It concentrates exclusively on providing a respite service for people living in the Craven area. There are 35 service users currently using the project. The home is located in a mid-terrace late Victorian property within walking distance of the centre of Skipton. There is a wide range of local amenities close by. This is an overview of what the inspector found during the inspection. This inspection was carried out on 17 May 2005. It was unannounced and took 4 hours to complete. The majority of time was taken up in discussion with the service user in the home at the time of the inspection and with the staff on duty. Both the Registered Manager and deputy manager visited the home during the inspection and made a contribution to the inspection. Service user files and were examined. A check was made as to whether the home had complied with requirements and recommendations from the last inspection. Verbal and written feedback was given at the end of the inspection. There are long-standing problems with regard to the suitability of the premises. This includes problems of access, limited storage space and a lack of specialist facilities such as tracking and sensory equipment. There are plans to move to purpose built premises by June 2006 which should resolve the issues. In order to ensure that medicines continue to be administered safely staff should complete training.

Prospective service users have an opportunity to visit and to “test drive” the home. Each service user has an individual written contract or statement of terms and conditions with the home. The Commission consider Standard 2 the key standard to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 2 Prospective service users are assured that the service will meet their needs. EVIDENCE: Each prospective service user is enabled to make a decision about spending time at 24 Gargrave Road. There is an introduction programme that includes visits to the home, the opportunity to have meals with other service users and to participate in social activities. The introductory period is not fixed but is tailored to suit individuals. The above was borne out in the files of two service users recently referred for respite stays. There were comments in the files indicating that one of the service users and his main carer had been consulted about the placement. Service users know that information about them is handled appropriately, and that their confidences are kept The Commission considers Standards 6, 7 and 9 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 6, 7, & 9 Service users make decisions through the care-planning process about how they are supported and enabled to achieve the goals they have set themselves. EVIDENCE: Case files contain care management plans and service user plans which identify individual service users’ accomplishments and the areas where they need support. This enables staff to plan each day with service users, making sure there are enough staff on duty. The staffing arrangements are sufficiently flexible to allow staffing to be adjusted to meet the needs of service users in the home on a day-to-day basis. There is evidence in the service user plans that service users have contributed their own ideas and made choices about the sorts of things they want to achieve during their time in the home. Each file contains an assessment of risk and is clear about activities outside the home which service users can undertake on their own. The service user in the home at the time of the inspection had not made his mind up about what he wanted to do for the evening but was talking to staff about the options available.

Service users are offered a healthy diet and enjoy their meals and mealtimes. The Commission considers Standards 12, 13, 15, 16 and 17 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12, 13, 15 - 17 Service users have a range of appropriate options available for personal development and social contact. EVIDENCE: The service user in the home at the time of the inspection had a timetable of weekly activities in which he could become involved. This has been drawn up by services outside the home. All service users attending the home live with family or other main carers and the activities in which they are involved in the home represent only a small part of the overall arrangements for personal development and education. Service users have the opportunity to attend the local college, and a variety of supported work based day services. The staff at Gargrave Road assist service users in accessing these facilities. Service users retain, administer and control their own medication where appropriate, and are protected by the home’s policies and procedures for dealing with medicines. The ageing, illness and death of a service user are handled with respect and as the individual would wish. The Commission considers Standards 18, 19, and 20 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19 &20 Service users health and emotional care needs are met. EVIDENCE: Service Users, their families/main carers and other social and health care professionals retain responsibility for health and social care needs. There is evidence in case files that these areas are addressed appropriately and supported as necessary by 24 Gargrave Road. In the last inspection report a recommendation was made regarding the need for staff to undertake training in the safe handling of medicines. To date this has not taken place. JUDGEMENT – we looked at outcomes for standard(s) 23 Service users are protected as far as possible from abuse. EVIDENCE: Service users are protected from abuse as far as possible. Staff who spoke with the inspector were clear about the reporting arrangements if they had any concerns. They have received training on this subject through compulsory modules in NVQ level 2 and 3 courses. Daily logs are kept in detail which recognises the importance of record keeping in identifying the development of patterns of behaviour, for example, which may give rise to concern. The County Council has procedures for dealing with the protection of vulnerable adults. JUDGEMENT – we looked at outcomes for standard(s) 24 & 30 Service users are provided with a safe and comfortable environment but access to parts of the building is poor. EVIDENCE: The home is in good decorative order throughout, having been painted in January 2005. Communal rooms and bedrooms are comfortably furnished and clean. There are long-standing issues related to the suitability of the premises including poor access, limited storage and the lack of specialist facilities such as tracking and sensory equipment. County Care recognised some time ago that 24 Gargrave Road was not suitable for providing care in line with the principles and values set out in the Statement of Purpose. With this in mind there is a plan to relocate the home to purpose built accommodation by June 2006.
