What the care home does well

Elaine Herbert House is a good care home which is clearly greatly valued by those who live there. The home is well managed and well staffed with an experienced and well trained staff team. Residents described the staff as kind caring and respectful. Residents rooms are generously sized and the environment as a whole is well maintained. The food served at the home is of a high standard, with much choice available.

What has improved since the last inspection?

Since the last inspection the homes care plans have been improved with greater effort to get residents and their families involved in the process. The corridor areas of the home have been redecorated as have some bedrooms. The home now has in excess of 50% of care staff who hold an NVQ level 2 or above which complies with government targets. Most staff have now received training in adult protection.

What the care home could do better:

The management need to maintain the current high standards and might usefully develop an internal audit process to help monitor their performance.

Date of last inspection 8 November 2004 Brief Description of the Service: Elaine Herbert House is a care home offering personal care and accommodation for 21 older people. The service is operated by the Great Hospital Charity and the care home stands within the overall complex of the Great Hospital. The site is located in the city centre of Norwich near to Norwich cathedral. The home has been open for some time, however it is new to registration having previously been exempt by royal charter. The home is purpose built with accommodation on the ground and first floor. Those living at Elaine Herbert house have access to the grounds of the whole site. There are 21 single bedrooms all of which are en-suite. This inspection was unannounced and took place over five hours. Whilst in the home the inspector shared lunch with residents and spent time talking to a number of those living there whose views are reflected in this report. Records were inspected a partial tour of the home undertaken and staff and management were interviewed. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3 The home admits people whose needs it can meet, having carried out an appropriate assessment. EVIDENCE: The inspector spent time studying the care plans of a number of residents including those of the people most recently admitted. In all cases the plans were based on a good range of information including assessments from the homes management and other professionals involved in peoples care. Later discussion with residents confirmed that they had felt part of the process and were happy that their needs were being met. The homes manager is clear about what needs can be met and has declined prospective residents when the assessment indicated that they could not provide appropriate care. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7,8,10 The home has a sound care planning process and residents are treated with respect by staff who protect their privacy and dignity. Residents health care needs are met. EVIDENCE: A range of care plans were inspected and the care of some residents was tracked by the inspector. Each plan seen by the inspector was well maintained , was based on assessment and had been reviewed. Wherever possible residents had signed to indicate acceptance of their care plan. Plans contain risk assessments and detail the arrangements for skin care where needed. In addition to the plans for direct care each person has a plan outlining social care needs. By looking at these records and talking to the homes manager and staff on duty the inspector could see that proper attention was given to peoples health care needs and the support of health care professionals was sought in an appropriate and timely manner. During the inspection a number of residents were talked to and all indicated that they felt well cared for, treated as individuals and given respect. People felt their privacy was respected and their dignity protected when receiving personal care. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15 The home is run in the interests of the residents who are able to exercise choice and control over their lives. Good quality wholesome meals are provided and residents are able to choose from a range of options. EVIDENCE: The inspector took lunch with residents and spent time during the inspection talking to a selection of people living at the home. All of those spoken to were very happy with their care and felt able to live a lifestyle that suited them. Residents told the inspector that their visitors were able to come at any time and were made welcome. Visitors are able to eat meals if they wish and are offered a hot drink. Several residents told of how they were able to follow their religious beliefs and of ways in which they could maintain social interests. People talked of being in control of their care and of staff respecting their wishes and later discussion with staff on duty indicated that they sought to follow this approach. The home offers a particularly high standard of food with choices always available and nicely presented tasty food. During lunch the inspector sat with a vegetarian who commented positively on the quality and variety of options provided to her. JUDGEMENT – we looked at outcomes for standard(s) 18 Residents are well cared for and protected from abuse. EVIDENCE: Since the last inspection the home has provided two courses on adult protection and the majority of staff have now received recent training in this area. Residents spoken to talked of always being cared for in a kindly and caring way and all were confident that if they had a concern they could share it and it would be addressed. Staff were confident that they would be supported in whistle blowing if ever the circumstances arose. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,2324,26 Elaine Herbert House offers well maintained comfortable accommodation, which is suitable for the care of older dependant people. EVIDENCE: The inspector undertook a tour of the home although not all service users rooms were seen. All areas were clean and bright and in good order. The homes corridor areas have been redecorated in line with a recommendation made at the last inspection. Although consideration has been given to replacing the carpet in the corridor which marks easily, it has been decided to instigate a different cleaning process to address the problem. Residents bedrooms are large in size with a kitchenette and en-suite bathroom. All of the rooms seen were attractive, highly personalised and enjoyed good views over the grounds of The Great Hospital. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27,2830, The home has a settled well trained staff team able to meet the needs of those they care for. EVIDENCE: The home has a settled staff team with a good range of experience and a low staff turnover. Staff on duty were interviewed and felt that the home has staffing levels to enable them to offer a good quality of care. Residents spoken to described staff as kind and caring who treated them as individuals. The inspector was told that staff find time to talk to people and do not have a routine dominated by practical tasks. More than 50 of care staff now have an NVQ level 2 or above which exceeds the government target for care homes. In addition to NVQ the home has an induction programme consistent with expectations and other training is also made available. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,35 The home is well managed and conducted in the interests of those living there. The financial interests of residents are protected by the homes policy and practice. EVIDENCE: The home manages monies for a limited number of residents and the inspector was shown records which accounted for all dealings. The registered manager is an experienced nurse who is currently near to completing the Registered Managers Award. The residents spoken to liked the manager and felt she was approachable and ran the home well. Likewise the staff interviewed felt well lead and supported and that the manager was good at her job.
