Conditions of registration: 1. The registered provider must at all times employ a suitably experienced and qualified manager who is registered with the NCSC 18th August 2005 Date of last inspection Brief Description of the Service: Glengarry is a care home providing personal care and accommodation for 15 older people. The home has a no smoking commitment. There are 15 single bedrooms, all of which have en-suite facilities. The home is owned by Mr and Mrs Hepwood and managed by Ms Susan Anne Cottell. The home is located in Poulton close to the shops and other facilities. There are 2 communal lounges, one facing the front of the home and one with views across the rear garden both are well maintained and in good decorative order. This is an overview of what the inspector found during the inspection. This home has been inspected against the National Minimum Standards for older people introduced in April 2002. This year, all registered Care Homes were to be inspected at least twice. This inspection was unannounced in that the homeowner, staff and residents were unaware the visit was going to take place on 30/03/06. During the inspection various aspects of care were looked at. In the report there are references to the “tracking process”, this is a method whereby the inspector focuses on a small group of residents. All records relating to these individuals are examined, along with the rooms they occupy in the home. Residents are invited to discuss their experiences of the home with the inspector; this is not to the exclusion of the other residents who contributed in many ways. This inspection included discussion with residents and staff, in addition to viewing the home’s required written information such as policies and procedures about various issues for instance ‘Health and Safety’. The residents written plans of care were viewed for 3 people. The plan of care is a document outlining the needs of the individual resident and how these are to be met. The plans of care cover all aspects of the individual’s life including health, personal care and social activities thereby ensuring people are content in the care they receive. The residents the inspectors spoke with were very happy with life at Glengarry Court. The staff enjoyed their work at Glengarry Court and spoke to the inspector in a professional manner about the residents. What the service does well:
Glengarry Court is a well maintained Care home, which has a very homely atmosphere. The people who live at Glengarry Court are full of praise about the care they receive. The Commission for Social Care Inspection sent written questionnaires to residents, their relatives and friends. In total 9 responses were received and the feedback from them was generally very positive. In the responses received from the residents or their representatives; • • • • • • All respondents said that they were satisfied with the overall care provided all of the time. All respondents said that they liked living at the home all of the time. All respondents said that they felt well cared for all of the time. All the respondents said that their privacy was respected all of the time. All the respondents said that the home provided suitable activities all of the time. All of the respondents said that they liked the food provided most of the time. Additional positive comments were made in writing to the Commission for Social Care Inspection, which are referred to later in the report but included; ‘Very efficient and caring staff’ ‘ ‘Always a very cheerful atmosphere and homely’ ‘Staff are very approachable and pleasant.’ The management team listen to advice offered by the Commission for Social Care Inspection and act upon the advice to ensure the home continues to offer an excellent service. They encourage staff training and team working. The home maintains a fairly consistent staff team and is well managed. What has improved since the last inspection?
The management team continuously strive to maintain their good standard of care and seek ways to further improve. This is evident from the comment cards received by the CSCI, see above. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. The intended outcomes for Standards 7 – 11 are: 7. 8. 9. 10. 11. The service user’s health, personal and social care needs are set out in an individual plan of care. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 8&9 Arrangements to meet the health care needs of the residents are good. Policies/procedures and staff training ensure the residents medication is stored securely and administered safely, resulting in people being healthily cared for. EVIDENCE: 3 Care plans were viewed as part of the ‘tracking process’. These reflected that the health care needs of the residents were identified and how they were being met. Each plan of care includes a record of appointments with/visits from health professionals such as G.P’s; District nurse; opticians; and dentists. The residents are weighed on a monthly basis to monitor any excessive weight gain or loss and take action to identify the cause. Risk assessments form part of the care plan.
Glengarry DS0000009677.V270587.R01.S.doc Version 5.1 Page 11 Each person has a full pre-admission assessment to ensure the home is aware of the individuals needs and are confident that they can be met at Glengarry Court. One resident said ‘I have been here for 8-10weeks and it is home from home. I am now selling my house as I intend to live here permanently.’ The medication administration Records (MAR sheets) were viewed and reflected that medication is signed for at the time of administration. Medication is pre packed into doses by the pharmacist. The medication records have a photo of the resident to ensure medication is given to the person it is prescribed for. The staff administering medication are trained to do so. People in receipt of medication should sign a medication declaration record, stating whether they wish to self medicate or have it administered by the staff. The intended outcomes for Standards 12 - 15 are: 12. 13. 14. 15. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 13 The resident’s benefit from being able to see their family and friends as they wish to. EVIDENCE: The residents care plans reflect that family and friends are made welcome at Glengarry court and the residents spoken with confirmed this. The residents stated that their family can visit at any time and are always made welcome and offered refreshments. Residents who choose are enabled to go shopping in the local community or go out for a meal. A comment card received from one friend of a resident made very positive comments about the home and the kindness of the staff. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 18 The homes policies and procedures are robust and ensure people are protected from abuse. EVIDENCE: The homes policies and procedures are in line with the Department of Health Guidance ‘No Secrets’ and as such ensure that the correct actions would be taken should an allegation of abuse be made. The outer doors of the home have appropriate locks installed to prevent intruders. All staff would normally undergo Criminal Records Bureau clearance prior to commencing employment. Where it was necessary for the person to commence prior to this the homes Manager would consult with then Commission for Social Care Inspection and would obtain a Protection of Vulnerable Adults First clearance prior to the person starting employment. The residents said that they felt safe at Glengarry and that Susan (homes manager) would ensure that they were protected from abuse. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. The Commission consider all the above are key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 29 The recruitment procedure of the home is good and ensures that people are protected from unsavoury characters. The calibre of staff is good. A dedicated staff team cares for the residents. EVIDENCE: The homes policies and procedures in relation to staff recruitment are robust ensuring appropriate people are employed. In addition to two written references being taken up all potential employees have to have Criminal Records Bureau clearance prior to employment in the home. The home is well managed and as a result staff rarely leave. The senior carer on duty during this inspection has worked at the home for 13years. Should it be necessary to have a person commence employment prior to receiving a Criminal Records Bureau clearance this should be in exceptional circumstances and agreed by the Commission for Social Care Inspection , following the home having received a Protection of Vulnerable Adults First check. Service users benefit from the ethos, leadership and management approach of the home. The home is run in the best interests of service users. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): 38 The residents live in a safe environment where their health and welfare is promoted. The residents live in a well managed EVIDENCE: Risk assessments are carried out in relation to health and safety in the home. The building is checked daily and any necessary repairs/maintenance required are recorded. The action taken to rectify the fault is also recorded, thereby ensuring the safety of the home. The fire alarm is sounded on a weekly basis to ensure it is in working order. Staff receive training in relation to Health and Safety. The residents said they feel safe at Glengarry Court.
