What the care home does well

The philosophy of the Community means residents live in an environment that is very supportive and very enabling for them. Residents and staff share responsibilities and decisions so that everyone`s viewpoints are seen as equally of value. The residents were very positive in their views of how the Community supports them. They said that staff are very respectful and very supportive. Residents and staff value how the Community is run. This is through making very positive communication, and by valuing everyone as unique individuals. The description of the environment that was made in the last inspection report is still very applicable and has been quoted below: `A superb environment is provided, set in large rural grounds and yet close to local amenities. The community has a very calming ambience, which is enhanced by the extensive grounds and a number of different buildings, offering a range of activity `.

What has improved since the last inspection?

The adult protection policy has been reviewed and it is fit for purpose. The staff have been provided with training to ensure they understand the adult protection policy, and the principal of `the protection of vulnerable adults from abuse`. The service user guide has been reviewed to ensure it meets regulations and national minimum standards. The guide now sets out the organisational structure, including relevant qualifications, experience and responsibilities of the responsible individual, registered managers, staff and trustees. Criminal Records Bureau checks and a POVA First check have been undertaken for all the Trustees who undertake a Regulation 26 visits to the Community.

May accommodate up to 21 persons aged 18 - 64 years of age Date of last inspection 1st November 2006 Brief Description of the Service: Cherry Orchards is a therapeutic community, set in 18 acres of land, yet it is within reach of local shops and amenities. It is registered to provide accommodation and personal care for people with mental health needs. The community is based upon the philosophy of Rudolf Steiner. The number of registered places includes all rooms inclusive of the co-worker’s rooms. The community feels that it works best with 12 residents. Voluntary co-workers live on site in the two main buildings with the residents, providing 24-hour support. Bedrooms are large with very pleasant views over the land. A large emphasis of the day-to-day living is based upon tending the land and the animals. There is a large landscaped garden with numerous areas to sit and relax. There is also a gardener and a part time therapist. The service is not suited for people who have difficulties with mobility. The cost of placement is £896.00 per week and people who wish to consider the Cherry Orchards are given information about the community and the service before they visit. The Home will be referred to as the Community in the report to reflect the philosophy of the people who run it. Five of the nine residents living at the Community were consulted during the inspection. Time was spent talking to one of the registered managers, and to four of the support staff. The Inspector had lunch with residents and staff at their invitation. A sample of records that relate to the day-to-day running of the Community, as well as care records were inspected. The Community was viewed throughout. The Community was operating within the required conditions of registration, which we impose. The conditions of registration set out the type of care and the needs of persons as well as the numbers of persons who may stay at the Community. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3. Quality in this outcome area is good. The statement of purpose and service user guide ensure people have the information they need about the service. Residents are only offered a place in the Community when their needs have been well assessed and can be met. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Since the last inspection the statement of purpose and service user guide have been reviewed and updated .The documents now include the structure of the Community, as well as the roles and responsibilities of the staff. There is information about the service provided, the qualifications of the staff employed, and the accommodation. The philosophy of the Community and how the service will meet residents’ needs is included. The complaints procedure is in the document for residents to know how to complain. There are pictures of the Community to help inform the reader about the service. A copy of both documents, and a copy of the last inspection report are available for all residents so they can read them if they so wish. Two assessment records were read. An assessment had been written about each person’s physical, mental health and social needs. In the assessment was information about the social and therapeutic activities they undertake in the Community. There was evidence that showed the residents assessment records and care records are regularly evaluated and updated with the resident concerned. This helps to demonstrate residents are involved in meeting their needs while they live at the Community. Information about prospective residents is discussed by the managers. If it is thought that the person may benefit from living in the Community then this is discussed with staff at what is called a `care circle meeting’. All prospective residents will come to the Community for at least a month on a trial visits to see if they like it. Residents meet with a `key worker’ (someone who provides extra support) at least once a week. This is time for them to review how they feel their needs are being met, and any difficulties they may have, and how to resolve them. