What the care home does well

The home provides a family style environment, which the members said they like. There has been one new admission this year, although most of the group have lived together for several years. The members are involved in the running of the home and staff supervise and assist them with cooking, and the domestic duties required within the home. The home was decorated to a reasonable standard and the members bedrooms that were inspected were personalised containing, personal items. There is a private garden at the rear of the home.Members were very positive about the services provided which include meals, personal care, activities, and also about the support that they receive from the staff, to maintain their independence. On the evening of the inspection relationships between Members and the staff were observed to be very good, and two members went out to a local pub for a meal, with a member of staff. Holidays are seen as a very important aspect of the overall care, provided in this home. Members were very excited about a forthcoming holiday that they were going on shortly, and had been involved in choosing and buying clothes and other essential holiday items. Staff interviewed said that they were encouraged to undertake training on a regular basis, so that they have the knowledge and skills to care for the needs of the residents

What has improved since the last inspection?

Since the last inspection the redecoration and refurbishment continues to improve on and maintain the standards of the environment: 3 bedrooms have been redecorated, 1 bedroom and a lounge re-carpeted and a new dishwasher has been installed. Staff training has included First Aid, Protection of vulnerable adults, manual handling and sign language. The home has introduced a flexible rota, so that additional staff are available to escort Members to activities of their choice, outside of the home.

