Conditions of registration: Date of last inspection 10th May 2005 Brief Description of the Service: Furzeham Lodge is an extended detached building that provides 24-hour residential care for up to 21 persons in the categories of old age and physical disability over the age of 65 years. The home has a large communal lounge with a dedicated dining area attached. All the rooms are on suite; there is one double room available the remainder are all single occupancy. The home has two floors with a vertical lift provided for less mobile service users. The home is situated by the green at the top of the hill above the harbour of Brixham. The home has a well-stocked garden with a central lawn area; to the side of the building is a hard standing car park, which is large enough for several vehicles. This is an overview of what the inspector found during the inspection. This unannounced inspection was undertaken over five hours on 6th December 2005. A tour of the home was completed that covered the communal areas of the building. Five residents were consulted individually about the service provided and a sample of care records were examined. The inspection was undertaken with the registered manager and the deputy. What the service does well: What has improved since the last inspection?
Since the last inspection in May 2005 the whole of the outside of the building has been painted and the redecoration and refurbishment of the inside of the home has continued. There were some changes to the care personnel earlier in the year, however, the home is now fully staffed with a stable team and a detailed training programme is in place. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 3 and 6 Detailed and comprehensive assessments are undertaken of all prospective residents prior to them moving into the home to ensure all their needs can be met. EVIDENCE: An assessment was available for each resident receiving a service at Furzeham Lodge and three were examined in detail. These demonstrated that needs were assessed in the areas of health, personal and social this comprehensive approach ensured that the resident received the service they required and there were no needs that remained unmet. Healthcare professionals and family were consulted as part of the assessment process if necessary. Standard 6 refers to a service not provided at Furzeham Lodge. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 7,8,9 and 10 Detailed appropriate care plans are available for each service user, needs in the areas of health; personal and social are clearly shown. Residents are able to self medicate. The administration records of medication undertaken by the home were incomplete which has the potential to put residents at risk. Residents were clearly treated with respect and their privacy was maintained. EVIDENCE: A comprehensive care plan was available for each service user in the home. Four of these were examined; they all clearly demonstrated the needs of residents in the areas of health, personal and social and how these were met. The care plans were reviewed on a monthly basis or more often if the resident was going through a period of constant change. Five residents were consulted individually in private and they were all very complimentary about the way they were cared for at Furzeham Lodge and advised that all their needs were met. Residents that wished were able to self medicate at the home subject to a risk assessment process to ensure that they had the capacity. The records of the
Furzeham Lodge Care Home DS0000063694.V271029.R01.S.doc Version 5.0 Page 10 administration of medication undertaken by the home were examined and several gaps were apparent in the issue record and the checking and booking in of medication when received was not complete, this has the potential to put residents at risk. A requirement has been raised in this report to ensure that all staff that administers medication adheres to the policy and procedure that is in place. When the medication administration sheets were examined it was clear that the home was undertaking a process beyond issuing medication according to the prescription. This approach has been discussed with the nursing regulation manager who has advised that the home should agree an approach with the resident’s doctor, ensure that all carers are appropriately trained and produce a protocol that must be adhered to. One element of the protocol should direct the member of staff concerned to advise healthcare professionals if the circumstances of the resident demonstrate that the medication should not be administered. This procedure will ensure that this resident is not at risk. Five residents were consulted in private individually during the inspection process; they were all very complimentary about the home and the service they received. Carers and the quality of the food were both praised. One resident who was interviewed said, “I don’t want to be here, but if I have to be somewhere this is the best place, I’m very well looked after” another advised, “I wasn’t sure what it was going to be like when the new people took over, but its turned out better”. Good natured banter was overheard time and again and it was clear from observations made that care was taken by staff not to rush residents and their privacy was maintained when they were assisted into the toilet etc. All residents consulted were very clear that they felt respected at all times in the home. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 12, 13, 14 and 15 The residents are offered a comprehensive social programme of activities and trips out that meet their needs, they are able to maintain contact with family and friends. Residents are also enabled to have control and input to decisions that affect them. A balanced diet is provided and choice is always available. EVIDENCE: Five residents were consulted as part of the inspection process, they advised that the lifestyle at the home had improved under the new management and no suggestions could be made about how things could be improved. A range of activities is provided for residents and trips out are regularly undertaken, accompanied shopping excursions are offered on a weekly basis. One resident remarked “ I have plenty to do if I want it or I can just do my own thing in my room”. There is an unrestricted visiting policy and procedure at the home and visitors are made welcome. Residents also confirmed that they could see visitors at anytime and come and go as they pleased. When residents are admitted into the home time is taken to explain the service that is available. This approach is continued and carers were observed
Furzeham Lodge Care Home DS0000063694.V271029.R01.S.doc Version 5.0 Page 12 explaining to residents their options and asking for their views about what they would like to do, time and space was given to allow residents to make up their own mind. Life is not regimented at the home residents decide when they would like to go to bed and when they would like to get up and when asked residents said they felt in charge of the decisions that affected them. All the residents consulted advised that they liked the meals served in the home; the quality and the quantity were praised. The meat course is taken into the dining room in a heated trolley and vegetables are then served at table. A glass of wine is offered to residents to enjoy with their meal if they would like it or alternative soft drinks are available. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 16 and 18 Residents were confident about using the complaints procedure at the home and prompt action is taken by the management to resolve issues raised. An appropriate policy and procedure is in place to ensure that residents are protected and carers are trained in its use. EVIDENCE: The home’s complaints policy and procedure was readily available in the home and residents were confident that if they raised an issue it would be taken seriously by the management and quickly resolved for them to their satisfaction. No complaints were received during the inspection and none were outstanding at the time of the inspection. The home has available an appropriate comprehensive adult protection policy and procedure, all carers are trained in its use to ensure that residents are safe at all times. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Furzeham Lodge provides a safe, comfortable and well-maintained environment for residents. The home is clean, pleasant with high standards of hygiene evident. EVIDENCE: A complete tour of the communal areas of the home was undertaken at the inspection, the home was clearly well maintained and a comfortable environment was provided. Five residents were consulted who were all very complimentary about the home. One resident said,“ The facilities suit me and I like the lounge and dining area running together, there’s plenty of room” another resident remarked “. ……there’s always somebody working here the new people keep it well up together”. The laundry floor surface was well worn and needed attention to ensure the surface was impermeable; a requirement has been raised in this report to ensure this achieved.

JUDGEMENT – we looked at outcomes for the following standard(s): 27 There are always adequate numbers of experienced carers on duty to meet the needs of the residents at the home. EVIDENCE: The no of care hours available at the home remains at the same level as at the last inspection, this provides sufficient staff to ensure that residents’ needs are met. Five carers were consulted during the inspection who confirmed that if they used their call system it was answered promptly and they did not have to wait when they needed assistance with personal care. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38 The health and safety and the welfare of staff and residents is promoted and protected by the management of the home. EVIDENCE: A tour of the communal areas of the building was undertaken during the inspection, no hazards were apparent to put residents at risk. All chemicals that could be a hazard are held in appropriate secure storage and reports are made of any dangerous occurrences as required. The management undertakes regular risk assessments of the building to ensure the environment is safe and all equipment is regularly serviced. The recording of the fire precautions undertaken were examined and found to be clear and up to date which ensures that residents are not at risk.
