Conditions of registration: 1. The Home is registered for a maximum of 27 service-users to include: *Up to 27 service-users in the category of DE (E) (Dementia over 65 years of age). 3rd February 2006 Date of last inspection Brief Description of the Service: Firwood House is owned by Bolton Council and is run by their Social Services department. It provides specialist care for older people with confusion (personal care only). Up to 27 people can live there. At this time the home has 3 long stay places. However, the home is now, and will in the future only be admitting people with dementia care problems for short stay and respite care, with some emergency beds also available. The home also provides a day care service from Monday to Friday of each week. Firwood House is in a residential area of Bolton, off Crompton Way. It is close to bus services with shops and pubs nearby. The building is on two floors with a passenger lift and there are 27 single bedrooms. The home has five small lounge/dining rooms and there is also a large, central dining area. The building is surrounded by gardens and there is parking to the rear (accessed via Tintern Avenue/Glaisdale Close). Firwood House DS0000030981.V312833.R01.S.doc Version 5.2 Page 5 SUMMARY
This is an overview of what the inspector found during the inspection. An inspection visit (which the home had not been told about beforehand) had been planned for 16th March 2007. However, when we arrived we were unable to go in as the home was closed to admissions/discharges/visitors due to an outbreak of vomiting and diarrhoea (the home had apparently told us about this, but we had not received the information). The two things the home had to do after the last time we visited have been done – the acting manager (Mr Alan Kirk) has been approved by us as the new Registered Manager and the home has updated its registration with us to show that all its places were now for older people with confusion. However, we have recently been told that from 10th April 2007, Mr Kirk is to be replaced by a new manager, Mr Rob Rimmer. What the service does well: What has improved since the last inspection? The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): As we were unable to gain access to the home, none of the above standards could be checked and a judgement could not be made. EVIDENCE: Since the last inspection the home had updated its registration to show that all 27 places provided were for older people with confusion. Due to the home temporarily not accepting any new admissions, the current occupancy level was low (15 residents). The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): As we were unable to gain access to the home, none of the above standards could be checked and a judgement could not be made. EVIDENCE: Since the last inspection the home had experienced two outbreaks of the NORO virus (the first in April 2006, the subsequent and current one began on 1st March 2007). The NORO virus results in vomiting and diarrhoea in those affected. The home had followed advice from Bolton PCT Infection Control service and was currently closed to admissions and discharges. Staff and residents must be clear of infection for 48 hours and the home thoroughly cleaned (including steam cleaning of all carpets) before it is able to re-open. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected. EVIDENCE: Good practice was noted as the home was intending to steam clean (rather than just shampoo) all carpets before re-opening (to properly remove all traces of the NORO virus). The intended outcomes for Standards 27 – 30 are: 27. 28. 29. 30. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): As we were unable to gain access to the home, none of the above standards could be checked and a judgement could not be made. EVIDENCE: The home had confirmed extra care staff were working to support residents who were unwell with the NORO virus and extra domestic staff were working to carry out frequent cleaning of communal areas (e.g. disinfecting door handles, hand rails, toilets at least every two hours). From information provided by the home’s manager, good practice was noted as 71 of care staff had NVQ level 2 or above. Further good practice was noted, as staff turnover was low, with only three new carers starting since the last inspection. We understand that following redeployment within Bolton MBC older people’s services, new staff will be shortly joining Firwood House. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): As we were unable to gain access to the home, none of the above standards could be checked and a judgement could not be made. EVIDENCE: The home had been without a permanent manager since January 2004. Following his successful application, the acting manager, Mr Alan Kirk had been confirmed as the new Registered Manager on 30th October 2006. However, the home is to experience further change, as Mr Rob Rimmer (currently a manager at another Bolton MBC older people’s home) was to replace Mr Kirk from 10th April 2007. Once in post, Mr Rimmer will need to make a formal application to us to become the new Registered Manager for Firwood House. The home may wish to send a letter to residents and their families to update them of the

From information provided by the home’s manager, up to date safety checks had been carried out on the gas boiler, fire equipment, hoists and hot water temperatures.
