Service Users up to 5 LD and up to 5 LD(E) up to a maximum of 5 Service Users 6th December 2005 Date of last inspection Brief Description of the Service: 24 Gargrave Road is a care home owned and operated by North Yorkshire County Council and registered to provide a service for 5 adults of either gender aged over 18 years who have a learning disability. It concentrates exclusively on providing a temporary home for people living in the Craven area. The home is located in a mid-terrace Victorian property within walking distance of the centre of Skipton. There is a lounge, dining room, kitchen and bedroom on the ground floor and three bedrooms on the first. Details provided in January 2006 outline the daily fees as between £8.91 and £56.00. This is an overview of what the inspector found during the inspection. The accumulated evidence used in this report has included: • • • • A review of the information held on the homes file since its last inspection. Information submitted by the Registered Provider in the Pre Inspection Questionnaire. Surveys received from four service users, and four relatives. An unannounced visit by one inspector to the home lasting seven hours. This visit included a tour of the premises, examination of records, observation of care practices and talking to care staff, relatives and management. Looking at four service user care files in detail. • What the service does well:
The home offers a friendly, clean and comfortable place for people to stay temporarily. The home has a established staff team who are fully aware of the needs of the service users. Relatives made positive comments about the home ‘its an excellent respite care,’ ‘such good support for us and may I add how helpful and accommodating the staff are even at short notice.’ The home understands the importance of keeping in touch with service users until they are admitted for temporary care. This prevents the service user from becoming anxious about their approaching stay at the home. Service users are encouraged and helped to continue with their normal daily routine whilst staying at Gargrave Road, this provides the service user with some stability and enables them to settle quickly into the home. Service users are involved in meaningful day time activities of their own choice and according to their individual interests and capabilities, they have been fully involved in the planning of their stay. This helps them maintain control of their lives. North Yorkshire County Council is in the process of building a new home. They hasve actively involved service users and their representatives with the development of the new home, this makes sure the new building will meet their needs and gives service user a sense of ownership.

The intended outcomes for Standards 1 – 5 are: 1. 2. 3. 4. 5. Prospective service users have the information they need to make an informed choice about where to live. Prospective users’ individual aspirations and needs are assessed. Prospective service users know that the home that they will choose will meet their needs and aspirations. Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 1, 2 and 4. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users and their representatives have the information needed to choose a temporary home, which will meet their needs. EVIDENCE: The home provides service users and their relatives with written information about the home before they go to stay. A relative confirmed they had received enough information about the home. Staff and the acting manager both explained following referral from a care manager for temporary care and receipt of an outcome based care plan from the manager. Staff visit service users at their homes and complete a assessment. Service users are then encouraged to visit the home. The allocated worker will then attempt to keep in contact with service users and relatives until they come into the home for temporary care using both the telephone and a newsletter. Gargrave Road (24) - North Yorkshire County Council DS0000034639.V327650.R01.S.doc Version 5.2 Page 9 This was also confirmed by a relative who explained how staff from the home had attended care planning meetings and visited to ask about the likes and dislikes of the service user in detail before they went to stay in the home. An example given of the information the home asked for was whether the service user preferred a light on during the night. Four-service user records were looked at all contained a detailed assessment of the service users needs by the care manager, all had been reviewed six monthly. A member of staff explained some of the service users use a communication book, which the service user keeps with them. This is where the staff and service user write together about their stay in the home, and the relatives respond writing about the service users day whilst at home. This helps the home continue the daily routine of the service user. The acting manager explained how they would try to make sure service users who know each other well come in to the home at the same time. A relative commented how helpful the staff had been when accommodating a service user to stay at short notice. Service users during the site visit made positive comments staying at the home and how the staff were able to meet their needs. Comments made were ‘Its lovely’ and ‘comfortable’. Five relatives who returned surveys all stated they were satisfied with the overall care. The Commission considers Standards 6, 7 and 9 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 6, 7 and 9. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Although service users are involved in making decisions about their lives and play an active role in specifying the care they receive, this is compromised by the homes written records. EVIDENCE: The home has a established staff team who are fully aware of the needs of the service users. Relatives made positive comments about the home ‘its an excellent respite care,’ ‘such good support for us and may I add how helpful and accommodating the staff are even at short notice.’ Four-service user files were examined, each contained an outcome based care plan completed by the Care Manager before admission. Generally, all had a service user plan that showed involvement of the service user. However, they did not have all the information from the care management assessment

