This inspection was carried out on 27th September 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

This is a homely, caring environment for those people who live there. The building is well decorated and furnished creating a bright and airy atmosphere. The staff were found to be caring and well motivated with the service users all speaking very highly of them. They were particularly complimentary about the food.

What has improved since the last inspection?

The monitoring of bathwater temperatures now takes place on a regular basis reducing the risk that residents will be scalded.

What the care home could do better:

The number of staff working during the afternoons and evenings is not enough to meet the needs of the service users living at the home at this time and this deficit must be rectified as soon as possible.

The Home is situated on the edge of the small Shropshire Village of Ditton Priors with most rooms enjoying uninterrupted views of the Shropshire countryside. It has extensive well-maintained grounds with many shrubs, trees, flower borders, lawns and patios, and an area where vegetable produce is grown for use in the Home’s Kitchen. The Home, purpose built as a residential care Home during the 1970’s, has since been extended to provide additional care places. Mr Gary Hartland is the Registered Responsible Person and Mrs Margaret Allsop is the Registered Manager. The Home is part of the ‘Guardian Care Homes’ Group. This inspection was unannounced and took place over two hours during the morning. Information was gained from documentation made available by the home, by discussions with the deputy manager, the staff and the service users as well as by reading documents kept within the home and by looking around the building. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 The home has an appropriate admissions assessment procedure. EVIDENCE: A number of new people had started to live at the home in recent months. The assessment system contained detailed information regarding their medical needs, social patterns and interests, personal care needs for both day and night. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, and 9 The personal and healthcare needs of the service users are met. Staff have received the training necessary to administer medication safely. EVIDENCE: Looking at the service users records showed that the pre-admission assessments enabled the home to create comprehensive care plans. These contained guidance for the staff on the range of assistance that each service user needs for both personal and health care. Talking to a number of the service users established that they were being cared for in the way outlined within the agreed plans. The home also has a system for storing and recording medication that complies with the guidelines issued by the Royal Pharmaceutical Society and staff have received training that contains all of the elements required by the National Minimum Standards for Older People. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): x EVIDENCE: None of the above standards were assessed on this occasion. JUDGEMENT – we looked at outcomes for the following standard(s): x None of these standards were fully assessed on this occasion. EVIDENCE: The deputy manager stated that there had been no complaints made since the last inspection nor had there been any referrals made in relation to abuse. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 25 and 26 The standard of the environment is good providing service users with an attractive and homely place to live. EVIDENCE: The home is situated in the village of Ditton Priors and is a property that has been developed to its present use in a sensitive and practical manner since it was originally built as a care home in the ‘70s. The home has its fully equipped laundry area situated so that access is through areas that are not used for food preparation or consumption thereby reducing the risk of cross contamination. The rooms were all seen to be bright and airy and kept at a comfortable temperature. Hot water temperatures were also seen to be monitored to reduce the risk of residents being scalded. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 29 and 30 The home has good staff recruitment and training practices, however, there are not enough of them available during the latter part of the day. EVIDENCE: Staff records contained appropriately completed application forms, two references were seen on all files and CRB disclosures had been obtained prior to staff starting work. Records showed and staff confirmed that they had received an induction that met the standard required by TOPSS. However, on the day of the inspection there were only two staff on the staffing rota for the afternoon and the deputy manager confirmed that this was normally the case. This is insufficient to meet the needs of the residents if, for example, both staff are required to carry out personal care on someone who requires the use of a hoist then there are no staff available to monitor and respond to the needs of the others. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 35 and 38 The home has safe systems for handling residents’ money. EVIDENCE: The home holds and records some monies on behalf of some of the residents. By checking the individual records against the amount held it was found that the system that the home uses is accurate, open and transparent ensuring that the money is managed honestly. A variety of documents were seen that showed that safety checks are carried out, incidents are monitored and safety training given to staff.
