This inspection was carried out on 26th July 2005.

CSCI has not published a star rating for this report, though using similar criteria we estimate that the report is Good. The way we rate inspection reports is consistent for all houses, though please be aware that this may be different from an official CSCI judgement.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Staff working in the home were observed to be caring towards residents and were aware of residents likes, dislikes and needs. Residents said that their relationships with the staff were good and staff were caring and hard working. Visitors said that they felt welcomed by the staff and could visit at any time. One visitor commented that "visitors are never made to feel like a nuisance" with another visitor stating "can`t speak highly enough of the staff."

What has improved since the last inspection?

The staff team have consulted with residents about the activities and outings they would like to have in the home and arrangements are place to put suggestions into effect. The laundry service has been improved with all residents having their washing done individually. This has ensured the residents receive their own clothing after laundering.

What the care home could do better:

Residents` personal accommodation and communal areas would be improved by replacing dated and worn wall coverings and carpets. The County Council are in the process of addressing the need for the registered provider or delegated person to visit the home monthly and write a report upon the conduct of the care home.

It provides permanent care, short stays and day care. The home is situated in the village of Studley, which has a variety of shops, churches, public houses, social clubs, a library and community centre. There is a regular bus service to Redditch town centre. The home provides accommodation on two floors. There are four units, one of which provides short stay or respite care, and one of which provides care specifically for people with dementia. Each unit has a lounge/diner with kitchenette, and there are additional communal areas, including two conservatories, on the ground floor. The home has a hairdressing salon. On each floor there are bathrooms and lavatories suitable for people with physical disabilities. The kitchen, laundry and staff offices are situated on the ground floor. There is a shaft lift to the first floor. This inspection was an unannounced inspection and took place over two days between the hours of 10am and 3pm. This was the first visit for this inspection year. The care officer on duty in the home was present during the inspection. The inspection process involved a tour of the home, talking with the care officer, examining records and care plans, observation of care practices along with discussions with residents, staff and two relatives who were visiting during the inspection. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 3. Standard 6 is not applicable for this service. Assessments are undertaken to ensure that the home can meet the person’s identified care needs. EVIDENCE: The admissions procedure includes an assessment by management or senior staff in the home completed before admission. Records showed that assessments had been completed and this provided the necessary information for the home to be able to provide the care needed. The community care plans provided by the social worker, as part of the individual needs assessment process, were seen within the residents care plans reviewed during this inspection. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8 and 10 Health needs of residents are met with evidence of liaison with health and social care professionals on a regular basis. Service users feel they are treated with respect and dignity. EVIDENCE: Three resident care records were examined. Service users health, personal and social care needs were set out in the care plans examined. Care plans were updated at least monthly, however, a completed monthly review form seen in one of the files had not been dated. The initial assessment of this resident had not been dated or signed by the member of staff undertaking the assessment. It is important that documents are signed and dated to ensure that staff deliver the up to date assessed care needs of the resident. One of the care plans had not been signed by the resident to demonstrate their involvement and consultation in this process. Daily entries seen in care records show that staff follow up health concerns and that there is good liaison between care staff, residents GPs and District Nurses who visit the home daily. Staff also seek assistance and professional help from others outside the home, such as dieticians, community nursing services, and community psychiatric nurses. Service users confirmed that they had received visits from the GPs and District Nurses when necessary. The residents were seen to be treated respectfully by staff and appropriate facilities and space is provided for their personal care needs and private or personal visitors. The residents spoken with confirmed that privacy and dignity was respected and that they were assisted tactfully and sensitively. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13 and 14 Residents were satisfied with their lifestyle in the home and they had been able to exercise choice and influence decisions affecting them. Contact has been maintained with relatives and friends of residents. Opportunities to access the local community have been made available. EVIDENCE: There is a planned programme of activities and entertainment in the home. The home employs a dedicated activities co-ordinator for 6 hours per week. Staff also undertake regular activities with residents. Activities listed on the programme displayed in the home include bingo sessions, reminiscence, life history, card making, painting, board games and manicures. A resident was observed to have a manicure during the inspection visit. Trips out to local places of interest are arranged. The home had the use of a minibus. Entertainers also visit the home and church services are held there. Residents said that they could choose how they spend their day and could take part in the social activities if they wished to. Residents meetings are held regularly. The minutes of the meeting held in May was displayed. The minutes demonstrated that residents have a say in the running of the home with several suggestions on activities they would like to see delivered. Most residents have good contact with their relatives and a good number of residents go out with their family on a regular basis. Family and friends are welcomed at the home and are invited to attend parties and other celebrations. A relative of one service user stated that she visits daily and has Sunday lunch with her relative every week. All relatives who spoke to the inspector said they are given a warm friendly welcome by all the staff whenever they visit. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 Concerns or complaints are dealt with promptly and professionally. EVIDENCE: The home has a clear complaints procedure, which is explained in the homes statement of purpose and given to all prospective service users or their family/representative before moving into the home. The complaints procedure is also located on a notice board in reception. The CSCI has not received any complaints since the last inspection visit. Residents and relatives spoken with were aware of the complaints procedure and how to make a complaint if they needed to. They were satisfied that any issues taken to the manager would be dealt with promptly. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21 and 24 Abbotsbury is generally well maintained and residents live in a safe and comfortable environment. EVIDENCE: A tour of the home was conducted during which both floors were visited. Some of the bedrooms were inspected together with the communal areas, toilets and bathrooms. Four Acres has ample communal space having 5 lounges and 2 conservatories, one of which is a designated smoking area. The walls and door frames in corridors were marked and scuffed. This creates a poor first impression for visitors. All bedrooms have an en suite toilet and wash hand basin. The home has five assisted bathing facilities, one of which is an assisted shower. There are sufficient toilets near communal areas. Personal possessions were noted in the bedrooms viewed during the inspection visit. Some bedrooms require re-decoration with torn or faded wallpaper evident. Carpets in two rooms were stained with an offensive odour evident in one of the rooms. The home was clean and tidy on the day of the inspection visit. JUDGEMENT – we looked at outcomes for standard(s) 27 The home employs sufficient staff to meet the needs of its residents. EVIDENCE: The current duty rotas were examined. On the day of the inspection visit there were six staff on duty plus the care officer. The home employs two waking night care staff, and sufficient numbers of domestic staff to ensure that standards relating to food and meals are met and that the home is maintained to a clean and hygienic state. There were 34 residents living in the home on the day of the inspection visit.