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9. Quality in this outcome area is good. Residents’ needs are very well met. There are informative care records that show how to support residents. Residents risk assessments help to promote their health and safety. This judgement has been made using available evidence including a visit to this service. EVIDENCE: To find out how effectively residents are being supported to meet their needs two residents care records and key worker meetings minutes were read. There was helpful information written about each person, with the full involvement of the residents concerned. Residents have a weekly one to one meeting with their key worker to review and discuss their needs. Cherry Orchards Camphill Community Ltd DS0000026530.V348560.R01.S.doc Version 5.2 Page 11 The care records and the key worker minutes show that residents work very closely with staff to plan what their goals are, and how they need to be supported to work towards them. All of the residents spoke positively about how the staff help them, residents said the staff were very kind and helpful to them. Two residents told the Inspector that they attend weekly review meetings with their key workers. They said they sometimes have reviews with the psychiatrist, and community psychiatric nurse. One resident said, ‘ It’s much better here then in hospital, they listen to you here ’. There was information included in the care records about the potential risks the person may face, and any risks from particular activities that they take part in both in and out of the Community. There was information written in care records that showed staff were aiming to support the residents to maintain their independence in their daily living. All of the staff the Inspector met talked in detail about the philosophy of the Community. They explained that one key principal is that everyone needs to be treated with respect and to be valued as a unique individual. All of the staff were observed treating residents with the up most respect and courtesy. Residents were observed getting up at different times during the morning, which helps to demonstrate how their choices and different preferences are respected. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 11,12,13,17. Quality in this outcome area is excellent. Residents are given excellent opportunities to participate in a varied range of therapeutic activities. A very healthy diet is provided for residents, staff and visitors in a calm relaxing atmosphere. This judgement has been made using available evidence including a visit to this service. EVIDENCE: There is a daily group each morning that all residents, staff, and visitors attend. This is used as time to share with other members of the Community what people are doing during that day, and any other matters people wish to share. There is also a range of groups and sessions that take place during the day. One resident said they attend the preserve making group and they said they enjoyed this. One resident told the Inspector that they regularly go to the nearby village of Westbury to use the computer at the library there. The Community grows much of its own food and most of this is organic. Residents and staff will take turns to cook the meals each day. The inspector was invited to join residents and staff for lunch. An important part of the philosophy of the Community is for staff and residents eat together every single day .The lunchtime meal experience was a very calm, relaxed experience. Some of the children of staff live at the Community and they also eat meals with staff and residents. There are many communal and quiet areas for residents to enjoy in the Community .The environment is considered to be vital for residents to have time and the right atmosphere to be in, for reflection. The Community grow their own food, and sell food produce they have grown at a weekly Farmers Market in Bristol. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20. Quality in this outcome area is good. Residents are being supported with their needs in the way preferred by them, and their needs are met. Residents ’ medication is being stored administered and disposed of safely. This judgement has been made using available evidence including a visit to this service. EVIDENCE: All of the residents expressed very positive views about the way the staff support them. Examples of comments made included, ‘ I really like it here’, and ‘you couldn’t be anywhere better ’. The procedures in place for administration, storage and disposal of medication were checked. All staff administering medication completes regular training to enable them to do this safely. The medication administration charts of three residents were inspected. The stock of medication held in the Community was satisfactorily organised. Medication no longer needed is being returned to the pharmacist. Cherry Orchards Camphill Community Ltd DS0000026530.V348560.R01.S.doc Version 5.2 Page 15 The medication administration charts were legible, up to date, and contained the signature of the dispensing member of staff. This demonstrates residents’ medication is administered safely. The reasons for any omissions had also been written on the charts. There are regular care reviews held in the Community involving residents, staff from the Community and the psychiatrist and team. All residents are registered with local GP Practices. The Psychiatrist continues to support residents with their mental health needs. There was information seen in residents care records from the psychiatrist, who gives advice and support to residents. One resident said that the Community Psychiatric Nurse was coming to take them on a camping trip. There was information written in the care records about the residents day to day routines, and what sessions and groups they are taking part in each week. This means residents are met in the way that suits them. There was information in the daily records that staff monitor and observe the health of residents and call the doctor, if they were concerned about the person. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 22,23. Quality in this outcome area is good. Residents are very well supported by the staff to make their views known and express any concerns or complaints they have. Residents are protected from the risk of abuse or harm. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The residents said staff listen to their views and will always respond to them. They said that if it is needed an extra meeting will be called with staff and residents to address concerns. The complaints record book was reviewed to find out how effectively residents complaints. There had been no complaint made since the last inspection. The staff and residents see each other every day and live together. The Inspector was invited to sit in on the daily morning meeting. This is used to make time for residents to discuss their needs and if they have issues or matters that concern them. This is a good way to support people to make complaints if they so wish. There are procedures and guidance information on the topic of ‘ the protection of vulnerable adults from abuse’. This helps to protect vulnerable adults who live at the Community if staff have the necessary information to ensure their protection. This training is beneficial as it makes staff have a better understanding of what abuse is, and should protect residents as a result. