This inspection was carried out on 10th May 2005.

What the care home does well

The home has a core staff team that are experienced and committed to providing a high standard of care to the residents at the home and a positive impact has been made on the quality of life experience by the change of management. Residents consulted advised that they were happy at the home and had positive comments to make about their carers. Appropriate interaction was observed and good relationships were clearly demonstrated between carers and residents during the inspection. Meals are varied and appear well balanced with choice always available. Home made cakes and tarts etc are produced on the premises by the home`s cooking personnel.

What has improved since the last inspection?

The lounge/dining room of the home has been redecorated and soft furnishings have been replaced with new comfortable chairs provided. Two bedrooms have been redecorated and new carpets supplied. The call bell system has been replaced and the kitchen has been refurbished with a new cooker made available. Medication has been moved to a locked dedicated facility with a new secure metal trolley used when medication rounds are undertaken. A key worker system has been introduced with some development of the senior staff roles and new detailed care plans have been produced for each person receiving care at the home. Activities have been developed further and a programme is displayed on the wall to enable residents to plan their time. Meals are no longer plated up in the kitchen but are served at the resident`s dining tables with vegetables available where residents can help themselves.

The home has a large communal lounge with a dedicated dining area attached. All the rooms are on suite; there is one double room available the remainder are all single occupancy. The home has two floors with a vertical lift provided for less mobile service users. The home is situated by the green at the top of the hill above the harbour of Brixham. The home has a well-stocked garden with a central lawn area; to the side of the building is a hard standing car park, which is large enough for several vehicles. This announced inspection took place over 12 hours on 10th and 11th May 2005. A pre inspection questionnaire had been received from the home. Since the last inspection the proprietors have changed. A complete tour of the home was undertaken inside and out, a sample of staff and care records were examined. Five residents were asked for their views of life at the home; this was undertaken in private and three carers were also consulted. Two visitors were spoken to and the district nurse. The inspection was undertaken with the assistance of the registered manager. What the service does well: What has improved since the last inspection? Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 - 5 Comprehensive information is available to ensure that prospective residents are able to make an informed decision about admittance before moving in. Good quality assessment is undertaken and time is taken to explain the service available at the home. Prospective residents are encouraged to visit the home prior to moving in and use the services and a written contact/terms and conditions document is provided promptly. EVIDENCE: A detailed statement of purpose and service user guide was available and this provided the necessary information to ensure that prospective residents were able to make an informed choice about being admitted into the home. Residents consulted during the inspection advised that they felt they had all the information they needed about the home prior to moving in and also confirmed that they were encouraged to visit the home with their family to sample the service before making a decision about admittance. If a person is admitted on an emergency basis an assessment is undertaken as a matter of priority to ensure that their needs are met. A written contract/terms and conditions document is provided to new residents shortly after moving in. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 - 11 Good quality care plans are available for each person receiving care with health and social needs recorded. Service users are given the option to self medicate and they are treated with respect at all times and their privacy is maintained. Service users are secure and the arrangements for dealing with their death are appropriate. EVIDENCE: The new management of the home have introduced a more comprehensive care planning approach to ensure that all residents’ needs are met, the plans are agreed with the resident concerned and signed by them. Good detail is recorded of health needs and appropriate access is provided to healthcare professionals. A comprehensive social programme of activities and trips has been introduced which residents said they enjoyed. The social events are displayed on the wall to enable residents to make their choices about what they would like to attend. Residents are enabled to go shopping with a carer on a weekly basis. Care plans are reviewed on monthly to ensure that they are kept up to date and changing residents needs are met. Residents can self medicate if they wish and they have the capacity, this is subject to a risk assessment approach to ensure residents’ safety. Appropriate interaction was observed during the inspection between residents and their carers, residents were respected and care was taken when toileting and bathing to ensure that residents’ privacy was maintained. Staff were also seen to knock on doors before entering a residents room. The residents consulted advised that they were very happy with the service provided, one resident said “I’ve never been so well looked after” and another advised “ I cannot think of anything else they could provide to make life better for me”. Two relatives visiting Furzeham Lodge said, “We were not looking forward to visiting in a residential home but have been surprised by the quality of the service and surroundings and we would quite like a bed ourselves”. The home has a policy and procedure in place concerning the issues that arise from the terminal phase of a resident’s life to ensure that this is appropriately handled. Details are held in the care plan of the resident’s wishes at their death. These matters are dealt with sensitively by the management of the home and are usually brought up by the residents themselves. