Conditions of registration: Date of last inspection 18th November 2004 Brief Description of the Service: Charnwood is a care home providing personal care for older people with mental health needs. It is a large Victorian/Edwardian detached house providing both single and shared accommodation for 16 Residents. There are 12 single bedrooms and 2 double bedrooms. The single bedrooms are a minimum of 10 sq.m and the double bedrooms are a minimum of 16 sq.m. There is a passenger lift giving access to the upper floor. There are three lounges (one of which is for those Residents who smoke) and a large communal dining room. There is a small car park at the front of the home. This is an overview of what the inspector found during the inspection. The inspection commenced at 10.40 a.m. and concluded at 1.45 p.m. Four Residents, one visitor and two members of staff were spoken to during the inspection. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 and 2 The Residents have the information they need to make an informed choice about where to live. Residents have a written contract with the providers of their care. EVIDENCE: The home has a statement of purpose and a Service User Guide, which can be accessed by anyone who wishes to know what facilities and services the home can offer. There was a written contract in the two sets of Residents documentation that was audited. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 8 The Residents health, personal and social care needs are set out in an individual plan of care, and their health needs are fully met. EVIDENCE: The care documentation that was audited contained in depth information about the Resident. Care plans were written once a need had been identified within the assessment process. Risk assessments had been written once a risk had been identified in the assessment process or following an evaluation of a care plan. Care plans are reviewed on a monthly basis. It was evident from reading the documentation that a lot of care and attention had been taken by the carer recording the information. All the documentation had the Residents next of kin’s signature to show that they had been involved in their Relatives care planning. Doctors, District Nurses, Chiropodists etc are requested to visit the Resident as and when necessary. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 15 Residents receive a wholesome appealing well balanced diet. EVIDENCE: On the day of the inspection lunch was mince and onion pie, vegetables and mashed potatoes with cornflake tart and custard to follow. The Residents who spoke to the Inspector said that the lunch had been very nice as were all of the meals that were served. One Resident informed the Inspector that his favourite meal was Sunday lunch because he loved roast beef, Yorkshire puddings and roast potatoes. There was staff available to give assistance to those Residents who required it. The visitor who spoke to the Inspector told how her relative had had a birthday tea last week, which everyone had enjoyed. The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Relatives are confident that a complaint would be taken seriously. Staff have had training in the protection of vulnerable adults. EVIDENCE: There have been no complaints regarding the home in the last twelve months. The visitor who spoke to the Inspector stated that she had never had to make a complaint but was confident that should she need to do so it would be dealt with appropriately. There is a policy and procedure for staff to follow should they receive a complaint. There was evidence in the training files that the staff had received training in abuse and the protection of vulnerable adults. The staff who spoke to the Inspector confirmed they had received training and were aware of what action to take should they become aware of any form abuse towards the Residents. The home has a copy of the No Secrets Protection of Vulnerable Adults Teeswide Guidance. The visitor who spoke to the Inspector stated that she could go home after visiting her Aunt and be confident that she would be well looked after and that the staff would contact her if there were any worries or concerns regarding her Aunt. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 23 and 24 Residents live in a well-maintained environment with bedrooms that are comfortable and suit their needs. There are sufficient toilets and bathrooms to meet the needs of the Residents. EVIDENCE: The home is well maintained, the Residents are encouraged to make their bedrooms as homely as they wish, one Resident had brought her own bedding in to the home with her. One of the double bedrooms is currently being used as a single bedroom; two Residents who both agreed to share occupy the other double bedroom. There is a curtain that is drawn to ensure privacy and dignity is maintained at all times. There are three lounges to give the Residents a choice of where they would like to sit. There is a quiet lounge, smoking lounge and a lounge with a television in it. There is a garden area that the Residents and relatives can access should they chose to do so.

The Residents, visitor and staff who spoke to the Inspector all stated that the environment was homely. The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 28, 29 and 30 Staff are being trained to NVQ level 2 that should ensure that they can meet the Residents needs. Residents are protected by the home’s recruitment procedures. Staff receive training to ensure they can meet the needs of the Residents. EVIDENCE: 62.5 of the staff have their NVQ Level 2 or 3 in care and the remaining staff are currently studying for their NVQ Level 2 or 3. The personnel files that were audited contained the required information as stipulated in schedule 2 of the Care Homes Regulations 2001. An audit of the training files showed that mandatory training (fire, and manual handling) and Resident specific training such as Optical Awareness, Dementia Awareness and No Secrets, had taken place. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31 and 36 The home is run and managed by a person who is fit to be in charge. Staff are appropriately supervised. EVIDENCE: The Manager has the required nursing and management qualifications to ensure that the needs of the Residents are fully met. The Residents, visitor and staff spoke highly of the Manager. The care staff receive regular formal supervision. This was evidenced within the personnel files and confirmed by the staff that spoke to the Inspector.
