What the care home does well

NHS and other health professionals are actively involved in the care of residents as needed. The residents spoken to told the inspectors that the staff are kind and caring, and that their privacy and dignity are respected. A growing range of activities is offered, with both individual and some group activities being available. Ongoing training and development was seen for all staff. The residents are looked after as individuals, and all residents told the inspector that they enjoyed the "family atmosphere" of the home.

The home is a large detached house standing in attractive landscaped gardens in the Aigburth area of the city. The accommodation comprises of 33 single bedrooms with en-suite facilities and 1 bedroom without en-suite. There are two lounges, spacious dining room and a hairdressing room. The home has a main laundry and a resident’s laundry to encourage independence. Tea making facilities are available on each floor for residents and visitors use. Fees are from local social services rates to privately agreed individual prices. This unannounced site visit took place over five hours. The manager, Ms Ester Markey, accompanied the inspector throughout the inspection period. Personal files of both residents and staff were inspected, and a tour of the building took place, which included all communal areas and a selection of resident’s bedrooms. Staff and residents were spoken with as regards their view about the care home. The care home was clean tidy, had no malodour, and had adequate staffing levels to care for the residents. What the service does well: What has improved since the last inspection? The overall management of this home continues to improve. The registered manager is experienced and shows a good understanding of the National Minimum Standards and accompanying regulations. The environment of the home has improved in most areas, especially communal rooms and corridors. Required documents and records have also improved, in particular the new care files for residents. The activities offered and conducted continue to improve – much to the appreciation of all residents spoken to. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 1, 2, 3, 4, 5. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The residents assessed needs are being met, and the home is able to provide assurances to residents and their representatives that assessments will be a continuous process throughout the resident’s stay. EVIDENCE: As part of the site visit the inspector case tracked a number of resident’s personal files from pre-admission up to date. All residents have a pre admission nursing assessment before they are admitted to the home; the assessment is undertaken by the manager or one of the deputy managers in the home. Residents, family and other health care professionals known to the resident contribute to the assessment. On the basis of the completed pre admission nursing assessments, the resident’s care plan is drawn up with the involvement of the resident, their Fulwood Park (Methodist Homes For The Aged) DS0000025389.V314059.R01.S.doc Version 5.2 Page 9 family / representative as appropriate, and key staff. Multidisciplinary healthcare team (MDT) involvement was also clearly apparent. It was recommended to the manager that greater clarity in the recording and documenting of care plan reviews would further improve standards, and provide additional evidence of the actual care assessment and care delivered. Fulwood Park does not provide intermediate care, so Standard 6 is not applicable. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7, 8, 9, 10, 11. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There is a care planning system in place for all residents. This provides staff with most of the information they need to meet the resident’s needs. Medication management was compliant with current good practice requirements and guidelines, thus helping to protect the safety of residents. EVIDENCE: A random sample of new residents files and long stay residents were reviewed during the case-tracking process. The pre-admission documentation used was informative, concise and contained sufficient detail to inform the care plans once the resident was admitted. The care plans were generally completed to a high standard and contained evidence of multi-disciplinary involvement in residents care. The care plans were reviewed on a monthly basis or more often if required. The administration, recording and storage of medications were examined on all units and evidence that they were managed and recorded in accordance with current good practice requirements and recommendations. Staff were observed knocking on resident’s bedroom doors before entering, and seen discreetly assisting residents with their daily needs. Residents spoken to told the inspector that the “staff are nice”, and that they felt “looked after”. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14, 15. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Residents have choice and flexibility how they spend their day in the home, and pursue leisure activities according to their choice and preferences this allows independence and individuality for each resident. EVIDENCE: Residents in the home are asked on admission about their lifestyle, choice of foods, and choices and preferences of the social activities they would like to participate in. On admission to the home the resident with help from a family member completes “Getting to know you” questionnaire, which is a “Work life History” of the resident, and includes schooling, work, hobbies, food likes and dislikes etc. This information is used to facilitate organised activities for the resident. The garden area has been developed with garden furniture and patio areas. The inspector was informed that further developments, such as a flower planting area for residents, are being considered for the summer. Fulwood Park (Methodist Homes For The Aged) DS0000025389.V314059.R01.S.doc Version 5.2 Page 13 The home employs a full time activities coordinator for the resident’s activities who is knowledgeable and experienced in suitable activities for the residents, and is aware of the National Association of Patient Activities (NAPA), which is a national organisation that promotes activities for residents in all health care settings. The kitchen areas were clean and tidy, and appear well managed. A wide choice of foods is on offer each day, with additional choices being offered to each main meal. Residents spoken to told the inspector that the “food was lovely”, and they looked forward to their meals. Staff assistance was offered to les able residents in a discrete and unobtrusive manner. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. This judgement has been made using available evidence including a visit to this service. A complaint and adult protection policy and procedure was in place that helps ensure the safety and welfare of residents. EVIDENCE: Since the last inspection there has been no complaints to the Commission about this home. Many of the residents used their postal vote in the last General Election. The care home has up to date information on the Protection of Vulnerable Adults (POVA), and this information is communicated to new employees on their induction course. On the day of the site visit there was evidence that many of the staff in the home had undertaken training on Wirral POVA protocols, and the Whistle Blowing Policy. Advocacy information is available on each unit for residents and their families as needed. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 19, 20, 21, 22, 23, 24, 25, 26. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. The standard of decor within this home is good, with evidence of continuing improvements, through maintenance and planning. The home presents as a homely, safe and comfortable environment for the residents. EVIDENCE: Fulwood Park (Methodist Homes For The Aged) DS0000025389.V314059.R01.S.doc Version 5.2 Page 16 The premises were clean and odour free, and there was evidence of on-going re-decoration and refurbishment. All rooms seen were decorated and furnished to a high standard, and many areas of the home had been decorated and / or refurnished since the previous site visit. Most of the residents had personalised their rooms with small items of furniture, photographs and soft furnishings; and they are able to have a direct input into their colour schemes, etc. The communal lounges were bright and inviting, and the dining room was nicely furnished and the tables well presented. All required safety certificates for the building were up to date and valid, including the fire risk assessment and related staff training. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 27, 28, 29, 30. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. There is a consistency of care within the home provided by permanent staff, which helps to offer safety and stability for the residents. EVIDENCE: References were seen in all staff personnel files, and CRB and POVA information was also evident. The registered manager was clearly able to inform the inspector of their knowledge of the National Minimum Standards and accompanying regulations regarding the recruitment and retention of staff. Training and inductions, including specific training on the protection of vulnerable adults was seen for most staff. The manager has developed a training plan for all staff, accessing many external training providers. The range of training and development includes medication management, moving & handling, health & safety, pressure sore prevention and NVQ in Care. This should continue to be built upon. The manager has also established inhouse “link people” to obtain additional information and training as needed. Their role extends to cascade new information to the remainder of the home’s staff. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. This judgement has been made using available evidence including a visit to this service. This home benefits from a competent manager and staff team, thus helping to improve the quality of care given to residents. EVIDENCE: The manager (RM) is registered with CSCI, has completed her NVQ4 Registered Manager’s Award, and has, in conjunction with her deputy managers and staff team, made an ongoing improvement to this service. Staff morale appears to be high, and the staff spoken to said they feel valued and appreciated, and are happy coming to work. Records are kept in accordance with the Data Protection Act 1998, and audits were available for most aspects of record keeping. The company’s representatives also carry out regular internal audits. This system helps to maintain high standards and to further develop the service in line with changes in legislation and good practice.