Also, the plans had not been reviewed regularly. Where the plans contained more detail, they gave examples of many choices made by service user in regards to leisure time and personal hygiene. During the site visit a service user made the choice to go out in the morning for breakfast, another chose to go shopping in the late afternoon. Staff were observed allowing the service users to make their own decisions about what they wanted to do in regard to their daily activities. The acting manager and a member of staff agreed some of these documents needed further detail to enable staff provide for the service user. They explained and the records confirmed any service users who were to stay at the home, care needs were discussed at the staff meeting in detail and this information was recorded as part of the staff minutes. This issue of recording service users names in staff minutes had been discussed at the previous inspection and a requirement made. The home needs to consider reviewing where it records the discussions in the staff meetings about the care needs of the service users who are coming into the home. To make sure all staff has information of how to provide for the service users and to make sure the home is working in line with data protection guidelines. The home does keep some money for service users when requested; a service user money was checked was found to correct. All files contained risk assessments, which were generally detailed, up to date and had been reviewed dependent upon how long and at what intervals the service user stayed at the home for. Discussion with the acting manager and staff evidenced they were aware of any possible risks to the service user. Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12,13,15,16 and 17. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. People who use this service are able to make choices about their lifestyle and are supported by staff to carry these out. EVIDENCE: Generally, service user files examined contained some details of the service users preferred daily routines and leisure interests; details included work, day centres and colleges. The service users generally only stay at the home for short periods of time, therefore Gargrave Road will try to ensure activities carried out at home are continued when they stay. This was evidence by a relative who said the service user had been taken out into the community during their stay, and daily contact sheets which showed service users attending work or day centres or carrying out social activities. During the site visit service users returned at teatime following attending various activities.
Gargrave Road (24) - North Yorkshire County Council DS0000034639.V327650.R01.S.doc Version 5.2 Page 13 The Registered Manager explained some service users carry a communication book which details the activities the service user has been involved in at their own home and whilst staying at Gargrave Road, this enables the staff to provide the service user with a consistent lifestyle. Generally, all the relatives who returned their surveys said they were informed of important matters. The contact sheets showed the home attempts to keep in contact with the service user and their relatives through out the year by telephone and newsletter. The home has a laptop computer for the service users to use in the lounge. The notice board in the hallway had details of forthcoming events in the community, and a staff member explained they try to keep up to date with community events. Some service users chose to go out during the day and stay in on an evening. Staff were observed cooking for service users during the site visit, service users were given a choice of menu and the food was well presented and nourishing, all service users during the site visit said the food was very good. The intended outcomes for Standards 18 - 21 are: 18. 19. 20. 21. Service users receive personal support in the way they prefer and require. Service users’ physical and emotional health needs are met. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 18, 19 and 20. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users health and emotional care needs are met. EVIDENCE: The home operates a key worker system, and staff confirmed they attempt to maintain contact with the service user whilst they are at their own homes. Overall, the established staff were aware of the personal likes and dislikes of the service users in regards to their personal care and daily routine. Service users said the staff were lovely and staff were observed to be responsive to the varied and individual requirements of the service users. Where service users have difficulty in communicating verbally staff have been trained to use sign language and visual picture cards. Due to the length of stay, most of the service users access specialist services and general practitioners whilst at their own homes. Although there was
Gargrave Road (24) - North Yorkshire County Council DS0000034639.V327650.R01.S.doc Version 5.2 Page 15 evidence, the home was monitoring service users health whilst staying at the home and of visits by the service users to a chiropodist and a nurse. Relatives who returned the survey stated the home generally kept them well informed. Staff were aware of the how the layout of the building caused difficulties in the promotion of privacy and dignity for service users. They were keen for changes to be made so they could maintain the service users dignity. The acting manager explained of those service users who are able to selfmedicate a locked box is provided in their own room. However, for those service users who are unable to self-medicate the unit has a medication system in place and procedures are followed accordingly. Medication was stored securely and medication records were accurately maintained. There are currently no service users that require controlled drugs. Staff explained they had carried out Safe Handling of Medication training. The Commission considers Standards 22, and 23 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 22 and 23. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users have access to an effective complaints procedure and are protected from abuse. EVIDENCE: Service users spoken with were comfortable in making any dissatisfaction known to the staff. There is a book in the dining room for service users to write any comments they may have. Relatives also stated they found the management approachable. The complaints bureau at North Yorkshire County Council or the acting manager responds to complaints. The acting manager and operations manager also review any complaints on a monthly basis to identify any similarities. The home has received two complaints since the previous inspection; about the environment and care, both have been resolved. Policies and procedures are available to ensure that service users are protected from abuse. The manager was aware of the procedure and said this was regularly reinforced with staff. Staff spoken with were also aware of the actions to be taken if they suspected abuse, and had received training as part of their national vocational qualification course. Shared spaces complement and supplement service users’ individual rooms. Service users have the specialist equipment they require to maximise their independence. The home is clean and hygienic. The Commission considers Standards 24, and 30 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 24 and 30. Quality in this outcome area is adequate. This judgement has been made using available evidence including a visit to this service. Although the staff at the home ensure the service users stay in a safe and clean environment, the registered provider needs to make sure it is maintained until the intended move. EVIDENCE: The home is located in a mid-terrace Victorian property within walking distance of the centre of Skipton. There is a lounge, dining room, kitchen and bedroom on the ground floor and three bedrooms on the first. There are long-standing issues related to the suitability of the premises, which do not promote the privacy or dignity of the service user. Theses are due to the layout, poor access, limited storage and the lack of specialist facilities such as tracking. With this in mind there is a plan to relocate the home to purpose built accommodation and service users and staff have taken part in the design of the new building.
