This inspection was carried out on 5th July 2005.

The inspector found there to be outstanding requirements from the previous inspection report but made no statutory requirements on the home.

What the care home does well

The home continues to provide a good standard of care for the residents. Those residents spoken with were unanimous in their praise for the staff and in expressing satisfaction with the care and the food they received. The staff team is well trained and able to meet the needs of the residents. The premises were clean, odour free, well maintained and provided a comfortable and homely environment.

What has improved since the last inspection?

The home has met the requirements identified at the previous inspection. The upgrading of a bathroom to contain a sit down shower has been appreciated by the residents. Staff one to one supervision is now being achieved on a regular basis.

What the care home could do better:

Further development of the home`s care planning system is required to ensure it is more user friendly and duplication of records is avoided.

Date of last inspection 16th March 2005 Brief Description of the Service: Elgin Rest Home is a small family run home, owned and managed by Mr and Mrs Dickson. The home is a detached property situated within close proximity of local amenities, public transport and the sea front. The home is decorated, furnished and maintained to a good standard throughout.The home has three shared rooms and eleven single bedrooms.The home has a passenger shaft lift and electric stair lift and all bedrooms have a call alarm facility. There is an attractive garden to the rear of the property and limited car parking is available to the front. This was a routine unannounced inspection combined with a complaint investigation, which lasted seven hours. During the inspection, there was a tour of the premises and records and documents looked at. Time was spent with the residents and twelve were spoken with about their life at Elgin Rest Home. Discussion took place with the proprietor/manager and two staff, thirty two of the thirty eight standards were inspected. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) The home’s admission process is well managed and residents are given clear verbal and written information regarding the home to enable informed choices. EVIDENCE: The home has an appropriate Statement of Purpose and service users guide. Each file contained a copy of the home’s Terms and Conditions of residence, signed by the relevant resident. Pre-admission assessments were evident for the two latest admissions. Both were seen to be detailed and informative. Prospective service users are encouraged to visit the home and spend an afternoon, wherever possible. However, relatives or residents representatives always visit the home. The manager has many years experience in caring for older people. Staff receive a wide range of relevant training and those spoken with demonstrated a wide knowledge of residents needs. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7-10 Residents are well looked after in respect of their health and personal care needs. Appropriate administration of medication procedures are in place with clear arrangements to ensure resident’s medication needs are met. EVIDENCE: A random sample of three residents care plans evidenced good pre-admission assessments and comprehensive instructions for staff to meet residents’ assessed needs. In addition an “Assessment for good care planning” format was used. Both documents contained similar information which led to some to be partially completed. Medication administration procedures and practices were well maintained although minor improvements were required in the guidelines for medication administered as and when required. Care plans evidenced that service users health needs are being met. Observations throughout the day showed that staff have a good awareness of how to respect residents privacy and dignity. Residents confirmed that staff always speak to them in a respectful way. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12-15 Social activities and meals are both well managed, creative and provide daily variation for the people living in the home. EVIDENCE: Residents’ likes and dislikes are recorded in their care plans and are able to choose which daily activities they wish to participate in. Residents are taken out on a regular basis particularly throughout the summer months with visits to the sea front and other local attractions. Three staff are booked for training in “activities for older people” in September. The home operates an open visitors policy and residents confirmed that they could have visitors at any time. The home provides a wide range of nourishing and well balanced meals to meet individual needs. Menus are discussed in residents meetings. Further development of the home’s nutritional records is needed. All residents spoken with were complimentary regarding the quality of food provided. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16, 18 Arrangements for protecting residents and responding to their concerns are appropriate. EVIDENCE: Home has an appropriate complaints procedure, leaflets are available for residents and their families. No complaints have been received by the home since the previous inspection although two have been received by the CSCI and were investigated during the inspection. The outcomes will be detailed in the next inspection report as investigations have not at the time of the report had not been fully completed. All staff apart from one have attended protection of vulnerable adults training and the home has appropriate protection of vulnerable adults policy and procedures. A copy of the Southend’s social service procedures was available in the home. Many compliments were seen to have been received by the home. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19,20,21,23-26 Elgin Rest Home was clean, bright and well maintained and provided a homely, comfortable environment for the residents. EVIDENCE: The home was found to be generally well maintained. It was decorated and furnished to a standard that creates a comfortable and homely environment for the residents. Communal space was adequate with two lounges and a dining room. A small visitors room is provided. One bathroom has been upgraded and now contains a walk in, sit down shower. Improvements to the second bathroom are planned and a ramp is to be provided at the entrance to the dining room. All bedrooms were seen to be well furnished and evidenced personal; items brought in by the residents. The home was found to be clean and tidy and odour free. A well maintained garden is provided to the rear of the property. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27-29 Staffing levels and skills were appropriate to meet the needs of the residents. The vetting and recruitment of staff were appropriate to protect residents. EVIDENCE: Staff rotas indicated that agreed staffing levels were being maintained. However rotas should indicate when the manager is supernumerary. Files of two staff inspected detailed that the home had undertaken all the necessary recruitment checks. Six staff are currently undertaking training to achieve NVQ2. Residents spoken with said the staff were kind and caring and do find time to talk to you. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31-33, 36,38 The home is managed effectively and staff within the home are well supervised and supported by the manager. EVIDENCE: The manager/proprietor has owned and managed the home for 15 years and has recently achieved qualification at NVQ Level 4. There was evidence of good day to day contact between the manager and staff. The manager has hands on approach and frequently works alongside care staff. Regular staff and residents meetings are held and planned formal one to one supervision is maintained. Staff spoken with said the manager was approachable and supportive. The manager has adopted a proprietary quality assurance system and questionnaires have been completed by all the residents. Safety certificates were seen for services and equipment. Records were maintained for weekly and safety checks.
