Conditions of registration: 1. 2. The home may only admit service users aged 18-65 No person falling within the category MD may be admitted to the home unless that person also falls within category LD - i.e. dual disability 19th June 2006 Date of last inspection Brief Description of the Service: 34 Park Road is situated in a residential area close to the town centre of Wellingborough. The home is one of four registered homes within easy walking distance of each other supported by a Head Office and Day Centre in separate premises. Further support services are provided within the organisation for those moving on into a more independent lifestyle. The collective facilities are known as the Delos Community where residents are known as members. The home provides personal care and support for up to four members whose primary care need is due to having a learning disability. The environment is that of a large family house. Members have their own bedrooms but there are no en-suite facilities. The range of fees is £500- £1380 per week. This was confirmed at the inspection by the Registered Manager. The fee is all inclusive and there are no extra charges. Further information about the home, including the most recent inspection report can be obtained from the Registered Manager. This is an overview of what the inspector found during the inspection. The focus of inspections undertaken by the Commission for Social Care Inspection (CSCI) is upon outcomes for Service Users and their views of the service provided. This process considers the home’s capacity to meet regulatory requirements, minimum standards of practice; and focuses on aspects of service provision that need further development. The primary method of inspection used was ‘case tracking’ which involved meeting with the two service users who were at home, and tracking the care they receive through a review of their records, discussions with the care staff and observation of care practices. The Inspector visited during the early morning to observe practices by staff and to meet with members. Members have Learning disabilities, and some have additional mental health needs, but members “case tracked” were able to communicate effectively with the Inspector. The Inspector also undertook observations of practice and members relationships with staff to establish their content with their lifestyles and routines. Two members were away from the home at the time of the Inspectors visit. The Inspector spent two and a half hours in the home, and has recently spent time in the Delos Central offices reviewing centrally held records and Quality Audit processes. Three comment cards were received from members and one from a relative, and the comments on these were taken into account as part of the overall assessment of the home. The Inspector noted that both care plans and risk assessments had all been appropriately updated. The Registered Manager and staff continue to look at what parts of the service can be developed and improved, and this commitment to good quality is evident in a number of areas. For example the Registered Manager is currently reviewing the Quality Audit system and revising members questionnaires to ensure a fuller more “in depth” survey. Staff training and supervision are recognized as important areas of development, which improve the quality of care. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 2 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Members’ admission to the home is supported through thorough and flexible processes that ensure that members will benefit from the placement and that the home can meet their needs. EVIDENCE: There have been no recent admissions to the home but discussion with the Registered Manager and a review of the information, which is collected and evaluated pre-admission, demonstrates a professional and effective assessment process. Arrangements for preadmission visits to the home are fully flexible according to need, and the professionalism of the process leads to a gradual introduction to the home for new members, and good outcomes for all concerned. Policies and procedures support effective communication from the outset and the Service Users Guide, recently updated, is enhanced by illustration. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 6,7,9, Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Members can be confident that their feelings, needs and choices are recognized as central to the care planning process and that they will be treated with respect and properly involved in the running of the home. Information on members care plans is excellent, being sufficiently detailed in all areas to protect members rights, promote best practice and ensure consistency of care. EVIDENCE: Members expressed content with the service that they receive from the home and said that they were very happy. Staff felt that members’ quality of life was exceptionally good and that members were fully supported in everything that they wished to do. Care plans were detailed in all relevant areas, prioritizing members’ views and choices and promoting best practice. They were reviewed regularly with the members, and information was made available in user friendly formats. The

