This inspection was carried out on 26th January 2006.

The inspector made no statutory requirements on the home as a result of this inspection and there were no outstanding actions from the previous inspection report.

What the care home does well

Numbers of care staff are increased according to the dependency of service users. This ensures there is always enough staff to provide quality care. Service users stated, "The staff are lovely." A physiotherapy room was available, which was fully equipped. This allows service users to have physiotherapy on site to prolong independence and wellbeing. Food served was of a high standard and presented well. Special dietary requirements were catered for. Service users stated, " Its excellent here, the food is good". " I am looked after well, the food is delightful, I`m very glad I came here, there is nothing I would change, and I cannot fault it, its excellent here." Privacy was respected; Do not disturb signs were available for each service users bedroom door. A large display telephone was provided in each bedroom. A guest flat was available for service users relatives and friends to rent when visiting.

What has improved since the last inspection?

A learning resource area has been created for staff to utilize. This was stocked with educational material.

What the care home could do better:

The high standards of service and care should be maintained. A system should be sourced to ensure service users who wish to have their bedroom door open could do so.

Age range for Service Users in category (PD) and in receipt of nursing care is 50 years upwards. 4th October 2005 Date of last inspection Brief Description of the Service: Granby Extended Care provides nursing care for up to 41 clients. The service adjoins Granby Court and provides both permanent and respite/convalescence care. The home is situated on three floors and has comfortable lounge and dining areas. The bedrooms have en suite facilities in addition to assisted bathrooms. A physiotherapy room is also available. The unannounced inspection took place over one hour fifty-five minutes. A half days preparation was undertaken prior to the inspection. The registered manager and deputy manager assisted with the inspection process. A tour of the building was undertaken. Staff, service users and a visitor were spoken with. Records examined included staff rotas, activities programme and information within the reception area relating to the home. The policy regarding visiting was also observed. At the end of the inspection a feedback session was undertaken with the registered manager, deputy manager and regulatory inspector. Service users, where appropriate, are responsible for their own medication, and are protected by the home’s policies and procedures for dealing with medicines. Service users feel they are treated with respect and their right to privacy is upheld. Service users are assured that at the time of their death, staff will treat them and their family with care, sensitivity and respect. Service users find the lifestyle experienced in the home matches their expectations and preferences, and satisfies their social, cultural, religious and recreational interests and needs. Service users maintain contact with family/ friends/ representatives and the local community as they wish. Service users are helped to exercise choice and control over their lives. Service users receive a wholesome appealing balanced diet in pleasing surroundings at times convenient to them. The Commission considers all of the above key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standards 12 13 15 Service users interests and needs were met; Food provided was of a high quality, and served within the service users choice of environment. EVIDENCE: The home has an activities programme available to service users. The activities were being provided through the residential unit, the activities co coordinator was due to re commence her duties imminently. Activities would then be available without service users leaving the unit. Care staff had provided activities over the Christmas period upon the unit. They continue to provide activities whenever possible. Service users maintain links with the local community. Visitors were welcome at any time. There was also a guest flat available for relatives who wished to visit service users, but live a long way from Harrogate. This facility could also be utilized when a service users condition was deteriorating and family need to be close at hand to spend quality time with the service user. Information relating to Advocacy services was displayed throughout the building. It was transported a short distance within a hot trolley to a well designed dining area. Service users could eat within their own bedroom, or have their meal in the dining area. Assistance would be given to service users in an unhurried manner. Family and friend could eat with service users if they desired. Breakfast was served to service users in their bedrooms to allow them to rest, if they wished they could go to the dining area. Morning coffee, afternoon tea and supper was provided for service users. Special dietary needs were catered for. The kitchen was inspected, it was clean, tidy and all relevant records pertaining to cleaning schedules, fridge and freezer temperatures were recorded. Records of cooked meat temperatures were also inspected. The chef had a record of service users who required special diets. This included diabetic and puree meals. The food was presented well and looked appetizing. Service users stated, “The food is good”. Service users’ needs are met by the numbers and skill mix of staff. Service users are in safe hands at all times. Service users are supported and protected by the home’s recruitment policy and practices. Staff are trained and competent to do their jobs. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 28 Service users are in safe hands, and are cared for by well-trained staff. EVIDENCE: Induction and foundation Training was provided to care staff, this was well documented. The training officer and the registered manager ensures that care staff complete the induction training within the correct time frame, before signing this training off. The training officer was available on a part time basis to encourage and support care staff to achieve the National Qualification in Care at level 2 or 3. The home has achieved the recommended 50 ratio of staff holding this qualification. Service users are safeguarded by the accounting and financial procedures of the home. Service users’ financial interests are safeguarded. Staff are appropriately supervised. Service users’ rights and best interests are safeguarded by the home’s record keeping, policies and procedures. The health, safety and welfare of service users and staff are promoted and protected. The Commission considers Standards 31, 33, 35 and 38 the key standards to be inspected at least once during a 12 month period. JUDGEMENT – we looked at outcomes for the following standard(s): Standard 38 Service users may be placed at risk due to bedroom doors being held open by inappropriate means. EVIDENCE: This standard was not fully inspected, however during a tour of the building it was noted that one service users bedroom door was held open by a chair. The service user had asked that the door remained open; staff needed to observe her condition easily. The bedroom door had a sign on it stating it was a fire door and should remain closed. The service user therefore may be placed at risk by the fire door being held open by inappropriate means. This was discussed with the registered manager and deputy manager. Advice must be gained from the necessary authorities to ensure a system can be adopted to ensure service users safety is maintained if bedroom doors need to remain open.
