What the care home does well

Members are well supported by staff, as individuals and as a group. The ethos of the home is to promote members rights as individuals and to ensure that they are enabled to lead active and satisfying lifestyles and where they wish, to achieve their potential for independence. The atmosphere of the home is calm and nurturing, and conducive to members` happiness and wellbeing. The staff at the home are well supported by the organization (The Registered Owners, Delos Community), which shows a commitment to professionalism, development and good outcomes for members.

What has improved since the last inspection?

Staff and members commented on a number of improvements since the last Inspection. Members` involvement in the running of the home is being enhanced through increased use of visual and communication aids, and staff are committed to this process. Staff commented that the organization in a number of management areas is much better. There has been an increased, and successful, focus on encouraging members towards healthy eating patterns; the medication audit process has been improved; and staff have been given delegated areas of responsibility such as Diet and Nutrition, Social activities and Report planning. Comments were received on Feedback cards noting that members and staff seemed happier since the new Manager has been in post, that activities, support towards independence, and the welcome given to visitors are all appreciated.

What the care home could do better:

Little advice was necessary from the Inspector as there was good evidence that the professionalism of the organization ensures that members wellbeing and happiness is central to the ethos of the organization. There is a good overview, issues are addressed as they arise, feedback and Quality Audit processes are taken seriously, and development is a central theme. Advice given on this inspection was minimal and related to minor issues and best practice.

