This inspection was carried out on 19th September 2005.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Eastwood Lodge provides a high quality of personal care, in a homely environment to the Residents that live there. Service Users commented on how kind the staff were and that nothing was too much trouble. The Manager and owner, is committed to continually improving the home and quite a bit of renovation of the property has taken place. The home as a very warm and homely atmosphere, it is well decorated and has an experienced staff team. The Manager and the staff continue to work hard to make sure that care planning is person centred and that all health and personal care needs are met.

What has improved since the last inspection?

Since the last inspection, the home has worked very hard to address all the Requirements from the last inspection. Staff are now very careful about the language that they use and write, when discussing Service Users. The Manager is also making sure that all staff views are taken into account. The Manager is now fully aware of the correct procedure in relation to the protection of vulnerable adults. The Manager now carefully reviews the numbers, and type of staff employed. There is now a cook and the home is in the process of employing another, to make sure holidays and sickness is covered. The Manager has forwarded to the Commission of Social Care Inspection, a copy of the Quality of Service and the Service User survey. All incidents and accidents are being reported to the Commission.

What the care home could do better:

During the tour of the home it was noted that a number of the cupboard doors were hanging off their hinges, or were off completely. This does pose a risk to staff and these need to be either refitted or replaced. It was also noted that a number of opened containers in the fridges and freezers, were not labelled. Although the lounge carpet has been re-laid, it is starting to come loose again and the Manager must make sure that it does not become a hazard. The home is in the process of creating a new laundry room and it is a recommendation that home does have sluicing facilities. The Manager does need to make sure that all areas of risk are covered in their risk assessments.

