Conditions of registration: Date of last inspection 23.12.04 Brief Description of the Service: The home is owned and run by Methodist Homes for the Aged a national voluntary organisation. Glen Rosa and Kitwood House is situated on Grove Road in Ilkley, close to the town centre, close to public transport. The house is situated in large, well-maintained grounds and gardens that provide for ample car parking. The main builidng is a period property that has been converted and extended. The original building has been converted to provide care to 33 older people. The extension, Kitwood House has been built for the purpose of providing care to 14 older people with dementia. There is ramped access to the side of the building to provide access for those using mobility aids. This is an overview of what the inspector found during the inspection. This inspection was unannounced and undertaken by one inspector on 13 July 2005 between 10:15 and 15:45. The registered manager was on duty. The following methods were used. 8 service users provided feedback about the care and services. Several service users invited the inspector to look at their bedroom. The meal provided at lunch was seen and food quality discussed with these residents. Two visitors provided feedback in respect of the care home. CSCI comments cards were provided and left in the home for completion by service users and visitors. These were freely accessible in the entrance. Two staff, the chef and manager were involved in the inspection by providing feedback or documentation as requested. The general staffing levels, supervision and training were discussed with the manager and records inspected. Four case records and two staff personnel files were inspected. The content of the statement of purpose/service user guide was inspected. The building was inspected. What the service does well:
The environment is a great feature and strength of this care home. The environmental standards are mostly exceeded. The service users changing needs are considered in developing and upgrading the building. The aims, policies and practices ensure person centred care practices and a service that is based on each individual’s expectations. The staff team ensures that the service users right to self-determination and privacy are protected. Residents spoken to were highly satisfied with the environment and practices. The level of ownership in respect of bedrooms is high and residents generally hold their own keys and rooms seen were used as bed-sits that had been personalised by the individual to a very high degree. Staff only enter with the permission of the service user or in case of emergency. Staff within Kitwood House spent much time in conversation with the residents. The residents are free to use any area of Kitwood House including a

Residents are supported by staff outside of Kitwood House and are enabled to go out and about. Staff were confident, skilled and discreet when assisting and supporting service users. The intended outcomes for Standards 1 – 6 are: 1. 2. 3. 4. 5. 6. Prospective service users have the information they need to make an informed choice about where to live. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1, 2, 3 & 6 The statement of purpose/service user guide needs amendment and a copy provided to prospective residents and their relatives. The residents or their representative are provided with a copy of the terms and conditions. Appropriate pre admission assessments ensure that the needs and expectations are met. EVIDENCE: The content of the statement of purpose/service user guide was checked and the content audited against the NMS and Regulations to ensure that all the details required being included are provided. The document is in need of amendment in respect of the registered manager as there has been a change since writing this. The registered managers experience and qualifications must be included. Point 3.13 must be amended in respect of this authority and changed to Commission for Social Care Inspection and the appropriate local contact address and telephone number provided. The statement of purpose must set out clearly the physical environment standards. The statement of purpose gives otherwise very good and clear information about the policies aims and practices at the home as well the expectations of the service users. Copies of the statement of purpose must be provided to

There was no evidence that this is the case. This document underpins the care to be provided and must be kept under regular review. It is recommended that copies are made available alongside other materials on display and provided. Residents or their relatives are provided with a statement of terms and conditions. Individuals are informed in writing in of changes in fee. Copies are also held on the service users file for their information. Case records seen include the pre-admission assessment. These give good details in respect of the needs and expectations of the new service user. The manager or a trained senior member of staff undertakes this assessment in the prospective service users current situation. The initial assessment underpins the initial care plan. There was good evidence in the documentation that service users and their representatives are involved in the process and their opinion recorded. Residents and their visitors confirmed that they had been involved. The home does not provide intermediate care hence this standard is not applicable. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7, 8,10 & 11 The care plans are service user friendly in format and style and residents and their representatives are involved in developing it. The details describe what staff have to do in meeting the residents needs. Medical needs are met by input from NHS staff. Appropriate care is provided to those who are dying and their final wishes honoured. EVIDENCE: The care plans are based on the ongoing needs assessment and give clear details of how service users needs are to be met by staff. The service users own skill level is well considered to ensure that existing skills are not compromised by too much input from staff. The resident’s expectations form the basis of the plan and the care and services provided. The resident’s emotional and cognitive needs are well addressed within Kitwood House. Triggers for aggression, anxiety or challenging behaviour are investigated and identified and included in the care plan in order to minimise incidents. Staff review the care plans monthly. All residents have a choice of a local GP. Some said that they had been able to keep their previous GP when local. NHS staff provide any nursing input. Service users are regularly assessed in respect of their dental and optical care.

The assessments and care plans identify any concerns with regards of nutrition, manual handling and skin viability. The residents spoken to see their right to privacy was an important aspect that is well considered and a high degree of privacy achieved. Service users were clearly in charge of their own room and most were key holders. Residents generally keep their doors locked and notices are available showing when residents do not wish to be disturbed. Residents said that personal care and treatments are only provided in private and key workers are responsible for assisting with bathing when needed. Residents choose a key worker and may change key worker if they wish. When service users become ill and are deteriorating relatives and friends are encouraged to provide comfort and are able to stay with the service user. The service users last rites are given and their final wishes followed. When residents are unable to leave their room staff keep additional records to show the ongoing contact and care provided. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 13 &15 The visiting arrangements are flexible and suit the needs of visitors and residents. There is a varied diet provided that takes account of good nutritional values and the resident’s needs and preferences. EVIDENCE: Two visitors were spoken to and both felt that high standards were maintained in the home. The visiting arrangements suited them and are seen as very flexible. Visitors felt that they were kept up to date by phone and during visits. Residents are able to go out within a system of risk management. Any restrictions are recorded as part of the assessment and care plan. There is a full time chef employed five days each week and a part time vacancy is being advertised in order to achieve full permanent staffing. Agency cooks currently cover the vacant hours. The manager feels that the standard of food will be more consistent with the expected standards achieved by the full time chef. The meal served for lunch provided for several choices. Residents spoken to said they enjoy the food. Fresh meats, vegetables and fruit are delivered several times each week. Residents discuss menu options and food quality during service user meetings held periodically. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 Systems seen showed that complaints are recorded, investigated and action taken to make improvements when needed. EVIDENCE: A complaint procedure is displayed and part of the statement of purpose and service user guide. The complaint record showed that all complaints are fully investigated and written feedback provided detailing the findings and outcome. Visitors and residents knew how to make a complaint. Service users have access to safe and comfortable indoor and outdoor communal facilities. Service users have sufficient and suitable lavatories and washing facilities. Service users have the specialist equipment they require to maximise their independence. Service users’ own rooms suit their needs. Service users live in safe, comfortable bedrooms with their own possessions around them. Service users live in safe, comfortable surroundings. The home is clean, pleasant and hygienic. The Commission considers Standards 19 and 26 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 19, 20, 21, 22, 23, 24, 25 & 26 The environmental standards are exceeded in respect of room sizes of bedrooms and communal areas. The grounds and gardens are substantial and the secure garden meets the needs of the service users with dementia. The standard for equipment is exceeded as equipment is provided for residents to continue to undertake self-help skills such as laundry and ironing in their own laundry room. The needs of residents with dementia underpin the design and layout of Kitwood House. Areas seen were kept clean and provided a safe and bright environment. The environment is a great strength of Glen Rosa and Kitwood House. EVIDENCE: Glen Rosa and Kitwood House are located half a mile from Ilkley town centre. The original house, Glen Rosa, is a period property that has been converted. This has many original and period features that provide for many focal points. The bedrooms are situated on the ground and first floors. All are single and provide for an en-suite toilet and many also have a shower. All rooms are