No person falling within the category MD may be admitted to the home unless that person also falls within category LD i.e. dual disability. 30th January 2006 Date of last inspection Brief Description of the Service: 34 Finedon Road is situated in a residential area close to the town centre of Wellingborough. The home is also known as Avonlea and is one of four registered homes within easy walking distance of each other supported by a Head Office and Day Centre in separate premises. Further support services are provided within the organisation for those moving on into a more independent lifestyle. The collective facilities are known as the Delos Community where residents are known as members. The home provides personal care and support for up to seven members whose primary care need is due to having a learning disability. The environment is that of a large family house. Members have their own bedrooms but there are no ensuite facilities. The range of fees is £621- £860 per week. The focus of inspections undertaken by the Commission for Social Care Inspection (CSCI) is upon outcomes for Service Users and their views of the service provided. This process considers the home’s capacity to meet regulatory requirements, minimum standards of practice; and focuses on aspects of service provision that need further development. The primary method of inspection used was ‘case tracking’ which involved selecting 2 service users and tracking the care they receive through meeting with the service users, a review of their records, discussions with the care staff and observation of care practices. The Inspector visited during the early morning period to observe practices by staff and to meet with members. Members have learning disabilities, and some have additional mental health needs, but most were able to communicate effectively with the Inspector. The Inspector also undertook observations of practice and members relationships with staff to establish their content with their lifestyles and routines, where verbal communication with the inspector was more difficult. The Inspector spent four hours in the home, and a further hour and a half (approx) in the central offices of the organization, meeting with the Manager and other centralized staff and reviewing the organization’s staff records and Quality Audit processes. Seven comment cards were received from members who were supported in completing their comment cards by staff, and ten feedback cards were received from relatives and visitors. Members’ admission to the home is supported through thorough and flexible processes that ensure that members will benefit from the placement and that the home can meet their needs. EVIDENCE: There have been no recent admissions to the home but processes were discussed with the Registered Manager and with staff. These evidenced that a professional and sensitive process are followed regarding preadmission assessment and admission, which leads to good outcomes for members. Policies and procedures support effective communication from the outset and the Service Users Guide, in the process of being updated, is enhanced by illustration. Members are enabled to enjoy a full and stimulating lifestyle with a variety of choices available to them. They are enabled to take reasonable risk in pursuit of their independence and such risks are well managed and monitored. EVIDENCE: Members talked to the Inspector about their choices within their lifestyles, often very enthusiastically, and expressed general happiness and content. Staff showed an awareness of the need to support and encourage less assertive members and were able to explain how they did this. Members’ decisions were evident within their care plans. Members are enabled to run their own meetings, and are consulted on and participate in all aspects of life at the home. There was a notable understanding and tolerance of each other’s ways, reflecting the guidance and understanding of staff. Members are enabled, after a proper assessment, to develop their potential for independence within a structure that properly balances the right to take risks with the right to be protected. This judgement has been made using available evidence including a visit to this service. Members are very happy with their various lifestyles, which are active and stimulating. EVIDENCE: Members talked to the Inspector about the things that they were supported to do and their enjoyment of these activities They participate fully in the local community and activities include regular daytrips and/or holidays according to their choices. Members spoke of, or showed their enthusiasm for their day placements, and showed the Inspector photographs of their holidays. Members are encouraged to maintain and develop good relationships based on mutual respect and consideration. Visitors feel warmly welcomed to the home. Members were aware of their rights and responsibilities and discussed how the rota for jobs was organized and undertaken. It was notable that members helped each other by, for example, offering to put on the shopping list an item asked for by a less able member. Members and staff expressed full satisfaction with the provision of food. Healthy eating patterns have been successfully implemented. Members are fully involved with choice, purchase and preparation of food according to their abilities. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18, 19, 20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Personal and healthcare support is good, and members’ physical and emotional needs are well met. EVIDENCE: Members expressed general content with the way in which they were supported and showed this in their interactions with staff during their early morning routines. Records demonstrated a good response to members needs. Community healthcare teams are appropriately accessed with or on behalf of members. Medication is very well managed and properly stored. Staff are given full information on the medications used by members, and there is a training element within many of the systems of the home. Advice was given that there should be evidence that the hand copying of prescription details onto Medication Administration Sheets has been checked by a second person to safeguard against mistakes being made. Members are protected by the quality of professionalism and the observation and listening skills of staff. EVIDENCE: Members said that they could approach staff or the Manager with their problems and showed confidence that they would be listened to and taken seriously. One member has recently been supported in making a complaint to an outside agency. The quality of care which includes listening carefully to members spoken and unspoken needs is likely to pre-empt complaints being made. There is no document on premises enabling members to have their complaints written down. Formal quality concerns and complaints are held in a file at the main office. Staff said any concerns mentioned by members were discussed and dealt with through the staff handover process. There is an ethos of empowerment at the home and this is supported by a policy for staff to familiarize themselves with. There is a prompt response to issues arising. Allegations are taken seriously, and most staff have had training in the Protection of Vulnerable Adults. The members’ house is homely, clean, comfortable and well maintained, suiting their needs and lifestyles. EVIDENCE: The home was seen to be comfortable, homely and clean at this early morning visit. Members showed pride in their house and in their individual bedrooms and said that they had chosen the decoration and some of the furnishings. Those who showed the Inspector their rooms expressed full satisfaction with the provision. There are no ensuite facilities at this home. This judgement has been made using available evidence including a visit to this service. Members are well supported by trained experienced staff. EVIDENCE: There were three staff on duty when the Inspector arrived and this was sufficient to fully meet the needs of members at that time. Staff were interviewed and showed themselves to be fully competent in their roles and aware of their responsibilities. The Inspector met with a recently recruited staff member and also discussed the recruitment process with the Registered Manager. This was found to be fully professional, properly recorded and reflected in policies and procedures. Staff training is ongoing. Staff expressed full content with the levels of training they were offered. Records on staff files were well organized, fully met the Standards and showed good levels of training and supervision of staff. A discussion with the Manager highlighted the need to ensure the suitability of staff being left in charge of the home; this was specifically in relation to the age of those individuals as the National Minimum Standards indicate that such persons should be at least 21 and this had not been met according to the information on the duty rota. This judgement has been made using available evidence including a visit to this service. Management of the home is effective and responsive and members thereby benefit from a well run service. EVIDENCE: There was a good level of organization within the home and access to policies and procedures was appropriate, and the confidentiality of records was safeguarded despite there being no “office” within the home. It was evident from discussion with the Registered Manager and with staff and members that the management of the home is being effectively undertaken and improvements are being implemented. A member expressed confidence that members’ views were included in improvement plans. Policies and procedures demonstrate that positive outcomes for members are the focus of the service and that the principles of good care are adhered to. Health and Safety is properly promoted. The Inspector told the Registered Manager that it would be advisable to have disposable hand towels in communal areas to safeguard against the spread of any infection.
