The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

There is a friendly, calm atmosphere. Relationships observed between staff and service users were based on respect, as stated by the service users spoken to and Comment Cards received. The relative spoken to was very impressed by the care supplied to her mother and said that she was always welcomed by the Home`s staff. The Home was very clean and tidy with no hint of odour. Bedrooms were homely and personalised with service users saying they could bring in their personal possessions. Service users were happy with the activities provided by the Home.

What has improved since the last inspection?

The Home continues to provide a high level of care to residents.

What the care home could do better:

To ensure that all information is received to carry out a thorough assessment of service users needs so that staff are fully aware and can act consistently on this information. To involve the resident in the making up of the Care Plan so that it can be checked that the Home have all relevant information to act on residents needs. To ensure that risk assessments are carried out on all safe working practices, e.g. the wedging of a fire door, so that residents safety is protected at all times.

No-one under the age of 55 years who fall within category PD to be admitted to the home. Date of last inspection 30/9/04 Brief Description of the Service: Charnwood Park Residential Home is registered to accommodate eleven service users. The home is an extended detached modern style house, within a residential area. The home is surrounded by a large accessible garden. The ground floor consists of a large dining room, large lounge, with two bedrooms. The home has a shower facility, incorporating a toilet and additional separate toilets on the ground floor and a bathroom incorporating a toilet and a separate toilet on the first floor. In addition three bedrooms have an en-suite consisting of a toilet and wash hand basin.The home is within easy reach of shops and facilities, in both Barwell and Earl Shilton. Charnwood Park C51 S1793 Charnwood Park V231682 090605.doc Version 1.30 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This was an unannounced Inspection. The Home’s Registered Manager was on duty. Planning for the Inspection included reading the Pre-Inspection Questionnaire completed by the Manager and the 17 Comment Cards returned by residents and visitors. The notifications of significant events sent to the Commission for Social Care Inspection by the home were also reviewed. The Inspection took place between 9.40 and 14.25 and included a tour of the building, inspection of records and direct and indirect observation of care practices. The Inspector spoke to nine residents, one member of staff, one relative and the Registered Manager. Comment Cards were very positive about the care the Home provides and everyone stated that residents were provided with a high degree of care and staff treat them well (the only comments received needing action were that four residents wanted to have more involvement in the running of the Home – the Registered Manager said this would be further discussed with residents to take it forward). One resident stated, ‘’Management and staff very kind to me and caring’’. One relative stated, ‘’I believe Charnwood Park take very good care of my mother and I am happy with the cleanliness, attitude and care’’. This situation is commended. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Service users Assessments need to be more thorough to ensure staff act on all identified needs. EVIDENCE: The Inspector looked at two service users files. Whilst there was a good deal of relevant information in terms of physical and social needs there was information that was not completed on the assessment – e.g. hearing, communication, eyesight, night care, toileting, mental ability, social skills etc. The Registered Provider said that files would be reviewed to include this so as assist with staff consistency in providing care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 8,9,10 Service users medical needs are generally met by the staff. Service users feel that they are treated with respect by the staff. EVIDENCE: Service users said that when they felt ill the Home would swiftly summon medical assistance. Service users contacts with medical personnel was well documented in Care Plans. The Registered Provider said that dental appointments would be made when needed. It was recommended that they are booked on a regular basis even if the service user has got dentures, to ensure prevention of dental problems. Medication records were generally well kept with only a small number of gaps, which the Registered Provider was to follow up. A number of staff have received medication training on 12/5/05 with certificates seen. Service user said that their privacy and dignity was respected with staff knocking on doors before they entered. A service user said she liked spending time alone and she was able to do this and have her meals in her room. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13,14 Staff welcome service users visitors and provide a range of activities that service users are satisfied with. EVIDENCE: Service users can go out if they wish – e.g. one service user goes to Age Concern and another to the Avic Centre for activities. The Registered Provider said no one wanted to go to church but this would be organised if this were required. There is a visit from the vicar on a monthly basis. The Registered Provider said there was an identified staff member who was responsible for organising the provision of activities. Staff organise a number of activities, e.g. sing songs, bowls, exercises, quizzes, board games etc. Service users said that they were happy with the Home’s activities and that they could choose their own lifestyles, unhindered by rules. A staff member also confirmed this. The garden is attractive and service users were seen to be sitting out in it enjoying the sunshine. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16,18 The Registered Manager has taken steps to ensure that staff would act appropriately if there was a complaint or a suspicion of abuse towards service users. EVIDENCE: Service user said they thought that if they had a complaint the Registered Provider would properly look it into. There is a well laid out, clear complaints procedure. The staff member asked was fully aware of the Whistle Blowing procedure. There is an abuse procedure. The Registered Provider is to extend the reporting procedure to include the Police and the Commission for Social Care Inspection. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 26 The Home is clean, pleasant and hygienic. EVIDENCE: Service users said that the Home was always kept nicely and it is clean and tidy. They liked their bedrooms. This was observed to be the case – the décor is fresh and attractive. Furniture was attractive and appeared comfortable. Facilities include aids and equipment – rails, toilet seats, an adapted bath etc. This situation is commended. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,29 Staffing levels are satisfactory. Staff checking procedures need to be more rigorous to ensure full service user protection. EVIDENCE: The Registered Manager follows the staffing ratios agreed by the last Registration Authority – this is two care/Management staff with one cook from 8am – 2pm, two care/Management staff from 2-9pm then one awake night staff member from 9pm – 9am. Staff records were inspected. The Registered Manager has ensured that a Protection of Vulnerable Adults first check and Criminal Records Bureau have been obtained but that there was no copy of passport or similar ID for the staff records inspected. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 38 Health and Safety issues are generally covered well in terms of servicing and training. There needs to be some work on a Risk Assessment for the Fire Safety Assessment and one communal radiator. EVIDENCE: The registered person has now ensured that electrical systems and appliances meet current legislative requirements, and that they are to be regularly serviced consistent with legislation. Water was tested and found to be within the National Standard of 43c to prevent scalding to service users. Radiator covers have been fitted except to the ground floor shower room – this needs to be Risk Assessed to see whether a radiator cover is needed. A ground floor corridor fire door was wedged open – the Registered Manager said that this would be Risk Assessed and included in the Home’s fire risk Charnwood Park C51 S1793 Charnwood Park V231682 090605.doc Version 1.30 Page 16 assessment (The Registered Provider has contacted the Commission for Social Care Inspection to state that an automatic closure has now been installed to this door to ensure proper fire safety). Regarding staff training all staff except two have completed first aid training, all staff except one have had annual fire training, all staff had had infection control training, all staff have or are undergoing Moving and Handling training and all staff have received Food Hygiene training – the Registered Provider is arranging for this training to be held in the near future. There are regular fire drills and alarm and emergency lighting tests. The Registered Provider stated that Risk Assessments have been carried out for window restrictors and it was found that there was no need to fit them as no service users are at risk. Some service users mentioned that they could be chilly in their rooms at times even in the ‘summertime’. The Registered Provider is to follow this up and stated that heating was on continuously in the winter and for the majority of the time for other seasons. There is a hoist though this has not been serviced, as no one currently needs to use it according to the Registered Provider who said it would be serviced when it is needed. Staff recruitment checks need to have a copy of a passport or similar photo identification.
