequently around 26 and 27 degrees Celsius that are higher than the required 25 degree maximum. In a discussion with the manager she advised that there were plans to relocate the medication cupboard to a cooler room in another area of the home. The registered person must ensure that the area where medication is stored is monitored daily, recorded and kept below 25 degrees Celsius. One service user self-administers their medication. An agreement form signed by the service user and a representative of the home was seen however it was still to be signed by the GP. In a telephone call with the manager after the inspection, she informed the inspector that the GP had assessed the service user and it was agreed that they would take their medication under close supervision by staff. Service users interviewed confirmed that they were treated with respect and their dignity upheld. The inspector observed that service users were well dressed and staff addressed them with respect. Care plans seen contained information on the way each service user liked to be addressed as well as their preferred times for waking up and going to bed. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 and 15 The home must to do more to listen and cater for individual social interests to ensure that all service users feel valued by the home and the staff working there. The lack of restrictions on visitors and availability of information on independent advocates ensures that service users have access to external agents who can act in their interests if required. By effectively catering for service user’s varying dietary needs and tastes the home has been able to ensure high levels of service user satisfaction. EVIDENCE: Service user records contained information on their life history and background, family, former occupations and current social interests. This information is used to inform the home on the type of activities that service users are likely to enjoy. A service user interviewed said he enjoyed listening to classical music in his room and reading The Times newspaper. The inspector was able to confirm that a copy of this newspaper was available in the home and engaged in a lively discussion on the contents of that day’s paper. Another service user expressed an interest in regular walks in the garden or the local area. When asked whether staff were able to meet this need they indicated that this was not likely to occur. This was discussed with the manager who advised this would be addressed. A service user expressed a desire to be Camden Lodge Residential Care Home G59 S10654 Camden Lodge V220948 06.06.05 Stage 4.doc Version 1.20 Page 13 involved in a community club that they used to attend before their admission to the home. The registered person must ensure that the wishes and decisions of the named service users regarding their social interests are taken into account. The inspector was able to speak to a relative visiting the home who confirmed that there were no restrictions on visitors. At the previous inspection it was recommended that the registered person inform service users about advocacy services. The inspector observed information from Age Concern displayed in the home. The home has revised its menu to include Afro-Caribbean and Greek food such as salt fish, sweet potatoes and lentils with rice. A discussion with the cook revealed that members of staff had shown the cook how to make these dishes. Two service users consulted confirmed that were offered culturally appropriate food and if they asked for something different they would be able to get it. A third service user said they did not care about culturally appropriate food, as they were “cosmopolitan.” Camden Lodge Residential Care Home G59 S10654 Camden Lodge V220948 06.06.05 Stage 4.doc Version 1.20 Page 14 Complaints and Protection The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 The home recognises the importance of complaints in improving the quality of care. Displaying the complaints procedure has given service users and other people confidence in the complaints system. Service users do not have the assurance of being looked after by staff that have the training to safeguard them from abuse. EVIDENCE: Information supplied to the inspector by the manager indicates that the home has received two complaints in the last twelve months. Both complaints were responded to within 28 days and substantiated. The complaints procedure is prominently displayed at the entrance of the home. One service user said they had nothing to complain about but if they did they knew where to direct their complaints. At the last inspection a requirement was made for all staff to receive adult protection training. So far only 3 staff have received training. The manager advised that there had not been enough places on the course therefore more staff should receive training this month. This requirement is restated. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 22, 24 and 26 The home has put a lot of work into improving the general environment. Service users live in a safe, comfortable, well decorated and hygienic living environment. EVIDENCE: A tour of the home revealed that areas identified at the last inspection as needing repairs and redecorating had been addressed. The carpet in the lounge had been cleaned and carpets in several rooms had been replaced. The inspector observed that in some rooms carpets had been replaced with lino flooring. The manager advised that this had been done to eliminate odours as a result of continence problems and was implemented following consultation with the service user’s family. The inspector did not detect any offensive odours in the home at this inspection. Interviews with service users revealed that they were happy with the décor, atmosphere and standards of hygiene in the home. A service user said the home was “nice and clean and that’s what’s important.” There was evidence to indicate that service users had been consulted about Camden Lodge Residential Care Home G59 S10654 Camden Lodge V220948 06.06.05 Stage 4.doc Version 1.20 Page 16 the colour of the lounge walls before they were repainted. Service users who had had their rooms redecorated were also consulted about their colour preferences. The inspector noted that locks had been installed on all the bedroom doors in the home. The inspector observed a service user locking her room as she was going downstairs for lunch. At the previous inspection the registered person was required to ensure that service users who were identified as suffering regular falls be referred to an Occupational Therapist [OT]. The inspector was shown documentation confirming that the home’s GP had been asked to make an OT referral for the named service users. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 29 and 30 Recruitment practices at the home must be improved to ensure that service users are not placed at risk. The investment in staff training means that competent staff care for service users. A staff training and development plan should be designed to ensure that staff training needs are not overlooked. EVIDENCE: There have been a number of staff changes since the last inspection. Seven new staff have been recruited. Four staff files for new members of staff were examined. All contained criminal records bureau checks and references. The reference for one member of staff did not show the reason for leaving their last job. The inspector informed the manager that where staff worked with vulnerable children or adults they must get written verification of the reason they ceased to work in that position. Three of the four files examined did not contain a statement by the member of staff regarding their mental and physical health. All staff are required to complete an application form that includes information on their qualifications and experience. Of the four files examined, three did not contain a full employment history. For example one member of staff only gave the details of her last employer and no information about other previous jobs. Two of the four files examined contained a recent photograph. The registered person must ensure that a recent photograph, a full employment history and a statement as to their mental and physical health is available for each member of staff. Where staff previously worked in a position involving contact with vulnerable children or adults, there must be written confirmation of the reason Camden Lodge Residential Care Home G59 S10654 Camden Lodge V220948 06.06.05 Stage 4.doc Version 1.20 Page 18 they ceased to work in that position. Individual training profiles were available for staff however it is recommended that an annual training and development plan be compiled. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 33 and 38 By carrying out a quality assurance exercise, the home has shown its commitment to listening and valuing the views of service users and other interested parties. Staff training and regular health and safety checks ensure the safety of service users is maintained. EVIDENCE: Following the last inspection, the manager has been registered by the CSCI. The inspector was informed that the area manager had carried out a quality assurance exercise in the last two months. The inspector was able to see the results of this exercise displayed by the visitors signing in book at the entrance to the home. At the previous inspection, the registered person was required to ensure that all staff receive moving and handling training. The inspector was able to confirm that nine staff have received moving and handling training therefore Camden Lodge Residential Care Home G59 S10654 Camden Lodge V220948 06.06.05 Stage 4.doc Version 1.20 Page 20 all staff now have training in this area. An interview with the cook confirmed that they together with their assistant had received food hygiene training. The manager advised that fire officers from Southgate had visited the home to familiarise themselves with the building in case of a fire. The manager explained that the fire authority following a risk assessment inspects homes and the home was due to be inspected on 3/11/08.
