What the care home does well

The Management team and staff continue to make improvements to the home, which benefits all the people living there.

What has improved since the last inspection?

The improvements identified at the time of the last inspection continue to be implemented. Admissions to the home continue to be well managed with all prospective residents being visited prior to moving in the home. All of the communal lounges and dining areas are very well decorated, clean and tidy. The two dining areas in the home are particularly improved, chairs have been replaced with new ones and the dining tables are now presented with linen table cloths, napkins, condiments and flowers. The staff team in the home is consistent, less agency are employed. Staff communication continues to be improved as does the clinical supervision. Active recruitment has taken place to fill vacancies. Corridors throughout the home have been decorated this has made them far more homely in their appearance.

What the care home could do better:

The present management team has a firm plan in place to make further improvements. Given the size and geographical lay out of the home, commendation must be given for what has already been achieved within a relatively short time scale.

Date of last inspection Brief Description of the Service: Cottage Christian is a purpose built Care Home registered to provide both personal and nursing care for up-to a maximum of 40 Older People. This may include Residents with a physical disability and people with terminal illness. The Home is owned by Southern Cross Healthcare and managed by Mrs Beverley Harvey-Howson, who is a Registered Nurse. The Home is situated near the centre of the small Shropshire Town of Newport and enjoys pleasant gardens, which are readily accessible to Residents, including those requiring a wheelchair to aid mobility. Shops are to be found nearby and a modern Health Clinic is immediately adjacent. This inspection took place over a period of four hours it was unannounced and formed part of the two routine inspections per year undertaken by the Commission for Social Care Inspection. During the inspection a tour of the premises took place, and discussions with management, residents, and staff occurred. The care records of residents were perused together with the medicines administration system. The records indicated that the home continues to have the capacity to meet the needs of the current users of the service in a commendable manner. The Manager and staff on duty were welcoming and helpful throughout the inspection. It was found that all of the National Minimum Standards assessed had been met. It was considered that the overall quality of care provided was good. All residents appeared happy, content and very well cared for and those who were able expressed complete satisfaction with their quality of life at the home. Visitors, relatives and all visiting professionals have also expressed satisfaction with the service and care the residents are receiving and have been complimentary regarding the management and care practices at the home. Comments received from questionnaires used as part of the inspection process included: ‘A pleasant friendly atmosphere.’ ‘Staff are always polite and friendly. My mother seems in better health physically since she has been there.’ ‘My wife always looks clean, her hair and clothes are tidy.’ ‘Excellent care by friendly staff.’ ‘ I carried out a number of reviews of our service users within Cottage Christian, positive responses from the service users, no identified problems, care plans working very well’ ‘Very helpful when dealing with enquiries about placements. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1 to 5. Service users are assessed before moving into the home. Relatives are provided with the opportunity to visit the home to assess its quality, facilities and ability to meet an individual’s needs prior to admission. The homes statement of how they operate and the Residents User Guide provide present and prospective service users with detailed information which would assist them in making a decision on whether or not they wish to become a resident in the home. The personal records of the residents in the home were very detailed indicating that the home continues to have the capacity to meet the needs of the residents in a commendable manner. EVIDENCE: Pre-admission assessment documentation was found in all Resident’s files examined. The assessment documentation indicated that service users had been visited prior to admission to the home. Residents who have EMI needs are assessed by a community RMN nurse. From review of a resident recently Cottage Christian Nursing & Residential Home DS0000065121.V261506.R01.S.doc Version 5.0 Page 9 admitted it was evident that the admission had been well planned to the home, with necessary equipment identified and in place for the particular resident. Duty rotas showed staff numbers and staff skill-mix to be in accordance with the Statutory Staffing Notice. Staff files demonstrated evidence of training relevant to ensuring that staff are kept up to date in their practice. The Homes Complaints Procedure is clearly outlined, as is the procedure for obtaining a copy of the most recent Inspection Report. Prospective Residents are only admitted to the home following a care needs assessment by the Registered Manager, or senior member of staff. Where the Residents requires nursing care this is managed by the Registered Nurses. The documented assessment, care planning and reporting of care delivery indicated the homes capacity to provide care to meet the assessed need. At the time of the inspection the number and skill mix of the staff on duty, and their deployment, was consistent with meeting the overall care task. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7 to 11 The health and personal needs of residents appear to be well met with evidence of regular review and of good multi disciplinary working taking place on a regular basis. The Residents Care Records were well written, the system of medicine administration was observed to be well managed and safe. It was felt by the inspector that the home had met all the standards listed in health and personal care. The advice given by the Pharmacist Inspector on his visit in March 2005 had been adhered to. EVIDENCE: Cottage Christian Nursing & Residential Home DS0000065121.V261506.R01.S.doc Version 5.0 Page 11 It was evident during the inspection from looking at records, inspecting the facilities and chatting to staff, visitors and residents that individual health, personal and social care needs were being met. Staff who were spoken to during the inspection were extremely knowledgeable regarding the residents in their care. A Key worker system appears to be working very well, with staff very committed to their roles. Securing, administration and the recording of medication appeared satisfactory. Care and assessment regarding the prevention of development of pressure ulcers is good. The home has established close links with the District Nurses and Tissue Viability Nurse. Residents were being treated with respect and staff were working both professionally and sensitively in meeting individual needs. Those residents and visitors spoken to were complimentary regarding the quality of their lives at the home and comments received from the Social Work team (see summary at front of report) indicated maintenance of care at the home. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12 to 15 The routines of daily living at Cottage Christian are flexible and each resident finds the lifestyle experienced in the home meets their individual needs. Many activities take place, there is an open visiting policy and the menu offers a choice of well balanced and wholesome meals. The home has good links with the local community which enriches the social life of the residents through the homes activities coordinator. EVIDENCE: The residents are encouraged and enabled to personalise their bedrooms, enjoy good meals in the pleasant dining areas or in their own rooms and have a number of activities arranged for them within the home and outside. Individual needs, likes and dislikes are clearly shown in the care plans. Residents are certainly enabled to exercise choice and control over their own lives as far as they are able and there is a good range of information for residents and visitors within the home. Visitors are made welcome, are included in events and are given all the necessary information on aspects of the home and the welfare of the residents. Visitors spoken to were complimentary regarding the quality of life for the residents at the home. Religious Services are held three times per week for those residents who wish to be involved. The food menu for the residents showed a good range of “ traditional meals” and the residents engaged in conversation told the inspector they enjoyed the meals they were offered and that they were adequately portioned to suite their appetite. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Concerns and complaints are dealt with promptly and professionally and robust procedures and practices are in place to ensure that individuals are protected from abuse. EVIDENCE: The home has a clear complaints procedure which is given to residents and their relatives before they move into the home. One complaint has been received since the last inspection by the CSCI. Minor concerns, received by staff at the home, from residents, are dealt with in a professional manner without delay. The home has all necessary documentation in relation to the protection of vulnerable adults and this subject has been included in staff training. The Homes Complaints Procedure was displayed throughout the home, and the ethos presented by the home is one that encourages, and enables the residents and their families to make suggestions and make comments before they make a complaint. Monthly residents and family meetings are held which are minuted, and the manager has an open surgery once per week. The residents engaged in conversation with the inspector had no complaints about the home, and could not foresee any circumstances in which they may need to do so. There is an Adult Protection Policy and Procedure in place, with which staff are fully conversant and are able to operate effectively. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19 to 26 The standard of the environment within the home is good, providing residents with a warm, safe and homely place to live All the residents rooms are individually personalised to their taste and layout. Externally the grounds are easily accessible to the service users. EVIDENCE: The location and layout of the home is suitable for elderly residents. Communal rooms are well equipped and are warm, homely and welcoming. Many improvements have been made to the lounge and dining areas. All of the dining room chairs have been replaced with new ones and both dining rooms have been redecorated. Bedrooms are personalised and suit individual needs and the gardens and grounds are attractive and well maintained. Further plans have been identified by the management team, these include replacement of carpeting and flooring and refurbishment of bathrooms. At the time of this inspection the standard of hygiene and cleanliness was commendable. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27 to 30 Staff numbers and skill mix on duty were consistent with that shown on the rota and were sufficient to meet the assessed care needs of the current Residents. EVIDENCE: The current staff team, including qualifications of staff and their experience was fully discussed with the Manager. It appears that there has been considerable progress in the recruitment and retention of staff over recent months which has resulted in a strong committed staff team. Staff personal files seen during the inspection showed evidence of full compliance with the Standard and Schedule 2 of the Regulations. Discussion regarding staff training indicated all Staff are subject to a thorough, and relevant orientation/induction programme, which is followed by comprehensive ‘foundation’ training e.g. manual handling and lifting, fire safety, simple infection control and opportunities for ‘developmental’ training, including access to NVQ Courses. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 31 to 38 There are clear lines of accountability within the homes management structure and the management approach creates an open and positive atmosphere. The home is organised in the best interests of the residents and their financial interests. Health, safety and welfare are protected at all times EVIDENCE: It is evident from the main body of this report that the present management structure is effective. All the improvements that have been noted are as a direct impact of the management. The structure of the home, décor, atmosphere, committed staff team, individualised care, key worker system, communication and staffing levels have improved meaning that the overall quality of care is good. The Manager and her team all communicated a sense of motivation and commitment to further drive standards at the home forward. A range of approaches contribute to the development, establishment and maintenance of a good quality service, including questionnaires which are sent out to 10 of the residents and their families once a month.
