What the care home does well We looked at the care records for two people who live in Craignair and tracked aspects of their care to ensure they are being supported properly and safely. People who live in Craignair have been diagnosed with dementia. We found that a lot of information had been gathered about each person by staff before they moved in. This was done through meeting them and consulting with health professionals, social workers and family representatives. The assessment of each person`s needs and preferences continued when they were living in the home. Their care plans had been reviewed by staff every month and/ or in accordance with any change in their condition. There were individual updated risk assessments in place to identify and address hazards to their safety. In this way the home ensures that people are getting the support they need and risks to their wellbeing are well managed. We found people`s physical and mental health needs were being well supported and there were clear records of contacts with health professionals, social workers and paramedical services. We looked at how people`s prescribed medication is managed by tracking medication records for two people and looking at the storage arrangements and procedures in place in Craignair. Medication is blister packed stored in a locked trolley which is chained to the wall to keep it safe. For staff guidance there are medication procedures in place and they have received relevant training. The records of medication given to each person were checked and found to be in order, and all the medication for two people which we checked was in stock. We saw records audit trails, for example for checking drugs into the home each month and for returning unwanted medication to the pharmacy. The manager carries out monthly audits of medication to ensure the systems are being followed and that all medication accepted into the home will be accounted for. There was evidence that people`s medication is reviewed in response to any change in theircondition to ensure that any side effects they may experience will be identified and properly managed. We found that a lot of effort has been made by staff to ensure people`s wishes and opinions are recorded and respected in care giving. There are arrangements in place to ensure those in residence receive religious support if that is their choice. The manager confirmed that the services of independent advocates will be arranged for those who have no family to represent them. We looked at arrangements in place for managing any complaints. The complaints procedure is in picture bank format to make it easy for people to understand. This is given to people who live in the home and their representatives. In this way people know their complaints will be taken seriously. To ensure people will be protected in their home, there are safeguarding and whistle blowing procedures for staff guidance. Records showed us that staff have received safeguarding training and updates to ensure they are aware of the indicators of abuse and how this should be reported for investigation. We received sixteen completed surveys from people who live in Craignair, their responses about quality of care, staffing, meals and complaints management were positive. Some of their views are as follows :"I am happy here." "I have a lie in." "They look after me well." "I like the garden and bring in washing." "My mother is very happy in Craignair. She receives a very high standard of care and attention from staff. I receive a warm welcome when I visit." "Craignair is well kept and well maintained. Staff are good and my mother has a good relationship with them." We received twenty-two surveys from staff who work Craignair. They gave us positive information about training and guidance available , management support and care planning systems. Some of the comments received are :"We provide excellent quality of care and there is a brilliant staff team. There are activities for service users and a nice homely environment." "Staff work well as a team." "Families are kept up to date with the wellbeing of their relatives." "Good teamwork. Caring staff. Home is always clean and tidy and smells nice." "Service users` views, dignity, privacy and independence are promoted." What the care home could do better: The service was rated as excellent during our key inspection 2007. We have not given any requirements or recommendations as a result of this visit because the service was found to be meeting the standards we assessed. From reading the AQAA and in discussion with the manager we found evidence of ongoing reviews and audits of the service. This is to ensure the service will develop and improve in line with current best practice guidance and be influenced by the opinions of people who live in Craignair and their representatives. The service is provided in a converted Edwardian house and provides care and accommodation in a homely setting. Home cooked meals and a laundry service are also provided. The home also provides day-care places for people who have dementia who live in their own homes. Craignair has a spacious and secure garden and is situated in a pleasant residential area, close to a train station and bus routes. The home is owned by Mr. S. and Mrs. B. We carried out a random inspection at Craignair. We have not received any complaints or safeguarding referrals since our last visit and this inspection was scheduled as part of our regulatory activities. We looked at how peoples health and personal care needs are being met in Craignair. We also looked at how complaints are being managed and the systems in place to protect people who live in the home from harm or abuse. The visit was just one part of the inspection. Before the visit took place, we asked the manager to fill in a questionnaire. This is called the annual quality assurance assessment (AQAA). This gave us information about how the home is meeting the needs of the people who use the service and includes plans for further development. We sent out surveys on the standard of service in Craignair to people who live and work in the home and some of their views are included in this report. During our visit, we walked round the home and we checked care and safeguarding records. We spoke with people who live at the home and with the manager and staff. One of the homes owners spoke with us about how the service has developed since our last visit in 2007 when Craignair was rated as excellent. We used a system called case tracking. This is a way of inspecting that allows us to follow or track a particular situation to ensure that it was managed properly. What the care home does well:
