Conditions of registration: None Date of last inspection 10th September 2004 Brief Description of the Service: Gateford Hill provides older people nursing, residential care for older people, younger disabled nursing unit and a residential unit for younger people. The care home is owned by Ashbourne Healthcare. The care home is divided into separate units: 9 older people nursing, (Milner Suite) 18 younger physically disabled nursing, (Cavendish) 27 residential older people, (Machin) and 12 younger people residential (Lindrick), 66 placements in total. The home as 64 single and one double room, 62 rooms are fitted with en-suite facilities. Each of the units has a lounge, dining room, assisted bathing facilities and kitchen facilities. The home, which has thirty three acres of land is located in a quiet area at the edge of a residential estate in Gateford, Worksop. Access is mainly by car, a regular bus service is available to the perimeter of the housing estate. JUDGEMENT – we looked at outcomes for standard(s) 3,5 Service users can be assured their needs will be met and they may visit the home and draw their own conclusions as to the suitability of Gateford Hill prior to making a decision. EVIDENCE: The manager, registered nurses or the unit managers assess prospective service users prior to them entering the home. The assessment is either performed in the community or within Gateford Hill itself. On occasion two assessments may take place so as to assess how the prospective service user would relate to life within Gateford Hill and to ensure all necessary equipment is available. Service users may visit for the day or have a respite stay. The pre admission assessment covers all the requirements of the standard. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users are supported to take risks as part of an independent lifestyle. EVIDENCE: Various assessment tools are in use to ascertain needs such as manual handling, pressure area care, risk and continence, information from assessments is extracted and placed within the plan of care, which is based upon the daily activities of living. Care plans covered all identified needs and were individualised. Choices, preferences, rights and risks were taken into

Appropriate risk management plans were in place. Information with regards to advocates was evidenced with the plan. Consideration had been given to ensure individuality is maintained and a personal life story was available to enable this. There was evidence of specialist services being accessed, appropriate equipment in place and service users receiving NHS entitlements. Staff spoken with were able to discuss service users needs, core values and principles. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. JUDGEMENT – we looked at outcomes for standard(s) 12,13,14 Service users are facilitated in experiencing a lifestyle that suits their needs and take part in appropriate activities and development opportunities. Contact with relevant others is maintained and supported and service users are helped to exercise choice and control over their lives. EVIDENCE: Service users are free to leave the home and return as they wish, two service users spoken with were able to substantiate this. Staff spoken with discussed the issues of rights, compromise and negotiation with regards to risk taking and further sources they may liaise with if required. Visitors are welcomed in to the home and there are no restrictions imposed. Visitors can be received in

Privacy is given at all times and evidence was available to substantiate that when a service user’s girlfriend visits he is satisfied with arrangements in place. There is a mini bus available to facilitate visits out and service users also use dial a ride if required. There are many daily activities available within the home; service users spoken with expressed there needs with regards to this were met. One service user assists staff in cleaning the dining room after meals, on speaking with her she expressed she enjoyed doing this. There is a link with the local college educational facility where one service user attends. Three service users have their own computer and there is also one available within the home for service users to use. No service user currently has a job, however the unit manager stated that this would be facilitated if required. Service users are able to bring in their own possessions should they wish. EVIDENCE: There is clear and simple complaints procedure on display and available in all service users rooms. The policy demonstrates appropriate guidelines and action to take. EVIDENCE: Gateford house is well maintained both internally and externally, decoration is of a good standard and a routine maintenance programme is evident. The home is clean, pleasant and hygienic in all areas and sufficient staff are employed to sustain this. EVIDENCE: All staff undertake induction training and have individual training files. Both the trainer and staff member sign the induction training; the programme is in depth covering all requirements. There was good evidence available to demonstrate staff training is taking place, training statistics are kept and sent to regional office. Staff spoken with stated there is opportunity for development and a good standard of training is offered. Service users spoke highly of staff and expressed their needs were met. These were evidenced and were satisfactory. Internal audits are also carried out for all units; following this an action plan is devised to work towards improvements if required. Service users or relatives are given a routine questionnaire, if concerns are noted a service users meeting is held. All maintenance and servicing checks and related health and safety documentation, such as fire system testing and accidents records were observed and were satisfactory.
