Conditions of registration: 1. Limited to 30 people of either gender who fall into the category of old age (OP) and who may have dementia (DE(E)). 2. One specific service user who is under 65 years of age can reside in this home. The condition will need to be reviewed when he vacates the home. Date of last inspection 14 December 2004 Brief Description of the Service: Dr. French Memorial Home is a care home which is registered to provide care for 30 older people, some of whom may have dementia. The home has charitable status and is run by a voluntary committee. The stated aim of the home is; “To support residents to continue living as independently as possible by receiving care and support consistent with their incapacities and disabilities”. The home consists of a large two-storey, detached building. There are 24 single bedrooms and 3 double bedrooms, located on the first and second floors. Fifteen single bedrooms have en-suite facilities, and there are eight toilets, three bathrooms and a shower. On the ground floor, there are two lounges, a dining room, a hairdressing salon, an activities room and two offices.There is a large, secluded, attractive garden at the rear of the premises and a car park at the front. The home is situated near the busy Tally Ho Corner in Finchley, North London and there are good public transport links. This is an overview of what the inspector found during the inspection. This announced inspection was completed in seven hours. The lead inspector was accompanied by Wendy Heal, another inspector from the Commission for Social Care Inspection. The registered manager was present throughout the inspection and fully cooperated in the process. The inspection included a tour of the premises, talking to ten residents, two relatives and nine staff, including carers, cleaners and catering staff. Residents’ case files and other documents pertaining to the efficient running of the home were examined as part of the inspection process. Prior to the inspection, eight relatives sent comments about the service to the inspector, which were very complimentary about the home, the management and the staff. What the service does well: What has improved since the last inspection?
The security of the premises has been improved by installing railing and lockable gates. An audit of the quality of the service has been carried out, which includes the views of the residents and their representatives. The recruitment of staff has improved by better checking with the Criminal Records Bureau, and staff now receive regular supervision. New staff receive a written induction when they start working at the home. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3, 4 & 5. Standard 6 does not apply. Service users and their representatives are able to make an informed decision about the suitability of the home from being provided with good information and the offer of trial visits. There are good assessments of service users to ensure that the home can fully meet their needs. EVIDENCE: The Statement of Purpose has been reviewed and amended to show the new management structure in the home. The records of one service user who had recently been admitted, were examined. They contained a local authority contract of terms and conditions, and included the fees charged. The contract was signed by all relevant parties. Standard 4 was assessed by reading care plans and other documents, observing the care provided and speaking to residents and relatives, all of whom expressed a high level of satisfaction with the service. Typical comments received were; “Excellent – we are very happy with the care received by Mum-in-Law. We visit the home regularly”, and “The home, under the excellent supervision of Florence Somers and her caring staff have brought my mother back from the brink and actually reversed the aging process”. Service users and relatives who were spoken to, stated that they had been able to visit the home prior to moving in. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8, 9, 10 & 11. Good care plans are drawn up for residents, which provide guidance and support for staff to meet residents’ needs. The staff provide a high standard of care, in a manner which meets the expectations and wishes of service users’ and their representatives, and medication is administered safely. EVIDENCE: Four care plans were examined. They contained thorough assessments, goals and objectives of care. The care plans were reviewed monthly. The case files seen, contained a range of healthcare appointments, for example; G.Ps, district nurses, opticians and dentists. At the time of the inspection, there were no incidents of pressure sores. The medication standard was inspected. The MAR sheets showed that there was proper recording of administration of medicines. There was a separate register and cupboard for storing Controlled Drugs. The residents stated that they were always treated with dignity and respect by the staff, who were observed to be very kind and courteous in their interactions with the residents during the inspection. The inspector noted that the funeral wishes of service users and/or their representatives are recorded in a special book.

The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 14 15. The home provides a good quality of life, and the residents’ experiences match their interests and needs. Residents are able to exercise choice about their lives and are provided with high quality food. EVIDENCE: An activities organiser provides a varied and appropriate activities programme, including arts and crafts, music and reminiscence. A hairdresser was present during the inspection. The home subscribes to the “Pet a Dog” scheme, which was observed to be very popular with the residents during the inspection. The visitor’s book showed that there were frequent visits to the home from friends and relatives. Residents’ religious needs are met by attendance at local churches and the provision of religious services in the home. An inspection of residents’ rooms showed that they were individually decorated to a high specification. There was ample evidence that service users were able to bring in personal items of furniture and other effects to personalise their bedrooms. The inspector was impressed with the quality of the personal touches in bedrooms, such as, the bed linen, which made the rooms very attractive.

