Date of last inspection Brief Description of the Service: Conifer Lodge is a care home for older persons, providing accommodation and personal care for up to twenty residents. The home is situated in Wigston Magna, close to shops and local amenities. Accommodation is on two floors, the upper floor accessed by lift or stairs. There are four lounges available to residents, two of which include dining space. All bedroom accommodation is in single rooms. The home has a decked garden area to the rear and to the front of the property. Both areas are accessible to service users and seating is available. This is an overview of what the inspector found during the inspection. This inspection took place over three and a half hours and commenced at 10.40am on 4th October 2005. The main method of inspection used was ‘case tracking’ which involved selecting three service users and tracking the care they receive through the checking of their records, discussion with them, their relatives, the care staff and observation of care practices. The Commission for Social Care Inspection is inspecting Conifer Lodge for the eighth time and is in its third year of inspections being inspected against the Care Standards Act 2000. The senior care assistant was most helpful during the inspection process. What the service does well: What has improved since the last inspection?
Since the last inspection the rear garden has been developed. An enclosed decked area is now in place and new tables and chairs have been purchased. Residents are able to access the new garden area through the patio doors in the new lounge. Staff have completed dementia training to ensure that they have up to the minute information and knowledge to care for residents suffering with this illness. Health and Safety training and first aid training have also been completed. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,5 Standard 6 was not applicable at the time of the inspection. Prospective service users and their relatives are able to visit and assess the quality, facilities and suitability of the home. EVIDENCE: • A statement of purpose is available. This document includes information about the facilities in the home and the services that can be provided. One of the residents spoken with confirmed that she had received a copy of this document. Three resident files were looked at during the inspection. All included a copy of the initial assessment carried out by the registered manager and all included a copy of their contract/Individual placement agreement. One of the residents spoken with during the inspection explained that the registered owner had visited her in hospital to carry out a needs assessment and her relatives were able to visit the home and look around before she moved in.

The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10 Residents are looked after well in respect of their health and personal care needs and personal support is offered in such a way as to protect the resident’s privacy, dignity and independence. EVIDENCE: • • Three care plans were looked at during the inspection. All were up to date and all included a moving and handling risk assessment. The Procedures for the administration of medication were in order with all paperwork completed appropriately. The contracting pharmacist last checked the homes medication procedures on 31st January 2005 with no issues arising. Staff spoken with were aware of the individual care needs of the residents and all the residents spoken with stated that their care needs were being met. • Conifer Lodge DS0000001677.V255975.R01.S.doc Version 5.0 Page 11 • Discussion with residents and staff and observations during the inspection showed that the staff had a good awareness of how to ensure a resident’s privacy and dignity are maintained. Comments made by residents during the inspection included: “The staff are very very good” “They are all very thoughtful” “They come as soon as you send for them” “One girl is so sweet when she’s bathing me, she says, “I’ll do your back then I will wait outside while you do your front”. • Conifer Lodge DS0000001677.V255975.R01.S.doc Version 5.0 Page 12 Daily Life and Social Activities

Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15 Visiting is encouraged to enable residents to maintain contact with family and friends. The meals in this home are extremely good offering both choice and variety. EVIDENCE: • Residents are offered choices on a daily basis. Choices include when to get up or go to bed, what to wear, where to eat meals and whether to join in activities. Residents enjoyed a music and movement session on the morning of the inspection and a sing along had been arranged for the following day. Family and friends are encouraged to visit the residents. One resident spoken with stated: “It’s a very free home, family can come and go as they please”. A relative visiting during the inspection explained: “The staff are very caring, if you want anything special they will give it”. • • Conifer Lodge DS0000001677.V255975.R01.S.doc Version 5.0 Page 13 • The meals in the home are good offering choice and variety. All residents spoken with stated that they enjoyed the food. Comments received included: “The food is nicely presented” “We had some gorgeous soup the other night” “The food is really quite nice” “I really can’t fault the food” “I have the choice of having my breakfast in bed or in my chair”. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,18 Suitable arrangements for the receiving and responding to complaints are in place and residents are confident that their concerns will be listened to, taken seriously and acted upon. EVIDENCE: • • A complaints procedure is in place and all residents spoken with were aware of who to go to should they have a concern of any kind. The senior care assistant on duty at the time of the inspection stated that no complaints had been received since the last inspection in May this year. This statement was supported on inspection of the complaints file. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25,26 The standard of the accommodation within this home is extremely good, providing residents with an attractive and homely place to live. EVIDENCE: • The home is well maintained both internally and externally. The decoration and furnishings throughout the home are of good standard and are presented in a comfortable and homely way. The rooms belonging to the residents whose care plans were checked were seen. These were clean, appropriately furnished and included the residents personal belongings. One of the residents spoken with explained that the manager was happy for her to bring in her own furniture and piano if she wished. All areas of the home seen on this occasion were clean, fresh and well cared for.

Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,29 Sufficient numbers of staff are currently employed to meet the needs of the service users, however, current recruitment practices within the home could potentially put service users at risk. EVIDENCE: • • • There were sufficient numbers of staff on duty on the day of the inspection to meet the current needs of the residents. Through discussion it was evident that all the staff on duty were aware of the care needs of the residents case tracked. Two staff files were checked. Whilst one included all the necessary information, including two references, proof of identity and a Criminal Record Bureau check (CRB), the second, belonging to a newly recruited staff member had none of the required checks in place. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31,32,33,35,38 The home is managed effectively ensuring residents receive consistent quality care. EVIDENCE: • The registered manager is currently completing a National Vocational Qualification level four in care and management, and all staff and residents spoken with stated that she was approachable and supportive. It was evident during the inspection that the residents benefit from the ethos, leadership and management that the registered manager and owner provide. One resident explained: “You couldn’t find a better place than here”. A second resident stated: If I had to recommend a home this would be the one”.