The home is privately owned and one of the two owners is also the registered manager. The building is an adapted two-storey property in keeping with other houses in the locality. There are ten single and one double bedrooms, most of which are on the upper floor, which is accessed by both stairs and a lift. There is an open plan lounge/dining area on the ground floor, leading through to a conservatory, which overlooks a patio area. Care staff assist residents with personal care to the level that each person needs, whilst encouraging independence. Care staff ensure that all health care needs are met by arranging appointments with health professionals. A hairdresser visits the home weekly. Care staff arrange both individual and group activities, but respect the wishes of residents who choose not to take part. The home is currently being extended through to the adjacent house, which will increase the number of bedrooms and provide a second lounge and toilet on the ground floor. 47 - 49 Eastwood Lodge DS0000025897.V251149.R01.S.doc Version 5.0 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This unannounced inspection took place over two and a half hours and was part of the annual inspection programme. Seven requirements made at previous inspections were checked for progress, as well as key standards being looked at. Service Users were asked for their views and experience of living in the home. The Manager and a member of staff were also asked their views and a range of written records were looked at and a tour of the home was undertaken. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2 and 3 Prospective Service Users do have information to make an informed choice about where they live. Each Service User has a written contract and statement of term and conditions with the home and no Service User moves into the home without their needs being appropriately assessed. EVIDENCE: During the inspection a Service User was spoken to and said that a relative was given information from the home, allowing them to make an informed choice. Records were looked at during the inspection and it was seen that each Service User had a contract. A copy of the statement of terms and conditions was also seen; the Manager said each Service Users has a copy. The file of a fairly new Service User was looked at and an in-depth assessment and an assessment from the Local Authority had been done prior to moving in. 47 - 49 Eastwood Lodge DS0000025897.V251149.R01.S.doc Version 5.0 Page 9 Health and Personal Care The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8 and 10 Each Service User’s health, personal and social care needs are set out in a care plan. Service Users’ health care needs being met. Service Users do feel they are treated with respect and their privacy is upheld. EVIDENCE: During the inspection three Residents care plans were looked at and each care plan set out their personal, social and health needs. It was noted that one of the Service Users had a history of falls before moving to the home; there was no risk assessment for falls. The Manager said that the Service User did not have falls since they had been in the home and their mobility had actually improved. However, the Manager agreed that it would be advisable for this Resident to have a risk assessment due to their previous history. The documents looked at during the inspection had case notes which included a pre-admission assessment, care plans, review notes and daily logs as well as record of all contact with health professionals. 47 - 49 Eastwood Lodge DS0000025897.V251149.R01.S.doc Version 5.0 Page 10 Service Users spoken to during the inspection said that the staff were very kind and nothing was too much trouble. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13, 14 and 15 The Service Users do maintain contact with family and friends, as well as being able to access facilities in the community. The Residents are able to exercise choice over their lives and receive wholesome and appealing meals at times that are convenient to them. EVIDENCE: One of the Service Users spoken to during the inspection said that their sister visits almost daily and their son came and takes them out. Another Service Users spoken to said that their family visit regularly and that they are still able to have contact with neighbours and friends. During the tour of the home, it was seen that Residents used various parts of the home. One Service Users spoken to liked to watch a particular programme on the television in their room before going down to the lounge. All the Residents spoken to commented on the good food, the fridges and freezers in the home were found to have a good variety of food. During the inspection a meal was being prepared by the cook and was seen to be wholesome and appealing. 47 - 49 Eastwood Lodge DS0000025897.V251149.R01.S.doc Version 5.0 Page 12 Complaints and Protection The intended outcomes for Standards 16 - 18 are: 16. 17. 18. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 17 and 18 The Home is protecting the Service Users from abuse and protecting their legal rights. EVIDENCE: During the inspection the Protection of Vulnerable Adults (POVA) policies and procedures were looked at and found to be appropriate and accessible to staff. The Manager said that staff had been on training and that they would be aware of any abuse. Discussion with the Manager and from the conversations with Service Users, that home does protect their legal rights. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 21, 24 and 26 The home is well maintained and safe, the home has recently increased the number and access to toilets and washing facilities. Each Service Users has their own rooms and each room is personalized. The home was very clean, homely and pleasant. EVIDENCE: Service Users live in a homely environment, which is decorated to a good standard and is well maintained. Service users have each of their rooms personalized and bring with them possessions and any furniture they want to. The home has had a number of improvements since the last inspection, which increases the number of toilets and their accessibility to the Service users. It was noted during the tour of the home that it was very clean and there was a pleasant atmosphere. During the inspection two Service Users were spoken to and both commented how pleasant the different areas of the home were. The home is planning to improve their laundry facilities and it is suggested that the home have a machine that has sluicing facilities. 47 - 49 Eastwood Lodge DS0000025897.V251149.R01.S.doc Version 5.0 Page 14 Staffing The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 29 and 30 The numbers and staff mix and the homes recruitment policy and practices support and protect the Service Users. All staff are trained and competent to do their jobs. EVIDENCE: Service Users were extremely complimentary about the skills and abilities of the staff and particularly commented on their commitment and warmth towards them. They were very appreciative of this and described the home as a lovely place to live. The home employ a cook and are in the process of employing a second cook to cover sickness and holidays. The Manager said that every effort is made to make sure that there are appropriate levels of staff at the home. During the inspection staff records were looked at and all staff had a CRB and two references before they were employed. On the staff records, a number of training certificates were seen as well as supervision and induction records. 47 - 49 Eastwood Lodge DS0000025897.V251149.R01.S.doc Version 5.0 Page 15 Management and Administration The intended outcomes for Standards 31 – 38 are: 31. 32. 33. 34. 35. 36. 37. 38. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 33, 37 and 38 The home is well run and is run in the best interest of the Service Users. The Service Users rights and best interests are safeguarded by the records, policies and procedures. The home does have a hazard to staff, with the cupboard doors in the kitchen, but this does not affect the Service Users. The home is not storing food correctly; food containers were not labelled or dated- this poses a risk to service users. EVIDENCE: The Manager is committed and does run the home in the best interests of the Service Users. Residents spoken to said that every effort is made to make sure they are looked after very well. The Manager is keen to continue to develop the home, making improvements where they are needed. During the inspection a number of documents were looked at and were found to appropriately kept. There were a number of accessible policies and procedures, which safeguarded the rights and interests of the Service Users. 47 - 49 Eastwood Lodge DS0000025897.V251149.R01.S.doc Version 5.0 Page 16 It was noted during the tour of the home that in the kitchen a number of door on the kitchen units were hanging off and one had come off. This is a risk to the staff and these need to be either repaired or replaced. The lounge carpet has been re-laid but is becoming loose again and will quickly become a risk to the Service Users. During the tour of the home, fridges and freezers were inspected and it was found that food containers were not labelled or dated.