Care files and personal information are stored securely and improvements have been initiated by the Registered Manager to make this storage more discreet, thereby enhancing the homeliness of the area. Members are involved in decision-making during the weekly house meetings and were also seen to approach staff confidently to assert their needs. It was clear from observation that members felt fully confident in their environment and were able to make choices (what to do, what to eat for breakfast) without any need to refer to staff for approval or guidance. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,15,16,17, Quality in this outcome area is excellent. This judgement has been made using available evidence including a visit to this service. Members are enabled to enjoy a full and stimulating lifestyle with a variety of choices available to them. They are enabled to take reasonable risk in pursuit of their independence. Members are very happy with their various lifestyles, which are active and stimulating, and they are fully supported by staff in achieving this. EVIDENCE: Discussions with members and staff and evidence in records showed the full and active lifestyles enjoyed by members. Members were seen to partake of the activities of their choice at the time of the inspection. They participate fully in the local community and their active lifestyles include work placements, daytrips and/or holidays according to their choices. Delos Community Ltd, 34 Park Road DS0000039663.V342027.R01.S.doc Version 5.2 Page 12 Members are part of the local community, visiting the local facilities, which are conveniently close, and can also share activities with their friends who are members in other homes owned by the organization. Members are encouraged to maintain and develop good relationships based on mutual respect and consideration. There was evidence in records of the ways in which members are fully supported to maintain contact with families and entertain their guests. Members’ rights and diverse needs are respected, for example members are enabled and supported to look after their pets, and there is a resident cat. One member was seen to be enjoying and benefiting from the comfort of caring for the cat. Members said that they enjoyed the food provision at the home; staff said the food is good, and that they share meals with members in a family type routine. Members’ individual needs in respect of their diets are fully recognized and respected. For example healthy eating and lifestyle habits are promoted for members, and vegetarian food is provided as needed. The inspector’s observations, records and discussions with the staff confirmed the evident good practice and the excellent outcomes for members in this area. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): 18,19,20 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Personal and healthcare support is good, and members’ individual physical and emotional needs are well met. EVIDENCE: Members expressed general content with the way in which they were supported in personal and healthcare routines, and said that they were happy with the help they got to see their doctors and attend appointments. The Inspector observed the early morning routine at the home and noted that staff were fully aware of members preferred routines as identified in the care plans and were meeting these preferences. From discussion with staff and from records it was apparent that service users healthcare needs are observed and responded to appropriately, and members are supported to access healthcare professionals.
Delos Community Ltd, 34 Park Road DS0000039663.V342027.R01.S.doc Version 5.2 Page 14 The Inspector discussed with the Registered Manager the very difficult judgements relating to the balance between promoting members rights to choice and privacy with regard to their healthcare, and maintaining an overview in accordance with the duty of care in a registered care home. The Inspectors judgement was that in the main this balance is well maintained at the home but potential risk areas were identified which breech best practice and the Registered Manager agreed to review and improve these areas. The medication system was well managed and the staff member administering medications at the time of the inspection had had the appropriate training, and was seen to be undertaking this duty in accordance with the homes procedures. The Registered Manager is currently reviewing the medication records with a view to simplifying the filing of and access to information. The intended outcomes for Standards 22 – 23 are: 22. 23. Service users feel their views are listened to and acted on. Service users are protected from abuse, neglect and self-harm. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Members are protected by the quality of professionalism and the observation and listening skills of staff. EVIDENCE: Members said that they could approach staff or the Manager with their problems and showed confidence that they would be listened to and taken seriously. Staff were aware of abuse issues and informed the Inspector that they are due to receive their update training on this in December. The Registered Manager said that no formal complaints have been received by the home since the last Inspection. Most concerns and complaints are dealt with at very early stages through the members meetings, which are supported by staff. The Commission for Social Care Inspection have not received any complaints about the home since the last inspection. There is an ethos of empowerment at the home and this is supported by a policy for staff to familiarize themselves with. There is a prompt response to issues arising. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The members’ house is homely, clean, comfortable and well maintained, suiting their needs and lifestyles. EVIDENCE: The communal areas of the home were seen to be comfortable, homely and clean. There is a pleasant garden area, which is being developed by a staff member, alongside members to enable them to grow some of their own vegetables and herbs. The Registered Manager confirmed that the overview and response to all maintenance issues at the home was good. This was recently inspected in relation to another home in the group at Delos central office with the staff member responsible for all the homes maintenance. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,35 Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Staff are caring and appropriately knowledgeable and members can be confident that their life at the home will be well supported by people who have their best interests at heart. EVIDENCE: Members told the Inspector that all staff, without exception were nice and helpful. Relations between members and the staff members on duty were seen to be working positively to promote members’ welfare and independence. The Inspector reviewed the staff training records recently at the Delos central office and found that training is undertaken appropriately. Discussions with staff on duty at the time of the inspection did not identify any training shortfalls. Recruitment processes are full and thorough and records evidenced this. The staff members on duty at the time of the inspection felt well supported by the training and supervision and management support at the home.

The Registered Manager described an effective and thorough supervision process. The staff member interviewed also showed confidence in the robust “on call” system whereby staff can be called to the home at short notice, and a manager of the organization is always available. This judgement has been made using available evidence including a visit to this service. Overall the management of the home reaches a high Standard. The Registered Manager is well supported by the organization, which has well-organized systems, checks and Quality Audit. EVIDENCE: The Registered Manager has much relevant experience and training and continues to welcome developmental opportunities and involve himself in professional projects. There was a good level of organization within the home, and the confidentiality of records was properly safeguarded despite there being no “office” within the home.

Policies and procedures of the organization demonstrate that positive outcomes for members are the focus of the service, and that the principles of good care are adhered to. Health and Safety is properly promoted. There are good systems and records to show the overview of premises issues, and staff files are well organized with the relevant information easily accessible. (These are held at the head office) The balance of members’ rights against the duty of care should be explored further and admission contracts in certain areas could be developed in relation to this.
