This inspection was carried out on 11th October 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Observations of staff interacting and working with the service users demonstrated that giving the client time to express themselves, negotiating and offering calming strategies and support was a main feature. The service users, staff and management appeared relaxed and at ease with each other. The service users are encouraged to express choice and preference in all aspects of their daily living and may lead full and active lives. A friendly and trained staff team supports the service users.

What has improved since the last inspection?

The home has aimed to provide a consistent service that meets the needs of the service users on an individual and group basis. There were no actions relating to NMS recommendation or requirements to be completed by this visit.

What the care home could do better:

The inspector considered the day-to-day interaction and work undertaken by the staff team to be of a good standard. There were some areas of administration and documentation that would benefit from further development. These were discussed with the management.

Parkcare Homes no2 (Craegmoor Healthcare ) are the registered providers and Mr K Yarnley is currently the registered manager in the day to day control of the home. The home is a large detached property situated adjacent to the seafront promenade. The accommodation comprises 8 single bedrooms on the ground and first floor. There are no en suite facilities or washbasins in the bedrooms. The service users have access to two day rooms and the kitchen area and the use of 4 WC’s, 3 bathrooms and a shower. Externally there is a reasonably well-maintained garden to the rear of the property that backs on to the local golf course. Local facilities (e.g. shops, pubs, church etc) are approx 1 mile away in New Romney with further facilities in the nearby towns of Ashford and Folkestone. The home provides a reasonably clean, tidy, comfortable and varied environment for the service users. Communal areas and bedrooms appeared to be adequately furnished and. reflected service user need, choice and involvement. The Inspector was able to speak with the Manager and staff on duty and observe staff interaction with a number of service users. There was limited opportunity for communication directly with the majority of the service users. The Inspector also viewed a range of records and toured parts of the premises. Any areas where action is required or recommended in order to comply with current National Minimum Standards are incorporated in the report. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 2. There is a system in place for undertaking pre-admission assessments of prospective service users and an understanding of the need to ensure compatibility with the existing resident group EVIDENCE: With the exception of one recent admission and one departure, the service user group have all lived at Cotswold Lodge for a number of years. The manager is aware of the level of detail required by the NMS relating to admissions. The company admission procedure and process allows for a protracted introduction and assessment period prior to any admission. It includes; offering opportunity for visits, overnight stays and re-evaluation before the final agreement and encourages the involvement of family and representatives in all stages. Service users know their assessed and changing needs and personal goals are reflected in their individual Plan. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate in, all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10. Service users are enabled to make decisions and express choices in their daily life. Risks are generally well managed. The Manager needs to ensure that fully documented justification is in place when restrictions to the freedom of movement and liberty of service users are imposed. EVIDENCE: Service users are given opportunity to express individual and group choices in all matters of daily routine and lifestyle. It was evident from observation and records seen that service users are encouraged to make choices on a daily basis and to express their willingness or reluctance to partake in any given event or activity.. ‘Longer term’ decisions may involve the individual’s family and care manager. Staff were observed interacting with service users in a respectful, friendly and non patronising manner. Efforts were made to include those with limited verbal skills in any interactions. There are reportedly few set routines in the home. A choice of day care programme is offered daily and service users indicate yes or no to inclusion. Risk is regarded by the management as part of living within the home and community. Any restrictions imposed also need to be documented. Staff are expected to respect information given in confidence by service users, and handle information about service users in accordance with the home’s written policies and procedures. There was no public display of private or personal information and records are securely stored away from communal areas.. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15,16 The home offers a range of recreational and leisure activities based on personal needs, wishes and interests. The home encourages service user family contact, visits and participation in the decision-making process. EVIDENCE: The manager reported that due to the complex nature of their presenting problems it is not appropriate for service users to pursue further education or employment opportunities. All day care is recreational with the exception of one college numeracy course attendee. Service users are offered daily access to a range of activities. They are encouraged but not compelled to join in and may decline participation if they so wish. In addition to the range of activities offered residents are encouraged to participate, where able, in domestic and catering tasks. The home makes use of the local facilities and services as far as possible. These are however somewhat limited. They endeavour to maintain good neighbourly relationships within the community but the property is relatively isolated. The home has two unmarked MPVs which are used daily to transport service users to various venues. It was reported that service users are free to come and go within and outside the home. Restrictions are placed only in the interests of individual safety and security. Risk assessments should contain justification for any restrictions imposed. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 20 The system for handling and management of medication is generally sound. Some minor improvements were discussed with the management. EVIDENCE: The arrangements for storage and administration of medication were satisfactory and up to date. The Management agreed to provide for easy access, a protocol for mis-administration and a copy of staff signatures.
