The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Conifers provides a clean, safe and hygienic environment for service users that is also pleasant and homely. The home is generally well decorated and the garden accessible and well maintained. Service users find the staff polite and friendly. The homes policies, procedures and staff training programme protect Service users. Care Plans are thorough and well organised.

What has improved since the last inspection?

Residents enjoy the activities that the home has put in place since the last inspection. The manager is planning to expand these further. New doors have been put on kitchen units improving the standard of safety and hygiene in the kitchen. A new medicine cabinet has also been installed to replace the trolley that was kept in a bathroom. The manager now ensures that care plans are reviewed monthly and that preadmission assessments are kept on service users` files.

What the care home could do better:

The on going programme of maintenance that the home has in place needs to include the redecoration of a stairway, bathroom and two bedrooms that are showing signs of wear and tear, to bring them up to the good standard in the rest of the building. Staff files need to include all the records required by the Standards.

Date of last inspection 14 October 2004 Brief Description of the Service: Conifers is a care home providing personal care and accommodation for up to twenty service users in the category of dementia, over sixty-five years. It is owned by Family Care Private Company Limited and managed by Mr Pelling. The home is located in Selsey and is close to shops and other amenities in this seaside town. The responsible person on behalf of the company is Mr Samarasekara. It is a detached property with accommodation on three floors. It consists of eighteen single rooms and one double room. There are sitting areas and a dining room. The exterior is well maintained with garden facilities accessible to service users. The announced inspection was carried out over a period of five hours. Prior to the inspection the inspector received, and noted the content of the preinspection questionnaire and five comment cards from relatives/visitors. The inspector also read information held on the service file since the last inspection in October 2004, and read the previous two inspection reports. During the inspection the inspector spoke to five service users and two members of staff. The inspector undertook a tour of the premises and looked at four care plans and six staff files. Various record books, policies and procedures were also examined. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 Service users’ needs are assessed before they move into the home. These assessments form the basis of the care plan. Conifers does not provide intermediate care so Standard six does not apply. EVIDENCE: Four service users’ files were examined during the inspection. All files held pre-admission assessments which form the basis of plans for care for daily living and risk assessments. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10 Service user’s health, personal and social care needs are set out in an individual plan of care. Their health needs are fully met. Service users say that staff treat them with respect and that their privacy is upheld. There are no service users responsible for their own medication. EVIDENCE: Service users’ care plans that were seen, clearly set out the action to be taken by care staff to ensure that all aspects of the health, personal and social care needs of service users are met. All care plans are reviewed monthly and signed by the manager. Short-term changes to care needs are recorded clearly and in detail. All health appointments and outcomes are recorded in a separate section of service users’ files. This enables the staff to monitor health needs and identify any patterns or changes that may be occurring. Records in service users’ file show that the home has regular contact with local community mental health services. Records show that nutritional and weight screening takes place prior to admission and is monitored continuously while residents are at the home. Medication records seen were up-to-date and correctly recorded. Due to the needs of the service users at Conifers, no one is currently self-medicating. A new medicine cabinet has been installed in the office to replace the trolley that was stored in a toilet. Residents spoken to said that their personal care is undertaken in private. One resident said that she can have difficulty understanding things now and that the staff take time to explain things to her. Residents said that they find the staff polite and friendly, and that they are addressed by their preferred name. Residents reported that they choose what clothes they are going to wear and always wear their own clothes. It was seen during the tour of the home that the shared room has a curtain dividing it to enable both residents privacy within the room. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15 Service users are provided with a choice of activities that they take part in if they choose to. Contact with family, friends and representatives is maintained and encouraged. Food is nutritious and looks appealing. The menu is changed regularly and snacks and drinks are available on request. EVIDENCE: Care plans record the social interests and hobbies of residents. The home has engaged upon a programme of activities since the last inspection. They hold quizzes once a week, have film evenings, organise trips to the pub, an entertainer visits every three weeks and one member of staff was seen to be doing needlecraft with a group of residents. The manager said he is planning on hiring a minibus to take residents out on trips during the summer. He is also trying to arrange someone to visit the home to do handicrafts with the residents. Two residents spoken to said they have taken part in the quizzes. One said she particularly liked the entertainer. Service users said that they have family and friends visit them at the home. One resident had her husband with her on the day of the inspection. Residents said that they like the food prepared by the home. On the day of the inspection dinner was seen being prepared and served. The meal smelt Conifers H60-H11 S46710 Conifers V223191 090605 Stage 4.doc Version 1.30 Page 12 good and was well presented. Staff were seen assisting service users eat their meals where needed. One service user said that the residents could ask for drinks and snacks at any time and the staff would get them. The menu sent with the pre-inspection questionnaire listed mainly plain but wholesome food that is nutritionally well balanced. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18 The complaints procedure is clear and complaints are dealt with within the required time period. Service users are protected from abuse by the home’s Adult Protection policy and whistle blowing policy. EVIDENCE: The complaints book was examined on the day of the inspection. One complaint was recorded. This had been responded to within the required time period. The home’s policies and procedures for Adult Protection and whistle blowing were seen and found to be clear and adequate for the purpose. A member of staff spoken to had a good understanding of Adult Protection issues and what to do if they had concerns about another member of staff’s practice. The home has a copy of the West Sussex Multi-Agency Policy and Procedure for Protecting Vulnerable Adults. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 26 Conifers provides a safe well maintained environment that is clean, pleasant and hygienic. The decoration of the home is mainly of good standard but some areas require attention. There remains an issue with the siting of the laundry but a policy is in place to minimise risk. EVIDENCE: On the day of the inspection a company was at the home inspecting fire safety appliances. A tour of the premises showed that the home was very clean and hygienic, radiators were covered and windows were restricted. New doors have been put onto the kitchen cabinets as required in the last inspection. Records inspected included electrical safety PAT testing, which took place in December 2004, and the Environmental Health Certificate. The home received an unannounced inspection from the Environmental Health Officer in April 2005. No recommendations were made from the inspection and the certificate states that on the day of the inspection “the standard of hygiene and cleanliness was excellent”. Conifers H60-H11 S46710 Conifers V223191 090605 Stage 4.doc Version 1.30 Page 15 Decoration within the home was generally of a good standard although one stairway, one bathroom and two bedrooms were showing signs of wear and tear, consideration should be given to redecorate these within the near future. The fitting plate of a bath chair hoist is starting to rust; this needs to be addressed, as it could be an infection control risk. The lounge has recently been redecorated and some new flooring was evident in communal areas. The outside of the building was being repainted on the day of the inspection. Access to the laundry is through the kitchen, although there is a policy in place informing staff to use a route outside the building to the laundry. A member of staff spoken to was aware of this policy and said that it is adhered to by staff. Mr Samarasekara is planning an extensive refurbishment of the property, which when complete, will address this issue. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29, 30 Staff at Conifers have the skills, competence and training to enable them to meet the needs of service users. The recruitment policy is thorough but the manager needs to ensure that all records required by the regulations are kept on staff files. EVIDENCE: Rotas show that adequate staff are on duty to meet the needs of service users. Domestic and maintenance staff are employed as well as care staff. Training records show that staff have undertaken a number of training courses to enable them to understand and meet the needs of residents as well as provide a safe environment for them to live in. Training undertaken includes; Abuse Awareness, First Aid, Food Hygiene, COSHH, Manual Handling, Confusion and Dementia and Fire Safety. Case tracking of staff files highlighted that some files do not include proof of identification, although the manager would have seen proof of identification to apply for CRB and POVA checks. The manager should keep photocopies on staff files. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35, 38 The health, safety and welfare of service users are promoted and protected, and their financial interests are safeguarded. EVIDENCE: Due to the needs of the residents at Conifers no resident is able to control their own finances. All current residents are either subject to Power of Attorney or Guardianship. The manager does not act as appointee for any of the service users. Staff records show that staff receive training in Health and Safety. Certificates and records were seen that show that fire, gas, and electrical equipment in the home are tested regularly. Water temperatures are checked weekly and a sample for Legionella testing was sent in May. The result had not been received on the day of the inspection.