There are various communal rooms that provide for space over and above the requirements. There is a hair dressing room, laundry and several snack making areas that residents are encouraged to use. It is seen as good practice that there is equipment available that ensures that the residents can continue with tasks such as laundry and ironing. The large grounds provide for parking and offer excellent outdoor facilities for the residents including a pond, gazebo, furniture and mature gardens. The residents also enjoy the varied views of the surrounding moors and watching birds and squirrels. Many residents were enjoying the nice weather in the gardens. Kitwood House is situated in an extension that has been designed to meet the needs of service users with dementia. All rooms are large and have an ensuite toilet and shower. Residents are able to access the large secure gardens form several exit routes allowing for free movement. There is a large dining/lounge area and several smaller rooms that are utilised for meetings, activities and religious worship and prayer. There were many photographs displayed within Kitwood House that encourage recall and reminiscence. Family photos prompt residents to remember family and friends. Keys to bedrooms are provided within Kitwood House within a system of risk management and many service users need to have their doors unlocked to enable free access and movement. There is a picture and nameplate on each door that provides visual prompts for the residents with dementia. Kitwood House is spacious and bright. There are several assisted bathrooms that have different equipment providing a choice to service users and meeting the various levels of dependency and need. There are several hoists, a stand aid and handling belts available to staff assisting service users with their mobility needs. Nursing beds were provided to those who needed one. Residents have personalised their own rooms by bringing in furniture, ornaments, pictures and books. The residents like their rooms and said that they are allowed to spend as much or as little time as they wish in it. Residents said that staff would serve meals in service users own rooms if and when preferred. Visitors can be received in communal areas or own room. The kitchen is situated within Glen Rosa. The kitchen is of an industrial style and is fitted with industrial equipment. There are several fridges and freezer available as well as a storeroom. Fresh fruit, vegetables and meat are delivered daily. Good hygiene practices were apparent including recorded cleaning schedules and properly dated containers. A laundry assistant is in charge of the area. The risk assessment was displayed for anyone using the area. Driers are regularly de-fluffed in order to reduce the risk of fire and records kept by staff. Anti-bacterial soaps and paper towels are provided in bathrooms and toilets to encourage adequate hand washing. The home exceeds the standards for hygiene as a washer-disinfector is provided and this is good practice as these are currently only required within care homes providing nursing. The manager was able to confirm that hoist slings are service user specific and was aware that the use by multiple residents could increase the risk that an infection is spread. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission considers Standards 27, 29, and 30 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 28, 29 & 30 The personnel files are orderly, up to date and included the information required. The staff training provided meets current standards. 50 of staff are qualified to NVQ level 2 or above. EVIDENCE: Two recruitment files were inspected. The content showed excellent standards of record keeping and practice. Documents included were application for employment including employment history, two written references, and evidence of CRB clearance prior to employment and interview records. When staff are foreign workers home office clearance and police references from the home country are also obtained. The registered manager has ensured that good practices are fully observed and home office advice followed. The personnel files included any NVQ certificates. The registered manager confirmed that more than 50 of care staff are qualified to NVQ level 2 or above and the level is continuing to increase. Agency staff are at times used to cover for shifts. The registered manager has confirmed with the agency that appropriate recruitment practices including CRB checks are ensured. The registered manager is hoping to continue to reduce the use of agency staff. The personnel files also included training certificates. These showed that the induction training targets are met.

Service users live in a home which is run and managed by a person who is fit to be in charge, of good character and able to discharge his or her responsibilities fully. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 31, 32, 35, 36 & 37 The registered manager is very experienced and undertaking training. She ensures that the home is well run and that residents are the focus of the care and services provided. Good practices are ensured and reflected within an excellent record keeping system. EVIDENCE: The registered manager is an experienced carer with substantial senior and care management experience. She has much experience with people with dementia and was able to discuss current good practice. Susan Sullivan has attended appropriate training and is currently undertaking the NVQ level 4 in care and management. She is hoping to complete in 2006. There are clear lines of responsibility within the home and there is a deputy manager and two assistant managers that assist Susan Sullivan with the management tasks. All continue to be involved in providing hands on care and

Additionally there are several senior care staff and care staff on each shift. The registered manager is supported and supervised by another manager within the organisation and monthly reports on the conduct of the home provided to CSCI. The registered manager ensures that residents are involved in decision-making and ensures that there are regular residents meetings. Residents are invited to discuss concerns, activities and menu planning for example. The personnel files included staff appraisal and supervision records. Training needs are identified during supervision and appropriate training ensured. The residents are provided with keys to their room and a lockable facility within a risk management system. Residents are also able to deposit money for safekeeping. Individual accounts are kept and money held in separate compartment for each individual. Susan Sullivan spoke to residents about aspect of safety during the most recent resident meeting. Written records of all transactions are maintained. The records kept at the home are kept secure in appropriate lockable cabinets within staff areas. Case records are service user friendly and clear in layout. All records are accurate and systems ensure good order and easy access to documents.
