Road links are via Broads and lanes, with no appreciable bus service. The three-storey house was converted from a private dwelling to provide residential accommodation and care for twenty-six persons with elderly care needs, three of who may have dementia type conditions. Far Fillimore has a Bungalow within the grounds of the home, which provides accommodation for four overseas staff. Far Fillimore consisted of twenty-two single occupancy bedrooms, two of which were equipped with en suite facilities; there were also two shared bedrooms that were provided with dividing screens to afford service users with additional privacy one of these rooms are currently being used as single. The home also provided two lounge areas and a separate dining room. Toilets were provided throughout the property and were in close proximity to bedrooms and communal areas. Ramp access and grab rails were provided at the external entrances, enabling service users to access the large attractive well maintained gardens where there is seating for residents and visitors. Far Fillimore Rest Home DS0000034076.V271087.R01.S.doc Version 5.0 Page 5 SUMMARY This is an overview of what the inspector found during the inspection. This announced inspection was carried out over half day 20th December 2005. During the course of the inspection the inspector spoke to all residents, staff on duty, visitors to the home and the visiting district nurse. Two residents were case tracked both were spoken with and all records relating to them were seen. Comments received from residents about staff and life at the home and the manner in which they are cared for were of a positive nature. Residents said that staff were caring and kind to them and as at previous visit a lot of cheerful banter was being exchanged. Information was provided by management and inspectors observations regarding staffing, staff training, menu and dietary provision all was seen by the inspector to be in place at the home. On arrival at Far Fillimore residents had completed breakfast and were moving freely around the home. There is a cook or kitchen assistant or both on duty for all mealtimes and individual preferences are discussed with residents. Resident accommodation is located on all floors and observations of the inspector were that the home was clean. Resident bedrooms were individualised with favourite items they had brought into the home with them. Far Fillimore has in place a Statement of Purpose and Service users guide to inform residents of the services and facilities provided this has been updated to include the new Care Manager. All residents have a full assessment of their needs carried out prior to admission to the home. Arrangements are in place for meeting health and personal care needs of residents and details are recorded in care records. Regular social opportunities are available for individuals who wish to take part and preparations are in hand for Christmas. There are some lovely photographs of the Christmas party that has already taken place. The home has in place a complaints procedure this is given and explained to residents on admission to the home. Far Fillimore Rest Home DS0000034076.V271087.R01.S.doc Version 5.0 Page 6 Staff on duty at the home were observed to be experienced and competent to care for older people and were able to discuss diseases associated with old age. Appropriate recruitment procedures were in place and all staff prior to employment has Criminal Records Bureau and Pova checks carried out. What the service does well: • Observations of the inspector are that residents are well cared for at the home and that staff treat them with respect, conversations with individuals during the inspection confirmed this. As at the previous inspection individuals were being offered choices and encouraged to present positive personal image, everyone was smartly dressed hair had been done nails were being manicured by a member of staff. All were interested and involved in Christmas preparations. There is a homely environment at Far Fillimore with a very relaxed and friendly atmosphere residents interacting positively with each other and staff. • • What has improved since the last inspection? • • Back Stair carpet has been replaced making the stairs safer to use and improved the environment. Six new radiators have been fitted all need to be guarded as they are not low surface temperature radiators. This work has commenced and is in hand. Three windows have been repaired and repainted and new sills fitted. Windows in the ten-bedded extension have been sanded down and repainted. Heron lounge one side has been decorated following a leak that has been dealt with. Alginate bags are now used for the laundry. Staff training has taken place and updates have been arranged for Food Hygiene, First Aid, Dementia Care, Fire. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1,2,3,4,5. The homes Statement of Purpose and Service Users Guide provides prospective residents with up to date details of the services the home has to offer enabling an informed decision about admission to Far Fillimore to be made. EVIDENCE: As at previous inspection the homes Statement of Purpose and Service Users Guide is given to residents and relatives clearly describing the services and facilities the home is able to offer. All residents have contracts of terms and conditions of residence at the home a copy of which is on resident’s files. A full assessment of individual needs is carried out prior to admission to Far Fillimore including district nurse assessment to ensure that the needs of residents can be met. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9,10,11. The health, personal and social care needs of residents is well met. The systems for the administration of medication were observed by the inspector to be good with clear arrangements being in place to ensure residents medication needs are met. EVIDENCE: Detailed care plans are in place and contain profiles of life history and information relating to the individuals diagnosis, care needs, any specialist intervention and support and assistance required to enable service users to live an independent life style. All special needs are identified on the care plan that is reviewed monthly on a dependency profile. Healthcare needs are met by two visiting GP practices, there are visiting optician, chiropodist and district nurses. Inspector spoke to visiting district nurse today who made positive comments about the home and the way residents are cared for. Nurse said that staffs refer appropriately, resident care Far Fillimore Rest Home DS0000034076.V271087.R01.S.doc Version 5.0 Page 12 is good moving and handling is good, pressure care is good and that overall there has been a general improvement at the home. Dementia care training has taken place and update is arranged, diabetes care training has taken place and specialist diabetic nurse contact number is readily available as at previous visit. Records are kept of GP and District Nurse visits. Two residents currently are self-administering medication and a lockable facility is provided all have risk assessments on file. The situation remains the same with regards to medication one of the GP practices is a dispensing practice and provides only bottle to resident system, the other practice provides prescription only so the home has a monitored dose system in place for these residents but the practice will only take ten residents. The homes Care Manager via PCT is still trying to get all on monitored dose system but this is proving near impossible as the home is isolated and they are restricted to the use of current GPs. Homely remedy policy is in place and a letter regarding homely remedies from the GP is also on file as requested at previous visit. Training in the management of medication has taken place and the medication fridge has a lock. The home has in place a policy on death and the dying, if someone was dying at the home and family wished to remain with them then they would be made comfortable and hospitality offered. