Conditions of registration: None Date of last inspection 28/01/05 Brief Description of the Service: The home is a five-bedded facility for service users within the category of learning disability. The home is part of the Leonard Cheshire Foundation. Elmers End Road is maintained in a domestic style. It is located in a residential area of Penge.The home has three floors and the upper part is accessed by steep stairs. and has it has no lift facility so it would not be siutable for persons with significant mobility difficulties. Service users in this home are encouraged and enabledto be as possible.Each service user attends the local day centres five days a week. visits to their families are encouraged ,including overnight stays. Leisure facilities in the local communityare also accessed. Group holidays are organised by the home and staff accompany service users as appropriate. The registered manager, Mr Lindon Philander, manages a team of four staff. Staff members in the home are recruited through the Leonard Cheshire Foundation. Staff training is provded through the organisation relevant to the service user group.Staff is provided throughout the 24 hour period. All local health provision is accessed; Domicilliary visits to the home are discouraged unless it is an emergency. Specialist support is offered via staff at day centrs and Bassetts Centre. This is an overview of what the inspector found during the inspection. This unannounced inspection took place over four hours as part of the statutory inspection programme. The inspection included a tour of the premises, inspection of some of the records, care plans and safety systems. Two members of staff and one service user spoke to the inspector and the inspector was able to see the interaction between staff and the service user. The inspector would like to thank all who participated with the inspection. What the service does well: What has improved since the last inspection?
The manager has worked to improve the risk assessments for service users by including more details about their abilities and their needs. The home now has robust recruitment procedures in place. The home now has a copy of the No Secrets and the Local Authorities Adult Protection guidelines available to all staff. The manager has amended the Statement of Purpose to inform new service users about the steepness of the stairs within the home. The service users room is now identified in the contract. The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users’ know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1,3,4,5 Staff, who work in the home, meet all the assessed care and personal needs of the service users except for health and specialist needs. Service users and their families are given all the information required to make a decision as to whether the home will meet their needs before making any decision. EVIDENCE: The Statement of Purpose has been amended to include the name and address of the Commission for Social Care Inspection and the manager stated they are going to introduce a new format in the near future using the widget communication system to enable the service users to understand the Statement of Purpose. The manager stated the Widget communication system would be used. All service users have a full care needs assessment before going to live in the home which is carried out by their care manager and reassessed on a yearly basis although very often the care manager, who attends the meeting is not known to the service user and does not know their needs. The service user visits the home and meets the other service users already living there before making a decision to live there permanently. The home has the services of a general practitioner and specialists to meet the service users assessed health needs and can also meet their social needs. The service user is always shown the bedroom they going to use before making a decision to live at the home and they choose the colours they want it decorated. Service users make decisions about their lives with assistance as needed. Service users are consulted on, and participate, in all aspects of life in the home. Service users are supported to take risks as part of an independent lifestyle. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6,7,9. The service users are given choices over the way they live and the activities they undertake, however, staff carryout risk assessments for every activity before it happens. Service users enjoy a full social life within their assessed limits. The outcomes of their activities are noted in the care plans for future reference. EVIDENCE: Services users care-plans were up to date and well documented noting their likes and dislikes in all aspects of their daily living, family contact and social requirements, but is still being developed to include a care plan written with the widget communication system. The care plans are reviewed on a monthly basis in house and a yearly basis with the family and care manager, unless the service user has specific needs to be met. Service users are helped by staff to find information to help them make decisions concerning their own life. Staff record the decisions, made by service users, stating how the decisions have been made, e.g. whether they have been helped by staff or relatives. All the service users need support with their finances but sign every transaction if they agree with it. The services users’ parents are appointees for them, but if they wish to use money from their accounts in the home, two members of staff witness the transaction and sign the record sheets. A risk assessment is carried out before the service users come to live in the home and is placed in the service users care plan. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14,15,16,17. The service users in the home enjoy a full life and a good standard of living . They are always asked what activities they would like to undertake and are supported by staff to enable them to participate in the activity of their choice. They use the local recreational facilities in their leisure time and some have employment to go to during the day, those who are not working go the local day centres or undertake leisure activities. The menu, for the week ahead, is planned by service users. Some service users use the services of Jobmatch to help them find a job which suits their assessed potential. Services users are encouraged to develop their skills in all areas of their lives. The home supports all service users to integrate into the life of the community whether, going to the pub, swimming baths, shopping or the cinema. Service users are enabled to go to the polling station and vote. This year the service users have voted to go to Majorca for their holiday and will be supported by staff. The service users have a well balanced diet and meals are served in the dining room. When service users are at home they have three meals a day prepared by staff and themselves. The menus are prepared after consultation with service users. Only one service user needs help with meals and the staff are always on hand to help when needed. The ageing, illness and death of a service user are handled with respect and as the individual would wish. JUDGEMENT – we looked at outcomes for standard(s) 23 Service users are protected from abuse, neglect and self-harm. EVIDENCE: All staff members have undertaken Adult Protection(POVA) training and the organisation policy and procedures manual contain the homes policy and guidelines on ‘Adult Protection’ that all staff members are made aware. The staff have a copy of the Local Authority’s Adult Protection Guidelines available to them. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 25,26,27,28,29,30 The service users live in a homely, comfortable and safe environment which suits their lifestyles and assessed individual needs. EVIDENCE: The home is within easy reach of the local shops and public transport links. The service users are very happy living in the home which is comfortable and provides a safe environment. There was one resident in the home during part of the inspection who when asked made it known she enjoyed living there and she was happy with everything, She left the home shortly afterwards to go swimming and out for lunch. All the service users bedrooms were very personalised with posters on the wall and family photographs were seen and every room was furnished to a high standard. The decorations were chosen by the service users. The home has specialist equipment for those service users who have been assessed as requiring to maximise their independence. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,32,35,36 The home is working towards having a fully trained and qualified work force which benefits the service users. EVIDENCE: The organisation has clearly defined job descriptions for all levels of staff and they receive induction training at the start of their employment. This is followed by NVQ training at all levels which is undertaken, by LEDAFF Training, on behalf of the Organisation. During induction training the staff learn about the main aims and values of the home and are taught about their roles when assisting service users. The organisation is working towards the target of having 50 of staff achieving a NVQ level 2 qualification. At the last inspection the home was told their staffing levels must be increased and the manager stated this had been achieved. Staff have a monthly staff meeting where they are kept informed of any changes to their work requirements and /or changes about the service users. Staff training is geared towards achieving the Learning Disability Award Framework which underpins their knowledge of the service users in the home. Staff receive monthly one to one supervision which is recorded and the notes are kept in their personnel files. Service users benefit from competent and accountable management of the service. The Commission considers Standards 39, and 42 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 37,38,39,40,41,42,43 All staff members receive induction and foundation training. The registered manager must ensure the health and safety of the service users and staff by having the patio re-laid and the garden tidied. The service users are to have a Quality Assurance survey developed for them in the Widget format so that they will be able to express their likes and dislikes. EVIDENCE: The manager, Lindon Philander, is qualified, to NVQ level 4 in management and is always updating his qualification. The manager is developing a service users survey in widget format, to enable the service users to tell the staff their likes and dislikes about the care they receive. The survey will also be sent to service users parents/relatives/supporters for their views about the care their service user receives and feedback will be sent to all parties. The homes policies and procedures had, stated, the manager all been updated recently but were found not to have been dated. The patio in the back garden was found to
