This inspection was carried out on 1st August 2005.

What the care home does well

A wide range of activities is available for the residents both in the home and in the local community. Resident`s bedrooms contained numerous personal possessions, which enhance the homely atmosphere.

What has improved since the last inspection?

Training for staff has increased, more than 50% of care staff have achieved NVQ level 2 in care. The qualified staff have completed First Aid training. A supervision system for staff is in place and up to date.

What the care home could do better:

Attention is required to the curtains/ window dressings in the majority of the resident`s bedrooms. Training for some staff is required in Health and Safety and COSSH.

Date of last inspection 11/11/2004 Brief Description of the Service: Elmridge Nursing Home is a 42 bedded care home with nursing, registered under the Care Standards Act 2000 to provide care to 42 adults with learning disabilities. The home has conditions of registration in place to enable it to provide care to 22 people with learning disabilities aged between 18 and 65 and no more than 20 people with learning disabilities aged over 65. The home was first registered under the Registered Homes Act 1984 in 1993. The registered provider is BUPA (Goldsborough) Ltd. The responsible individual is Mrs L Tart and the manager is Mrs Helen Saunders. Elmridge is a single storey building divided in 4 units with each of these areas opening into a large room used for day activities. This unannounced inspection commenced at 9.30am and lasted for four hours. The manager, qualified staff and the administrator were spoken to during the inspection. Numerous records including care assessments, care plans and maintenance records were examined. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 The homes assessment documentation is not always fully completed. EVIDENCE: The twenty- four hour life plans from two out of the three care plans examined were incomplete and did not show that individual needs and aspirations of residents were fully assessed. For one resident the risk assessment tool should have been completed monthly. Records showed that this had not been carried out since May 2005. JUDGEMENT – we looked at outcomes for standard(s) 9 Residents are supported to take risks within a risk management framework. EVIDENCE: Documentation in resident’s files showed that risk assessments were in place where needed to encourage an independent lifestyle. Staff support the residents and for one person an advocate was involved to assist towards independent living. Service users have opportunities for personal development. Service users are able to take part in age, peer and culturally appropriate activities. Service users are part of the local community. Service users engage in appropriate leisure activities. Service users have appropriate personal, family and sexual relationships. Service users’ rights are respected and responsibilities recognised in their daily lives. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 & 15 Residents are provided with a range of activities within the home and local community. EVIDENCE: Residents take part in many activities in the home, the local community and outings. There had recently been an educational display from the ‘Zoo Lab’. Reptiles and small animals were brought into the home for the residents to touch or just watch, whichever they felt more comfortable doing. Prior to the inspection residents had enjoyed a day out at Hartlepool Marina and a visit to see the tall ships at Newcastle. Local events are also well attended by the residents. There are in house activities such as arts, crafts, a sensory room is available and the home had recently purchased a ball pool. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 20 Good systems for storing and administering resident’s medication are in place. EVIDENCE: The home has a system for storing, administering and recording of medication. Records of medication received at the home, given to residents and returned to the pharmacy were seen to be accurate. Service users live in a homely, comfortable and safe environment. Service users’ bedrooms suit their needs and lifestyles. Service users’ bedrooms promote their independence. Service users’ toilets and bathrooms provide sufficient privacy and meet their individual needs. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 24, 25 & 30 EVIDENCE: A tour of the home was carried out. Residents appeared comfortable in their various lounges. Maintenance audits in the home are carried out on a regular basis, which was evidenced in the maintenance records. Residents’ bedrooms are individually decorated and furniture arranged to promote independence. In the majority of the bedrooms the curtains were not hung properly, some had missing curtain hooks and tab topped curtains were not hung on poles. Many of the curtains were of poor quality. Window blinds were damaged in two of the bedrooms and required replacing. In the communal areas many doors and walls were damaged due to hoists being manoeuvred around the home. In one bedroom copper pipe needed to be boxed in. The home was odorous in many of the communal areas. Service users benefit from well supported and supervised staff. The Commission considers Standards 35 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 35 & 36 The staff team have worked hard to ensure that in excess of 50 of them are qualified to NVQ level 2 in care. Staff receive regular supervision. EVIDENCE: Staff training records showed that more than 50 of care staff have achieved NVQ level 2. Training is ongoing in First Aid with the qualified staff recently completing a four-day course. A formal supervision system is in place and staff supervisions were up to date. JUDGEMENT – we looked at outcomes for standard(s) 42 Required policies and procedures are in place. Training for some staff is overdue. EVIDENCE: The home has health and safety policies and procedures in place and checks are undertaken regularly. However it was noted that staff training in health and safety and COSHH was required. A certificate was not available for the five yearly periodical installation tests and an immediate requirement notice was issued.