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12,13,14,15. The systems for resident’s consultation in the home are good and resident’s views are sought and acted upon. There was a relaxed and friendly atmosphere with residents moving freely around the home. Contact with the small local community is good a lot of residents are from the local area. The local choir and bell ringers are coming into the home over Christmas. The cook at the home provides excellent food with a choice at all meals. EVIDENCE: Conversations with residents identified that they enjoyed living at Far Fillimore and observations of inspector were that staff cared for them in a sensitive and supportive manner. As at previous visit there is a notice board in the hallway that displays the staff on a particular shift with a photograph, changed each shift by incoming staff, the first aid trained person on the shift, who is in the kitchen, and a full activities programme. At the time of this visit the board was also in the form of an advent calendar with a different resident opening each day. An activities file records activities that have taken place or are planned. Far Fillimore Rest Home DS0000034076.V271087.R01.S.doc Version 5.0 Page 14 Observations identified: Body shop party, Beaver cubs visiting with gifts, Musical entertainment for Christmas, Mattie the dog, Christmas church service, school choir, Christmas party, Hanbury bell ringers. The library delivers large print books. Monthly communion is held at the home and one resident goes out to church. Dementia Care activities training has been held at the home by local PCT. Advocacy services are identified in the home for residents who may need this service. Residents meetings are minuted. There is a monthly newsletter that contains resident views, up and coming events, activities, proposed inspections. There is also monthly news and action plan for staff includes areas of development and matters for action. All staff involved and there is a new aim identified each month for staff to achieve. The homes menu offered a balanced nutritious diet with a choice at all meals, home baking and fresh produce delivered weekly residents said they always enjoy their food. A cook covers all mealtimes or a kitchen assistant all have food hygiene training. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16,17,18. The home has a satisfactory complaints system with some evidence that service users feel their views are listened to. EVIDENCE: The home has in place a complaints procedure that has been discussed with residents and their relatives. All have a copy in their bedroom and it is displayed on the homes notice board and in the statement of purpose. The procedure includes information on how to contact the Commission for Social Care Inspection. Residents the inspector talked to said they were aware of the complaints procedure and that they would talk to staff or relatives if they had any concerns. Resident’s legal rights are protected by the involvement of relatives, advocates, and solicitors as appropriate. Residents are protected from abuse by staff awareness and training, and the policies/procedures in place at the home. Service users live in a safe, well-maintained environment. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19,20,21,22,23,24,25,26. Internally the home is generally well maintained there are however areas that still need attention. Externally work has started on areas identified in the previous inspection report, as needing attention and this is ongoing. EVIDENCE: As at the previous inspection the location and layout of the home is suitable for its stated purpose. Resident’s bedrooms were clean and comfortable and personalised as individuals wished, individuals were happy with their accommodation and felt secure in the knowledge that it was their private space. Bedroom 1,2,6,9 uncovered radiators that need to be guarded, as does the dining room radiator. Any hot pipework should be guarded. Far Fillimore Rest Home DS0000034076.V271087.R01.S.doc Version 5.0 Page 17 The carpet on the rear staircase has been replaced to ensure the safety of persons using this staircase; this has also enhanced the environment in this area. The office carpet is soiled and stained and needs replacing. The ground floor bathroom still need to be upgraded inspector understands that this bathroom is well used and it would enhance the environment for residents if a new bath were fitted. Bathroom radiator first floor, and first floor landing needs to be guarded, as does any hot pipe work. The block of bedrooms facing onto the car park the windows have been sanded down and repainted. Work has commenced on areas identified with the care manager at the previous inspection this includes loose roof tiles above the side door and the dining room. The fire officer visited the home and has identified a number of areas where work needs to be carried out a meeting is to be arranged to discuss these areas with the fire officer. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27,28,29,30. As at the previous inspection the staff on duty at Far Fillimore were competent and satisfactory in number to meet the needs of individuals in their care. Staff morale was observed to be high with the staff team working well together with residents to improve their quality of life. There are procedures in place for the recruitment of staff. EVIDENCE: As at the previous visit at the time of this visit to Far Fillimore observations identified that staff on duty were as the rota and were sufficient in number to meet the observed needs of individuals in their care. Robust recruitment procedures are in place to employ staff Criminal Records Bureau checks are carried out along with POVA checks prior to permanent employment being offered. CRB documentation is on staff files There is a staff-training programme in place with records being maintained of training carried out. The home promotes NVQ training ten staff have NVQ Level 2, the Care Manager has NVQ Level 3 the Deputy Manager has NVQ Level 3 and 2 years BA Honours Applied SW, both are doing NVQ Level 4 and the Registered Managers Award. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 32,34,36,37. The Care Manager is supported by the proprietors in providing clear leadership throughout the home with all staff on duty demonstrating an awareness of their roles and responsibilities. EVIDENCE: The Care Manager is experienced and competent to care for older people and is currently working towards her Registered Managers Award. The registered proprietors and care manager ensures so far as is reasonably practicable the health safety and welfare of service users and staff. Resident’s financial interests are safeguarded by the involvement of individuals themselves their relatives/solicitors and staff at the home. As at previous visit finances sampled by the Inspector identified that cash balanced with receipts and records maintained.
