Conditions of registration: Date of last inspection 8th February 2005 Brief Description of the Service: Graceland is a care home providing nursing and personal care for older people with mental health needs. It is a two storey purpose built home providing single and double accommodation for 53 Residents; the 31 single bedrooms are a minimum of 10 sq.m. The 11 double bedrooms are a minimum of 16 sq.m. There is a passenger lift giving access to the upper floor. There are a selection of lounges and three dining rooms, one on the first floor and two on the ground floor. This is an overview of what the inspector found during the inspection. The inspection commenced at 10.10 a.m. and concluded at 2.25 p.m. Two Residents, one visitor and three members of staff were spoken to during the inspection. Two other visitors to the home were heard by the Inspector telling the Manager that they didn’t wish to speak to the Inspector, as they were happy with the home. Each service user has a written contract/ statement of terms and conditions with the home. No service user moves into the home without having had his/her needs assessed and been assured that these will be met. Service users and their representatives know that the home they enter will meet their needs. Prospective service users and their relatives and friends have an opportunity to visit and assess the quality, facilities and suitability of the home. Service users assessed and referred solely for intermediate care are helped to maximise their independence and return home. The Commission considers Standards 3 and 6 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 1 and 3 Prospective Residents and their representative have the information they need to make an informed choice about where to live. Residents have their needs assessed prior to their admission to the home. EVIDENCE: The home has a statement of purpose and a Service User Guide, which can be accessed by anyone who wishes to know what facilities and services the home can offer. Residents had been assessed by the appropriate professional prior to being admitted to the home. Service users’ health care needs are fully met. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. The Commission considers Standards 7, 8, 9 and 10 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 7 and 8 The Residents health and personal care needs are recorded appropriately, and their health needs are met. EVIDENCE: The assessments audited contained the required information. Care plans were written once a need had been identified within the assessment process. Risk assessments had been written once a risk had been identified in the assessment process or following an evaluation of a care plan. Care plans are reviewed as stipulated in the care plan. Doctors, District Nurses, Chiropodists etc are requested to visit the Resident as and when necessary. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 12 and 15 Activities take place within the home on a regular basis and appear to be varied and can be on a one to one basis or as a group activity. The Residents receive a wholesome and well balanced diet. EVIDENCE: The home employs an activity organiser. There was a list on the wall in the foyer stating what activity would take each day covering the full month. There was a photo album available which showed the Residents enjoying activities such as baking and outings with the staff. There was a poster on the wall, which had recorded the Residents memories of the Second World War. One Resident told the Inspector how she had enjoyed playing golf when she had been younger but was not up to it any more. On the day of the inspection the lunch was cheese and onion flan or beef casserole with a choice of rice pudding or a yoghurt for dessert. There was also two choices of main meal and dessert for the evening meal. The Inspector asked one Resident if she had enjoyed her lunch and was told yes, this Lady went onto tell the Inspector that all of the staff were her very good friends A visit to the Kitchen showed that it was clean and tidy. There was a good stock of food both fresh and dried. The cook makes her own quiche, cakes and scones. Service users’ legal rights are protected. Service users are protected from abuse. The Commission considers Standards 16 and 18 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for standard(s) 16 and 18 Policies and Procedures are in place for staff to follow should they receive a complaint. The staff have a good knowledge of the action to take should they become aware of abuse within the home. EVIDENCE: There have been no complaints regarding the home in the last twelve months. There is a policy and procedure in place for the staff to follow should they receive a complaint. Policies and procedures are in place in relation to adult protection and prevention of abuse. There is also a policy on whistle blowing and staff spoken to stated that they were aware of these policies and explained the action to take should they witness any form of abuse taking place within the home. The staff has received training in Abuse, this was recorded in their training files and confirmed by the staff that spoke to the Inspector. JUDGEMENT – we looked at outcomes for standard(s) 28 and 30 Staff is being trained to NVQ level 2 which should ensure that they can meet the Residents needs. EVIDENCE: 65 of the care staff have their NVQ Level 2 or 3, the remaining staff are currently studying for their NVQ Level 2 or 3. An audit of six training files showed that induction and training takes place, both mandatory training (fire and manual handling) as well as Resident specific training such as wound care, eye care, nutrition and Elder Abuse. JUDGEMENT – we looked at outcomes for standard(s) 31, 33 and 36 The home is managed by a person who is fit to be in charge. There is a quality audit which is accessible to those who wish to read it. The Staff are appropriately supervised. EVIDENCE: The Manager has the required nursing and management qualification to ensure that the needs of the Residents are fully met. The staff spoke highly of the Manager. There is a quality assurance and quality monitoring system in place within the home. There are comment cards on the reception desk as you enter the home. The audits cover the different departments within the home e.g., Residents care documentation, the kitchen and the environment.