They also stated that they were consulted about what they would like to eat. Some service users have solicitors or relatives who manage their personal finances, and one service user has an advocate to act for them. The menus were very varied and nutritious in content. The residents confirmed that they were very satisfied with the food and that they could choose an alternative to the menu if they preferred. An inspection of the kitchen showed that food was stored and prepared safely. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, & 18 Appropriate systems are in place to address service users’ concerns and complaints. There are also adequate procedures to protect service users from potential abuse. However, staff training in this subject needs to take place so that staff are better equipped to protect service users. EVIDENCE: The home has an appropriate complaints procedure. No complaints were recorded in the last 12 months and residents and relatives stated that they were satisfied with the service and had no complaints. There is an adult protection procedure in place and although there is a “Whistle-blowing” policy, staff did not appear familiar with this term or the contents of the procedure. A recommendation is made for this to be addressed at staff meetings. The manager is an accredited trainer in abuse awareness, and stated that she intended to start training the staff in this important subject. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 23, 25 & 26 Residents benefit from an attractive, homely and generally well-maintained home. However, there is a potential risk to residents’ safety from non-adherence to some fire regulations. The home employs its own handyperson who is responsible for general maintenance and repairs. The furnishings, lighting and décor, provide a very pleasant and homely environment. The garden, which is also accessible by wheelchair users, looked well maintained and particularly attractive. New railings and gates at the front of the building, offer better security for the home, which has been a long-standing problem. The inspector noted that several bedroom doors were wedged open, and some radiators did not have covers to protect residents from injury. Requirements are made to address this issue. Otherwise, the home was very well maintained. Several bedrooms were visited. They were individually decorated to a high specification. The inspector was impressed with the quality of the personal touches which made the rooms very attractive. The home employs a team of cleaning staff. At the time of inspection, the home was very clean and tidy and there were no offensive odours. There is a control of infection policy in place, and the laundry was appropriately equipped. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27, 29 & 30 Service users’ needs are being met by a group of appropriately recruited and competent staff. EVIDENCE: At the time of the inspection, there were twenty-five residents living at the home and three vacancies. The staff rotas showed that there is normally four staff in the morning, five in the evening and three staff on waking night duty. A senior member of staff sleeps in and provides back-up support if necessary. In addition to the care staff, there is a team of cleaning, catering and administrative staff. The records of four new staff who had been recruited since the last inspection were sampled. All staff had an application form, references, fitness to work documents and proof of identity. Clearances from the CRB had been obtained. Staff had been given appropriate inductions to the home and ongoing training was being provided in food hygiene, manual handling, health and safety, etc. Staff who were spoken to, stated that their training needs, including National Vocational Qualifications, were being met by their employer. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 33, 36 & 38 Service users benefit from living in a home which is well managed, with excellent leadership being provided to staff. However, there is a risk to residents and staffs’ health and safety because of poor adherence to fire regulations. EVIDENCE: The registered manager is accountable to a board of trustees. She is supported by a deputy and two senior assistants. There is also two administrative staff. The manager stated that she had completed her training for the Registered Managers Award and was awaiting the results. The inspector noted that there was a good atmosphere and that the interactions between the manager, staff and service users were friendly, but business-like. The minutes of residents and staff meetings showed that they were involved in the running of the home. There was evidence that staff receive regular supervision to support them in their work. The fire logs were seen and confirmed that the alarms were being tested weekly in different zones in the building. Fire drills were also carried out. However, the inspector was concerned to find several bedroom doors wedged open, which contravenes fire regulations. A requirement is made to address this. Temperatures of food, fridges and freezers were recorded daily. There were certificates of safety for fire, electrical, gas and water installations and appliances, and there was a current employer’s liability insurance certificate on display. Cleaning materials were stored safely and securely. There were records that regular health and safety checks were carried out.