Gargrave Road (24) - North Yorkshire County Council DS0000034639.V327650.R01.S.doc Version 5.2 Page 18 The staff explained they have been waiting for this to progress, and due to the move maintenance had fallen behind, one bedroom had handles missing from a chest of drawers, also the shower room on the first floor had mould on the ceiling and the furnishings looked old. However, the acting manager explained she had just received £10,000 that she would be using to refurbish the home. The home was generally clean and free from odours. However one of the bedrooms had a offensive odour, which the acting manager and staff were aware of and had attempted to resolve. A range of checks is completed on a regular basis to make sure that the house is safe and secure. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 32, 34 and 35. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users are supported by a enthusiastic and skilled staff who are able to meet the needs of the service users. EVIDENCE: Observation of staff and service users and confirmation by relatives established a competent staff team that supports service users. Comments made during conversation with service users and relatives were ‘lovely, their kind ’, ‘like the staff’, ‘they listen to what you have to say’ ‘staff are very helpful’. Service users are allocated a named worker, who works closely with the service user and keeps in contact when they are not staying at the home. Staff also felt this helps the service users feel comfortable in expressing their preferences. The home has an established staff team, who said they had all undergone mandatory training, and most had completed a National Vocational Qualification in care at level two or above. All staff have recently completed equal opportunity and diversity training.
Gargrave Road (24) - North Yorkshire County Council DS0000034639.V327650.R01.S.doc Version 5.2 Page 20 However, the records showed that for some the mandatory training had not been up dated and a member of relief staff had not received any specific mobility assistance training. The acting manager explained management were aware of this and had now put into place a new system, which would make sure all mandatory training was kept up to date for all staff. The number of staff on duty in the home depends upon the number of service users staying, staff confirmed generally there are enough staff to meet the needs of the service users. All the surveys returned by relatives also stated there were sufficient staff. The acting manager explained all the records are held centrally in Harrogate and the home had attempted to create a second copy. Three staff files were examined. For staff recently appointed all the records were available. However, for those who had worked for the home for a number of years some of the copies records were absent. The acting manager explained all criminal bureau record checks (CRB) checks would now be carried out on a three-year cycle. Staff and records confirmed supervisions and staff meetings occur regularly and notes are taken. Service users benefit from competent and accountable management of the service. The Commission considers Standards 37, 39, and 42 the key standards to be inspected. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 37, 39 and 42. Quality in this outcome area is good. This judgement has been made using available evidence including a visit to this service. Service users best interests are promoted and reflected in the service provided. EVIDENCE: There is an acting manager at the home who is completing her National Vocational(NVQ) in Management award. Who receives regular supervision by the operational manager, who was previously the registered manager of the home. Despite the recent change of management staff said, they felt supported by the management of the home. Gargrave Road (24) - North Yorkshire County Council DS0000034639.V327650.R01.S.doc Version 5.2 Page 22 Staff also commented how they found it beneficial for the acting manager to work providing direct care for service users on a regular basis, they believed this kept her in touch with the service user needs. The home does not send out annual questionnaires to service users about the service, they rely instead on information gained from the service user reviews. The acting manager explained service users were involved in the improvement plan on a individual basis, where the key worker would ask the service user for there views, there were no records to support this. However, there was evidence service users were consulted and their views were listened to about the new premises. At the previous inspection, a requirement was made for the electrical certificate to be updated and for water temperature to be checked regularly. Information provided by the acting manager and a sample of health and safety records, checked evidenced the home protects the health and safety of service users. Also during the site visit maintenance were checking water temperature in the home. The acting manager and staff confirmed regular fire training is carried out, however at during the site visit a upstairs office door was held open with a wedge, this practice must cease as it may put people at risk in the event of a fire. Bedrails were in use and a risk assessment had not been carried out, the acting manager agreed to immediately carry out a risk assessment and make sure they were regularly maintained and checked.
