Date of last inspection Brief Description of the Service: Conifer Lodge is a care home for older persons, providing accommodation and personal care for up to twenty residents. The home is situated in Wigston Magna and is close to shops, pubs and other local amenities. Accommodation is on two floors with the upper floor accessed by lift or stairs. There are four lounges available to residents, two of which include dining space. All bedroom accommodation is provided in single rooms. The home has a decked garden area to the rear and to the front of the property and a large car park is available to visitors. Current charges are £445.00 per week and additional charges are in place for hairdressing and chiropody treatment. Details of all charges can be found in the homes Statement of Purpose document (a document which provides relevant information about the home) which is available to all prospective and current residents. This is an overview of what the inspector found during the inspection. This was an unannounced visit, which took place over a five hour period on Thursday 18th May 2006. When undertaking key inspections the Commission for Social Care Inspection (CSCI) focuses on the outcomes for residents living in a home. In order to do this, the inspector ‘case tracked’ three residents. This means the inspector checked their care records and met with them. Where communication was difficult, observation was used to evidence whether care needs were being met. The inspector talked with staff on duty at the time of the visit and observed them going about their daily work. The inspector also had the opportunity to talk with five other residents, four visitors to the home and the registered manager. Correspondence received since the last inspection and the last inspection report have also been taken into account when producing this report. What the service does well: What has improved since the last inspection?
The manager has completed the Registered Managers Award and the senior care assistant has commenced the NVQ (National Vocational Qualification) level 4. The manager has also retrained to be an NVQ Assessor; this will enable her to assess care workers completing an NVQ qualification. New care plan documentation has been developed since the last inspection. This new paperwork will provide a brief history of the resident and a more thorough picture of their individual care needs. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3,5,6 Quality in this area is good. This judgement has been made using available evidence including a visit to this service. All prospective residents have their needs assessed before moving into the home. EVIDENCE: The registered manager explained that all prospective residents are assessed before they move into the home to ensure that their needs can be met. On checking the paperwork/files belonging to two of the three residents case tracked, both were found to include an assessment of their needs completed by the residents social worker and/or the registered manager. A relative visiting on the day of the visit explained that someone from the home had visited their relative before they moved in and that she had been given the opportunity to look around the home to decide if it was the right place for their relative to live.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Quality in this area is excellent. This judgement has been made using available evidence including a visit to this service. Residents are looked after well, they are treated with respect and their right to privacy is upheld. EVIDENCE: The registered manager ensures that there is a plan of care in place for all residents living in the home. Care plans looked at during this visit were found to be up to date and had been reviewed with the residents themselves. The manager is in the process of transferring all the current care plans onto new paperwork that she has developed. This will provide a more thorough picture of the resident’s needs and identify actions to be carried out by the care staff. Conifer Lodge DS0000001677.V295387.R01.S.doc Version 5.2 Page 11 On checking the daily records kept by the home it was evident that health care services including GP’s and community nurses are accessed on the residents behalf. Staff working at the home are well aware of the individual care needs of the residents and all residents and relatives spoken with stated that individual care needs were currently being met. All residents and relatives spoken with felt that the care workers treated them with respect and throughout the visit care workers were seen interacting with the residents in a professional and dignified manner. Comments received during the visit included: “They are ever so kind, you couldn’t find a better home”. “It’s a lovely, homely home”. “They let me have my independence”. “When I have a bath they wash my legs then ask if they should leave while I do my front, they are lovely”. “The care is good, it’s brilliant, I can sleep in peace knowing that xxxx is here” Conifer Lodge DS0000001677.V295387.R01.S.doc Version 5.2 Page 12 Daily Life and Social Activities

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Quality in this area is excellent. This judgement has been made using available evidence including a visit to this service. There is a relaxed and homely atmosphere within the home and visiting is encouraged to enable residents to maintain contact with relatives and friends. EVIDENCE: Residents are offered choices on a daily basis. Choices include when to get up or go to bed, what to wear, when and where to eat their meals and whether to join in activities provided. An activities coordinator is employed three days a week and regular activities are provided. On the day of the visit residents enjoyed playing puzzles and card games in the morning and joining in a music session in the afternoon. Other activities offered include arts and craft, music and movement, outside singers and a boat trip has been arranged for June this year. Other comments received during the visit included: “I can come at any time”. “I can come and go as I please” “They put me at ease and tell me what’s going on” (with regard to their relatives care). The meals in the home are good offering choice and variety. All residents spoken with stated that they enjoyed the food. Comments received included: “The foods good, I like my dinners”. “The food is very good here”. “The plum crumble is lovely”. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Quality in this area is good. This judgement has been made using available evidence including a visit to this service. Residents and their relatives are confident that their concerns will be listened to, taken seriously and acted upon. EVIDENCE: A complaints procedure is in place and all residents spoken with were aware of who to go to should they have a concern of any kind. One resident stated: “I would go to her” [the manager]. A relative spoken with stated: “Id speak to Anne [the manager] or xxx “[the senior care worker] The manager has received one allegation since the last inspection. This allegation, which was staff related, was correctly handled and investigated appropriately. There was no evidence to support the allegation and no further action was necessary. Care workers spoken with during the inspection were aware of what to do should they suspect any act of abuse and the manager is aware of the procedure to follow with regard to adult protection. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,23,24,25,26 Quality in this area is excellent. This judgement has been made using available evidence including a visit to this service. The standard of the accommodation within this home is very good, providing residents with an attractive and homely place to live. EVIDENCE: The home is very well maintained and suited to the residents needs. Decoration in the home is of a very good standard and furnishings in the communal areas are domestic in character and in good condition. The rooms belonging to two residents were seen during the visit. Both were clean, appropriately furnished and included the residents personal belongings. All areas of the home seen on this occasion were clean, fresh and well cared for. One resident explained: “Its always nice and clean here”.

The decked area to the rear of the home is accessible via a ramp leading from the lounge area. One resident explained: “I can’t get out on my own, but they pushed me out there in my wheelchair in the nice weather, it was lovely”. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30 Quality in this area is good. This judgement has been made using available evidence including a visit to this service. Appropriate procedures for the recruitment of staff are in place to ensure residents are not put at risk. EVIDENCE: There were sufficient numbers of staff on duty on the day of the visit to meet the current needs of the residents. Residents and relatives spoken with felt that on the whole there were enough staff on duty to meet their individual needs and a staff member spoken with confirmed that staffing levels were sufficient to enable her to care properly for the residents. Two staff files were checked, both were found to include all the necessary information including an application form, two references and a criminal record bureau check (CRB). Training courses completed included: Emergency First aid Dementia Awareness Infection Control Moving and handling Food hygiene Safe Handling of Medicines Training in Nutrional awareness is arranged for June this year and a staff meeting is arranged for later this month to deliver in house training within the areas of: Data protection, Confidentiality, Security of the home and Health and safety in the home. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,35,38 Quality in this area is excellent. This judgement has been made using available evidence including a visit to this service. Resident’s benefit from the ethos, leadership and management approach of the home. EVIDENCE: The registered manager has many years experience in care and has completed her National Vocational Qualification level four and Registered Managers Award. Staff spoken with stated that they were well supported and positive relationships between management, staff, residents and relatives were evident throughout the visit.

It was evident throughout the visit that the residents benefit from the ethos, leadership and management that the manager provides. One resident explained: “You couldn’t get a better home”. All staff have received training in infection control and observations during the inspection confirmed that staff used appropriate protective clothing when carrying out their duties. Policies and procedures required for the safeguarding of residents are in place and all records seen on this occasion were accurate, relevant and up to date.