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 - 15 Residents are offered a comprehensive social programme of activities and trips out that meet their needs. Residents maintain contact with family and friends and are also enabled to have control and input to decisions that affect them. A balanced diet is provided with choice is always available for residents. EVIDENCE: The provision of a social programme has been seen as a priority by the new manager and residents confirmed that this has been a pleasant surprise for them. A range of activities is provided for residents and trips out are regularly undertaken, accompanied shopping excursions are offered on a weekly basis. Residents advised that they have never had so many options concerning what to do. One resident said, “ Things have improved a lot here”. A display on the wall shows what activities are available for a month, this enables residents to plan their time and make decisions about what events they would like to attend. One resident that suffers from cataracts on his eyes was pleased to say that he has been supplied with the talking book service for the first time which he said has made a great difference to him. Visitors consulted advised that they were made welcome when they came and were given tea if they wanted it. When residents are admitted into the home time is taken to explain the service that is available. This approach is continued and carers were observed explaining to residents their options and asking for their views about what they would like to do, time and space was given to allow residents to make up their own mind. The way meals are served has changed completely since the new management have been in place. Food is put into a heated trolley and taken into the dining room, vegetables are then put on the individual tables and the plates are served at the tableside. Mealtimes have become a social event in themselves with constant interaction taking place between residents. A glass of wine is offered to residents to enjoy with their meal and choice is always available about what foods they would like. All residents consulted stated that the meals provided at the home were to their liking. Service users and their relatives and friends are confident that their complaints will be listened to, taken seriously and acted upon. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 - 18 Residents were confident about using the complaints procedure at the home and appropriate action is taken to ensure residents legal rights are protected. An appropriate policy and procedure is in place to ensure that residents are protected and carers are trained in its use. EVIDENCE: Five residents were consulted during the inspection, they had confidence that if they raised an issue with the manger it would be dealt with quickly to their satisfaction. They were aware of the complaints procedure and were also clear that they could refer issues to the Commission if the home was unable to resolve the matter raised. Residents are offered a postal vote if they would like to participate in an election or they are offered transport to the local polling station. Residents advised that they were quite happy with the arrangements in place. The home has available a comprehensive protection policy for residents to ensure that they are not subject to any type of abuse. Carers are trained in its use and when three carers were interviewed in private it was clear that these arrangements were well understood. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 26 The home has been refurbished and is well maintained. Residents’ rooms are personalised and comfortable. High standards of hygiene were apparent throughout. EVIDENCE: The lounge/dinning room has been redecorated, soft furnishings have been replaced and new comfortable furniture has been provided. Residents were keen to say how much they enjoyed this change. Carpets have been replaced with new and two bedrooms have been refurbished. The kitchen has also been refurbished and a new cooker has been provided. The home has a well stocked garden with a substantial concrete path providing a circular walk, this provides an outside facility that some residents said they enjoyed the most. All the bedrooms at the home have on suite facilities and an assisted bathroom and shower is available for residents. All residents’ bedrooms were seen, they were comfortable and well maintained, ample evidence was observed of personalisation and residents said they had been able to bring some of their possessions with them when they were admitted and that they liked their rooms and the on suite facilities. All rooms in the home were seen and high standards of hygiene were apparent throughout. The laundry area is currently being refurbished; a new washing machine has been purchased by the new proprietors to ensure that appropriate washing programs are available for soiled items. Due regard is paid to infection control to ensure the safety of residents. Service users needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 27 - 30 Residents are appropriately cared for by a committed staff team that is recruited appropriately. Staff understand their roles and are trained and supervised by management. EVIDENCE: The home has available clear job descriptions for all employees. Carers understood what their roles were and readily accepted training. A training and development programme is in place. The deployment and number of staff available throughout the day and night is sufficient to meet the needs of the residents. Residents confirmed that their call bells were answered promptly. The management have ensured that appropriate references and checks were undertaken on new carers to ensure the residents safety. Two residents expressed regret that there has been staff changes in the care team when the new management took over as they said these people were their friends. They understood that they were free to visit if they wished and residents did say that the home had been improved. Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31,33,35 and 38 The staff group has good quality leadership provided by the manager and the home provides a flexible service for the benefit of the residents. Appropriate systems are in place to manage residents pocket monies and due regard is paid by management to ensure the health and safety of the staff group and residents. EVIDENCE: The registered manger of the home has some thirty years experience as a nurse and has a current pin number. She also has twenty years experience in residential care and is just completing the registered managers award. Staff and residents expressed their confidence in her. The manger currently looks after some residents’ pocket monies, this is held in a safe in individual containers. When withdrawals are made two staff signatures record the transaction and receipts are retained for any purchases made on behalf of residents. This system ensures that appropriate safeguards are in place for residents’ monies. The home does not get involved in any other financial affairs of residents. The manger gives a high priority to the welfare and health and safety of staff and residents. The regulations for the secure handling of hazardous cleaning materials are observed and appropriate reporting is undertaken of serious events in the home. Electrical equipment and the electrical installation of the home are appropriately tested and regular servicing of machines, lift etc is undertaken to ensure the safety of staff and residents.