Date of last inspection 15th December 2004 Brief Description of the Service: 34 Finedon Road is situated in a residential area close to the town centre of Wellingborough. The home is also known as “Avonlea” and is one of four registered homes within easy walking distance of each other supported by a Head Office and Day Centre in separate premises. The services offered are known as the Delos Community. Further support services are provided within the organisation for those moving on into a more independent lifestyle. The home provides personal care and support for up to seven Members whose primary need is having a learning disability. The focus of inspections undertaken by the Commission of Social Care Inspection is upon the outcomes for Members, and upon their views of the service provided. The primary method of inspection used was ‘case tracking’ which involved selecting 2 Members and tracking the care they receive through review of their records, discussions with them, and with the care staff, and observations of care practices. The inspection took place during an evening over a period of 3.5 hours and was carried out on an unannounced basis. Communal areas, and some bedrooms were visited. A selection of care records, and essential records of the home were reviewed. A number of the Members, living in the home, were spoken to as part of the inspection process. The Acting Manager was available for most of the inspection. The Commission had received a number of comment cards from residents and relatives prior to the inspection. In the main the comments recorded were positive and praised the home and the staff for the way in which the care is provided. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,2,3,4,5. The pre-admission process of all potential residents to the home is well managed. EVIDENCE: The Statement of purpose was not available as the Acting Manager said that it was currently being updated. The document has not been available for inspection since January 2004, and the acting Manager should ensure that a copy is retained within the home. The Service User Guide was well written, in a format that could be understood by most of the Members. One new member admitted within the last 6 months said that she was worried when she first came to live at the home, but staff had helped her to settle in and she now enjoyed living in the home with all of her friends. The Acting Manager said that there had been a long period of admission, for this new Member, with opportunities prior to admission, for visits to the home for meals and overnight stays, and to accompany the other Members to an evening social club. Meanwhile there was plenty of communication with parents to ensure that they were satisfied with the care being provided. There was no evidence within the file of a care management assessment although the Acting Manager said that all prospective Members are referred through a care manager. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,8,9, Member’s rights are respected and they are encouraged to be involved in decisions about the home, and in their own lives within the home. Risks are assessed and Members are given guidance and support to lead independent lives. EVIDENCE: Members were aware that records were kept about them, and said that they had been involved in the reviews. Two files were examined during the inspection. There was evidence of the Members signature when care plans were drawn up. One of the care plans was well written, and tasks required by staff were clearly defined. The other careplan was not available as a member of staff said that she had recently updated the plan and had sent it to the office for typing. The Acting Manager said that none of the Members currently need the services of an advocate, as families fulfil that role, however if the Members requested an advocate, assistance would be given to access an advocate service Delos Community Limited 34 Finedon Road D C51 S39665 Delos Community Limited (34 Finedon Road) V246032 080905 stage 4.doc Version 1.40 Page 12 Through the process of case tracking and observation of the staff interacting with the Members, there was evidence that the staff have the skills and experience to deliver the services and care the home has to offer. Staff communicated effectively with members, taking into account any communication difficulties. Staff were observed to be discussing with the Members their choices for the evening activities, and giving a member detailed reasons why it was not possible to agree to a request. The recorded risk assessments appraised were; for the most part very detailed and were reflected within placement plans with risk management strategies in place. Members were aware of areas in their lives that they needed to be “careful about”, they knew about the risks that had been identified and explained what actions they took to ensure that they were safe. One Member was quite clear, that he was not allowed to go out alone, as he may get injured on the road. During the inspection Members were observed to be given assistance and support to manage their own finances, and a sample of financial records reviewed were found to be satisfactory. Members spoken with confirmed that there are regular house meetings and said they were able to have their say at these meetings. The minutes of a recent meeting was inspected and indicated that items such as menus and activities were discussed. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 11,12,13,15,16,17 Member’s views are sought and acted upon in respect of their daily lives, and they are given opportunities to maintain appropriate and fulfilling lifestyles in and outside of the home. EVIDENCE: Within both of the files examined there was evidence that the Members attended a variety of day services, some provided by Delios, and others such as Teamwork, and Saunders Gate. One Member assists at the ‘Chat Shop’, a local café that is staffed by Service Users with learning Disabilities, providing drinks, snacks and meals to the local community. Members confirmed that they enjoyed their work placements and managed and controlled their own money that they earn. Members were enthusiastic in their description of the type of leisure and educational activities that they were involved with and described their participation in College courses, visiting the Cinema, Bowling, Knitting Club, Delos Community Limited 34 Finedon Road D C51 S39665 Delos Community Limited (34 Finedon Road) V246032 080905 stage 4.doc Version 1.40 Page 14 and learning to play the guitar. Evidence of their activities was provided by their weekly activity list located in their file. The Members have a day at home each week to do activities of their choice with their key-workers. The home has access to three vehicles shared with the other three houses run by the company. The Acting Manager said that at present no one had expressed an interest in attending one of the local churches, but if they did, visits would be arranged. During the evening of the inspection, a member of staff accompanied two Members to the local pub for a meal. All Members take part in the household duties, and are expected to make a contribution to keeping their rooms clean and tidy to encourage them to develop personal skills. The home enables Members to maintain contact with whosoever they wish to, and residents confirmed that friends visit the home; one Member said ‘My girlfriend comes to tea’. Parents are welcome to visit the home at any time and Members are encouraged to go home for weekend/ overnight stays if possible. Members confirmed they had unrestricted access to all communal areas and that they were offered the choice of where they wished to spend their time, and their wishes to be alone or in company were respected. Members are offered a well-balanced diet and take part in the menu planning, and purchase of food, and some of the members assist in the cooking of food and snacks. One Member prepared a cup of tea during the inspection. Members confirmed that they received three meals a day, 1 of which is hot, plus supper snacks and drink as they wish. The meal on the evening of the inspection consisted of shepherds pie, cauliflower, peas sweet corn, followed by jelly and ice cream. Likes and dislikes are recorded, and a record is kept of the food eaten, alternatives are offered if members do not like the meal on the menu. Members confirmed they are consulted and involved in menu planning. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 18,19,20 The management of residents’ health and personal care was supported by a sound risk assessment, and care planning process, with access to healthcare and professional support EVIDENCE: All of the members are able to manage their own personal care, however from observations made during the inspection and discussions with the staff it was evident that the staff are aware of privacy, dignity and independence issues. Records demonstrate that the Members followed for case-tracking purposes have access to a variety of health professionals. Records were maintained of the reason for the visit, and the outcome including any prescribed treatment. Planning was evident for annual checks with dentist and opticians, and although there are some local difficulties in accessing podiatry, private consultations had been arranged. Community nurses have delivered a health education program. One Member was particularly pleased that the staff had encouraged her to lose weight, and was very proud of her achievements. All members have a record signed by the Doctor, of homely remedies that can be given without prescription. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 22 Complaints are handled objectively and Members were confident that their concerns would be listened to, taken seriously and acted upon. EVIDENCE: A complaints procedure is available, that was written in a format that could be understood by most of the Members. A copy of the procedure was kept within the Service User Guide. Relatives commented through the feedback forms that they were not aware of the procedure. This was discussed with the Acting Manager who agreed to ensure that this matter will be addressed. Members spoken with were aware of the procedure, and one person said that he was confident that the Manager would do all she could to resolve issues raised. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24,30 The environment provides safe comfortable surroundings and is maintained to an acceptable standard. EVIDENCE: The standard of the environment was very good and the outcome that Members live in a homely, comfortable and safe environment is achieved. There is a large garden that is safe and accessible to Members. Bedrooms viewed were bright and cheerful and had been personalised by the Members assisted by the staff. There are no en-suite facilities at this home. One of the Members was particularly anxious to show his room that had been recently decorated and refurnished. Members have access to all communal areas and their rooms, and were observed to use all areas of the home. The home contains two lounges, one with a television, and one a quiet lounge where Members can relax to soft music. JUDGEMENT – we looked at outcomes for standard(s) 32,35 Staff morale is high resulting in an enthusiastic workforce that works positively with the Members to improve their whole quality of life. EVIDENCE: In discussions with staff it was clear they had a good understanding of their own job descriptions and that of other staff deployed with them. The arrangements for on-call support and management oversight of the home are appropriate, staff clearly knew what was expected of them while on duty and how they could call for support/consultation should they require clarification on any issue. The Staff training plan and members of staff, confirmed that the company have a strong commitment to staff training, although only a few staff have, obtained an National Vocational Training qualification. The pre- inspection questionnaire indicated that training delivered during the past 12 months includes First Aid, Learning Disability Award Framework, Protection of Vulnerable Adults, Sign Language, Manual Handling, Dementia, Fire Awareness, and Epilepsy. The duty system operates with one person on duty whenever members are at home with the flexibility of further staff, dependent on the needs of Members. On the evening of inspection an additional member of staff was employed to take two of the Members out for a meal to a local restaurant. Discussion with the Members and observations indicated that they were relaxed and comfortable with staff . Service users are confident their views underpin all self-monitoring, review and development by the home. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37,42. The management structures are adequate and effective in safeguarding the health, safety and welfare of the people using the service. EVIDENCE: The position of Registered Manager is presently vacant as the previous Registered Manager has other duties within the organisation. An Acting Manager who is awaiting her registration process is currently covering the post. Staff interviewed stated they feel there is currently an open and inclusive atmosphere within the home, and commented that they know what is required of them and feel able to make suggestions openly to the organisation. The provider undertakes visits as detailed in regulation 26 of the Care Regulations; it was noted that reports had not been submitted since November 2004, the Acting Manager said that she would ensure that these were made available for inspection purposes. A member of staff said that there was a good support system in place from senior staff on duty, at all times. The Delos community employs a health and safety officer for all of the four homes operated by the company. During the inspection the maintenance book was examined that provided evidence of weekly tests on water temperatures and on the fire alarms. Staff indicated that faulty equipment was quickly repaired or replaced. STATUTORY REQUIREMENTS This section sets out the actions which must be taken so that the registered person/s meets the Care Standards Act 2000, Care Homes Regulations 2001 and the National Minimum Standards.