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 24,27,28,30. Quality in this outcome area is good. The Community environment is very therapeutic and beneficial for residents. The environment is satisfactorily clean and well maintained. The Home is suitable for residents to live in. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Cherry Orchards is situated in Westbury a suburb of the City of Bristol. The Community is near local shops, a church and bus routes into the City Centre. The building is situated in its own grounds, and consists of two properties, as well as staff accommodation and very spacious grounds .The grounds are used to grow food produce. The environment is exceptionally comfortable and homely. The environment was clean and tidy, and looked satisfactorily maintained in all areas that were seen. The Community is television free, which reflects the philosophy of the service. The communal areas are decorated in light colours. Residents were observed sitting in communal areas looking very relaxed and comfortable in the surroundings. The bedrooms do not have en suite facilities, but there are bathrooms and toilets located within close proximity to rooms. The majority of bedrooms and all the communal areas were viewed. The majority of bedrooms are for single occupancy. Rooms were satisfactorily decorated and maintained. Service users benefit from well supported and supervised staff. The Commission considers Standards 32, 34 and 35 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,36.Quality in this outcome area is good. Residents are supported by a sufficient number of competent, qualified staff who are supported and supervised in their work. This judgement has been made using available evidence including a visit to this service. EVIDENCE: The staff duty timetables were reviewed to see if there is sufficient staff on duty at any time to meet residents needs. The philosophy of the Community is that all staff are regarded as equal . Tasks are shared, and this includes domestic tasks and cooking. All of the staff with the exception of the administration assistant live on site. The staff can be available at anytime if needed. Based on the evidence from the inspection the number of staff on duty means residents’ needs are very well met. Three staff employment files were reviewed. There were two written references for each member of staff who have started work at the Community. There was also a Criminal Records Bureau check carried out. This helps to demonstrate that suitable people join the Community to work there. There were records seen that demonstrate staff are very well supervised and supported. There is a range of in house as well as outside training for staff. Residents also undertake food hygiene training alongside staff. The staff also meet weekly and take part in a group supervision sessions, which is also used as a useful way of sharing learning. Service users are confident their views underpin all self-monitoring, review and development by the home. Service users’ rights and best interests are safeguarded by the home’s policies and procedures. Service users’ rights and best interests are safeguarded by the home’s record keeping policies and procedures. The health, safety and welfare of service users are promoted and protected. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 37,38,39,42. Quality in this outcome area is good. Residents’ benefit from having three very experienced managers of the Community. Residents are very well supported to raise issues with the management. The health and safety of residents and staff is satisfactorily protected. This judgement has been made using available evidence including a visit to this service. EVIDENCE: Stephen Sands, Valerie Sands, and Gitte H. Knudsen are registered as managers of the Community .The managers live on the premises, and it is clear there is very good communication, and support between all three managers. All three managers have completed a range of management qualifications. Cherry Orchards Camphill Community Ltd DS0000026530.V348560.R01.S.doc Version 5.2 Page 23 The career records of the managers show they have a number of years of experience working with people who have mental health needs, and drugs and alcohol dependency problems. This demonstrates the managers are suitable and qualified to fulfil their role of registered manager. All of the residents who spoke to the inspector were very positive about all three managers. The philosophy, and the principles and values behind the running of the Community have been written about already in the report. The outcome for residents is that the Community has a very open atmosphere. Residents are fully involved in the running of the Community, and staff seeks their views. Residents ’ records are kept in a locked metal cabinet in the office. The care records and records that were seen relating to the running of the Community were satisfactorily written, legible, up to date, and satisfactorily maintained. This helps to demonstrate confidentiality is being protected, and also shows legal records required for the running of the Community are in order. The monthly monitoring visits of the Community that must be carried out by a representative of the Community are being undertaken as required by law. The records that have been seen, demonstrate that the designated individuals responsible for the visits spend time consulting with residents and staff. As has been referred to already in the report there are meetings or ‘gatherings’ held everyday in the Community. There are also weekly keyworker meeting for each resident with his or her key worker. These meetings are a very effective ways of monitoring the quality of the service. The environment looked safe and satisfactorily maintained in all areas viewed. There is a record of the monthly checks of the environment. These checks show that a member of staff audits the health and safety of the Community environment on a regular basis. Staff undertake regular training in health and safety matters including first aid, and moving and handling practices. This should help protect residents’ health and safety if staff are knowledgeable and well trained in health and safety matters. Staff are checking the temperatures of all high-risk cooked food before it is served to people to make sure it is hot enough and safe to eat. The fire logbook record was checked and showed the required weekly and monthly tests of the fire alarms and the fire fighting equipment were being carried out and were up to date.
